Let’s get real: Healthcare is broken. You know it. I know it. Every clinician who came into this field from a desire to care for others, knows this to be undeniably true. We feel it everyday when we enter our clinics, hospitals, and practices. We feel the burden of time-based productivity metrics, utilization rates, and the expectation to behave like cogs in a giant, soul-crushing machine aimed at extracting revenue from our patients in exchange for “units” of treatment.

Healthcare should be about one thing: PEOPLE! Yet how many clinics, organizations, or healthcare professionals live their lives by the numbers? Metrics & spreadsheets drive most of our healthcare decision makers and administrators. That leaves patients -the people we serve- lost in the mix. Patient’s feel lost, forgotten, and ignored by a system that prioritizes efficiency & productivity over their own personal experiences, priorities, or goals.

It’s time for clinicians to finally stand up and say what we’ve all been thinking for so long: enough is enough!

It’s time for healthcare organizations to commit to serving the individual needs of each unique patient that they are charged with serving.

Digital Health is Failing: From Clinical Outcomes to Return on InvestmentMillions have been invested in digital health with relatively little true value creation in the form of clinical outcomes, population health, or return for investors. In whichever way you measure (economic or clinical) we are leaning towards the reality that digital health is failing. Why? I think it has to do with 2 things: 1) the disconnect between the perceived benefits and value of these solutions and the actual behavioral change required to implement them and 2) the context of today’s healthcare vs. the ideal of healthcare’s future in value-based care. Over the past couple of weeks, I made a LinkedIn post on the topic of the necessary behavioral changes needed to implement digital health solutions and the context of today’s western healthcare industry, where health insurance is conflated with healthcare. Both of those posts took off, with some lively discussion, so I figured it would be worth doing a podcast episode exploring the topic Why Digital Health is Failing.

So, this week, I share some of my thoughts on this topic and a couple of the solutions I see in this problem.

Is it possible to leverage technology and AI to improve Compliance and Certification in Healthcare, specifically in the software and device space?We’ve talked on this show before about the confluence between consumer technology & medical devices/software. With this comes an increasing emphasis on regulatory compliance, FDA certification, and the like. For example, with Remote Therapeutic Monitoring (RTM), providers must use an approved Software as a Medical Device to deliver that service to patients. And, for many of the clients I work with in the healthcare technology & innovation space, the discussion around FDA approval and certification plays a key role in the go to market strategy. And, historically, it required a lot in the way of man hours and financial resources to put together the required literature reviews, clinical evidence, and clinical evaluation reports (CER) for submission to the regulatory body. However, technology could potentially play a roll in streamlining and improving the efficiency required in compliance and certification in healthcare.

Wondering how you can build a Resilient Healthcare Business?So Change Healthcare was apparently hit with another extortion request on Monday (4/8/24). I was talking with a friend of mine the other day who’s involved with healthcare cybersecurity and he was telling me he thinks it’ll be a LONG time until Change is fully recovered from the initial attack in February. What many people outside of healthcare don’t realize is that healthcare is a very cashflow-dependent business. Once the money turns off, many providers simply don’t have the funds required to continue operating. This is especially true in private practice healthcare. While larger healthcare organizations and corporations may have economies of scale and cash reserves to help them through lean times, many privately held healthcare practices don’t. There will be private healthcare practices that go out of business as a result of the Change Healthcare cyber attacks. This bring up the importance of building a resilient healthcare business.

This week, I share are a few lessons learned from this whole Change Healthcare issue. I also share 5 tips or strategies you can employ to help a build a reliant healthcare business. I shared these briefly on a LinkedIn Post, but thought this topic warranted a whole episode.

What we cover in this Episode: 1. It’s important to understand the choke points in your clinic’s cashflow and how to mitigate them * 2. You must be not only profitable, but also have the necessary cash reserves on hand to float operations through the unexpected * 3. Waiting gets you killed. There’s no time to act like a deer in the headlights when something like this happens. I know practice owners who waited too long to begin pivoting and changing EDI enrollments, and they may end up paying the price for it * 4. Having a diversified income stream helps. Private-pay, non-covered services, etc. help offset the crunch caused by disruption in your typical RCM operations * 5. You need to be able to have those money conversations with everyone * Achieving Better Outcomes* by building strong, resilient healthcare businesses

Ever considered the process of Designing Healthcare Spaces?As an OT, I know my way around accessible design. In fact, I spent a good bit of time between 2017-2020 doing a lot of home accessibility assessments as part of a clinical consulting roll. But, there’s a difference between making sure something is physically accessible and designing healthcare spaces. Not only do healthcare spaces require more in the way of codes and regulations, but there are other factors that need to be considered. Things like space, traffic, bathroom locations, etc. all impact the efficiency and effectiveness of a clinic’s space. There’s also budgets that need to be considered.

So, this week, I sit down with Sharon Woodworth, MA from UCSF’s Healthcare Administration & Interprofessional Leadership Program. She’s also a healthcare architect with Harley Ellis Devereaux (HED). We talk about Sharon’s creation of the 5 Point Keystone Star and how it can guide healthcare space design.

Have you ever heard the saying “data is the new oil”?I heard that on a podcast the other day and thought: “True!” The crude oil market is projected to be around $2.9 TRILLION in 2024. That’s a huge market! But, like oil, raw data by itself is mostly useless in the day-to-day lives of most consumers. Unless that data is filtered, refined, and delivered in a useful way, it’s just something that takes up space on the cloud. I’ve said for years that data is a main value driver for many healthcare tech, SaaS companies, and even provider organizations. Data, when leveraged appropriately, provides the opportunity to: 1) Reduce the cost of delivering care (to patients, providers, & payers) 2) Improve clinical outcomes (which immediately helps patients & payers) & 3) Identify & capitalize on areas for improved financial performance (for payers & providers). But again, you need to do something to that data to make it valuable. In fact, I mentioned a few opportunities to do just that on this LinkedIn Post. That’s why I figured I’d do an entire episode with the title Healthcare Data is the New Oil.

Have you recently thought that it’s time to fire an employee?It’s an often said cliche that, “The hardest thing about business is managing people.” And, especially when you’re in the throws of managing team members who aren’t meeting performance expectations, that saying rings true. Managing, disciplining, and even firing employees are all just part of being a business owner and leader. That being said, I recently had a few different conversations with clients and prospects, specifically about the question, “Is it time to fire that employee?” Over the years, I have fired employees and team members for various reasons. Some firing resulted from clear, black and white ethical or procedural violations. However, the question of firing an employee over performance issues pose different challenges altogether. Especially in smaller organizations, like healthcare technology startups or small private healthcare practices, teams form close relationships. The thought of firing someone who you’ve come to form a personal relationship with brings up anxiety and other emotions around what this change in work arrangement will do to the personal relationships. Then, there’s also the question of whether or not it’s even justifiable to fire an employee over this specific performance issue comes up.

So, this week, I share some of my own thoughts, frameworks, and resources on the topic of managing team members, performance expectations, and even the decision to terminate an employee. I share some books and resources that I’ve used over the years to navigate some of these often difficult management issues.

Book Review and Commentary: The Art of Learning and Healthcare InnovationEvery year, I try to read 1-2 books a month. Most of the time, those books come from outside the healthcare world; business books, personal/spiritual growth books, and books on leadership & learning. Sometimes, I read books explicitly about healthcare, like Called to Care; but often, I like to read and pull insights from other industries. Just because these books don’t explicitly address healthcare, they may have some nuggets that we can apply to our profession and industry. When I read The Art of Learning earlier this month, I pulled out some ideas that I thought were applicable to healthcare innovation, go to market strategy, and addressing barriers to change in healthcare.

So, this week, I’m deviating from the normal format of the show and diving into a book review and commentary of sorts. I cover the main points or insights I gleaned from The Art of Learning and then discuss how I see that they apply to healthcare innovation. How do you successfully implement a Remote Therapeutic Monitoring Program in an Outpatient Clinic?If you’ve been following me for a while, you know that I’m a believer in hybrid healthcare, leveraging technology to innovate the way clinicians deliver services. Recent changes from CMS, creating new CPT codes, and technology now allow for Remote Therapeutic Monitoring. Yet, simply having the ability to implement virtual healthcare service delivery doesn’t guarantee success. What are three key actions that private healthcare practice owners that lead to success?For many clinicians turned private healthcare practice owners, challenges present themselves when it comes to stepping into the role of business owner vs. treating clinician. For those of us who started our healthcare careers as clinicians, it’s hard to separate from the identity of clinician. We go into healthcare from a desire to help and serve people. Ou clinical education and technical train us prepare us for the role of healer. But, opening up a private practice (or even starting a technology or digital health company) pushes us into the “business owner” role. And, unfortunately, this fact fails to receive proper time in most clinical programs.

What do you need to know about professional liability insurance in private practice healthcare?Now most clinicians I know treat professional liability insurance in private practice healthcare as one thing in a list of many pressing to-dos. For many clinicians, a quick internet search for insurance providers, a quick scan to find the cheapest rate, and an email reminder to renew next year make up the extent of their investigation (and hopefully use) of professional liability insurance. But, for something that could potentially protect your professional license, and career, perhaps there’s more that needs to be discussed. Now, I, like many people, begin to stare off into the distance when talk of insurance comes up. The last thing I want to do is sit through some sales pitch about why this product is better than that one….blah blah blah.

How primary care providers are innovating in healthcare through the direct primary care practice modelAs I discuss in the book Better Outcomes: A Guide to Humanizing Healthcare, many healthcare providers reach the point of burnout working in today’s healthcare system. High volume organizations, based on a fee for service revenue model push clinicians to see an ever-increasing number of patients each day. This leads to less direct, or face to face, time between clinicians and patients. It also means that many of our clinics and health systems fall into a rut of “slapping bandaids” on problems, rather than addressing root causes. If you don’t have enough time to dig deep, you simply can’t get to the heart of some issues like chronic disease management, healthy lifestyle habits, etc. That has driven some doctors to consider a Direct Primary Care practice model.

In this episode, I sit with Chris Habig from Freedom Healthworks, a company who’s mission is to help start as many Direct Primary Care Practice’s as possible across the US. What is the role in Entrepreneurship in Healthcare Innovation?As clinicians-turned business owners, we end up needing to figure out a lot of things on the fly. Most healthcare education programs focus most (if not all) of their energy and resources on clinical training and technical knowledge. And that makes sense. You need to have the skills and knowledge necessary to deliver clinical services when you graduate the program. However, this leaves most clinicians with a huge knowledge gap on the business side of things. Systems development, operations, HR and people management, which are viewed as an afterthought in school, take center stage when that clinician decides to open up a practice. And that can be overwhelming. However, it can also be rewarding personally, professionally, and even to the industry as a whole. As I’m a big fan of saying, I believe entrepreneurship is the mechanism by which innovation will most effectively occur in healthcare.

I found it interesting that much of the questions they had about these practices were not easily answered by the practices themselves. Either they had biased views about the potential risks to their businesses or they simply didn’t easily have the data required to answer the question on hand. I figured I’d share the 4 common Questions that Potential Buyers Have About Your Healthcare Practice and give you my take on them.

Hiring in Healthcare Practice: How to Think About Bringing on New Team MembersClinicians rarely get taught in their clinical training about hiring in healthcare, managing people, or even firing people. I remember the first time I had to fire someone. I spend the entire night beforehand with my stomach in knots. Part of that is understandable. It’s a difficult conversation. But, the main reason for my discomfort stemmed from my lack of training and preparation to be a good manager or leader in the first place. Good management, even if it involves firing someone, helps you sleep at night, because you know that you’ve done everything the right way. The same can be said for hiring and onboarding new team members in your healthcare practice. Things like clear expectations, defined goals and benchmarks, and objectives help ensure that new team members and the practice as a whole is better off after bringing that new team member on board.

In this week’s episode, I share some thoughts on how to think about hiring in healthcare, especially in private or independent practices. I’m calling it “reinvent your practice one hire at a time”. This comes on the heels of me having to replace a team member over at my practice ProActive Rehabilitation & Wellness. Basically, when it comes to the opportunities provided by hiring new team members, I think about it in 5 main parts: 1) correcting hiring mistakes, 2) upgrading the position, 3) raising up leaders within your organization, 4) ensuring mission, vision, and values fit, and 5) being a good leader yourself.

What is an Ecosystem Merger? How to use relationships to grow your healthcare practice.As clinicians, building relationships with patients is part of of our DNA. However, when it comes to generating referral partnerships, we sometime transform into beggars, coming with our hand out, hoping we get a few referrals out of a meeting, or lunch, or in-service. We focus on what we want to get out of the transactional relationship, rather than on building a real relationship with that referral partner. Enter what Andy Zapata calls the Ecosystem Merger. It’s a way to build real relationships through shared experiences, both with patients and with referral partners.

Andy Zapata is an Occupational Therapist and CEO of Physical Therapy Now, one of the fastest growing outpatient physical therapy franchises in the US. He’s an expert and putting effective marketing systems in place that grow business through relationships with what he calls Ecosystem Mergers. He used these systems to grow his first practice before selling it. Then, he leveraged the same systems to buy and sell nearly 40 other outpatient physical therapy clinics before starting Physical Therapy Now. I found his concept of an ecosystem merger interesting and insightful, especially when it comes to healthcare practice growth.

How can we leverage creative work, including graphic design, for knowledge translation?We’ve talked about knowledge translation on the show before. But I feel that it’s a topic that could use some more coverage. And I’m not alone. Many organizations, institutions, and companies invest money every year to fund studies to quicken the pace of knowledge translation. Unfortunately it still takes many years for current research evidence to become regularly implemented in healthcare treatments. I say it all the time, but clinicians should see themselves as knowledge translators. We bring value to healthcare by taking the bast amount of technical and boring (or nerdy) information in our specialty area and bring that to bear of our patient’s unique situation. This involves not only being up to date with the most recent evidence and research, but also communicating it effectively to patients and other stakeholders. And that is where something like graphic design comes into play.

Some clinicians leverage technology and a bit of creativity, including graphic design, to effectively communicate this research knowledge to their patients, and to other clinicians as well. My guest this week, Katie Caspero from OT Graphically is doing just that. Her company develops infographics and other graphic representations of current best practice evidence and research both to guide clinicians and to act as patient education aids and materials.

What steps can we take in healthcare to reduce cancellations?As I’ve written, spoken, and worked with clients about, cancellations cause your practice to bleed revenue and profitability. I’ve worked with some private therapy practices that have cancellation rates pushing 35-40%! So, understandably so, many private therapy practices spend a lot of time and energy on cancellation reduction. They text patients, call patients, email patients, send smoke signals or homing pigeons…well, maybe not those last two, but you get the point. Yet, despite all of this activity, they continue to experience high cancellation and no-show rates. Why? A lot of times, it’s because they’ve failed to recognize one key piece in the patient onboarding process.

Well, this week, I’m sharing an interview I did for Paul Wright from Health Business Profits for a program he was putting together about reducing cancellations at private physical therapy practices. How can medical virtual assistants streamline care in private practice healthcare?When it comes to administrative tasks and knowledge work, virtual and remote workers provide advantages in that they don’t require office space, may work more flexible hours, and can help streamline workflows and processes. However, despite these benefits, many private healthcare practices (like physical therapy practices and physician groups) have reservations about using medical virtual assistants in their practices. Perhaps they’ve had poor experiences in the past. Perhaps they don’t like the idea of the using a “call center” approach to providing care.

Well, my guest this week, Dr. Steven Kupferman from MedVA, developed a solution to some of these reservations that clinicians and practices may have regarding remote administrative work in their practices. MedVA helps private healthcare practices source, hire, and train remote medical virtual assistants who can provide high quality administrative work at a relatively low cost when compared to onsite and in-office staff.

He has experience in a variety of rehab settings, working with patients recovering from a variety of injuries and surgeries. Rafi has worked in a variety of settings, from orthopedic and musculoskeletal rehabilitation, to academia, and even healthcare consulting. At Charlie Norwood VA Medical Center, he as the lead clinician and clinical education coordinator for the outpatient specialty rehab program. In this role, he treated many veterans with chronic pain and helped to establish an interdisciplinary pain management program. He has worked on projects ranging from patient engagement initiatives to marketing communication campaigns to a multi million dollar project assisting the State of Georgia’s Department of Behavioral Health and Developmental Disabilities transition individuals out of state institutions to community residences. His work on Telehealth has been discussed in Forbes. He also has experience as a core faculty member at Augusta University’s Occupational Therapy Program, as a Licensed Board Member on the GA State OT Board, and he serves on the Board of Directors for NBCOT.
