This episode of PopHealth Week features Caitlin Donovan, Global Head of Uber Health. The Uber Health platform helps improve patient care by enabling healthcare organizations to arrange rides and services on behalf of others by using a centralized, easy-to-use dashboard or an API. We discuss their role in the healthcare industry, being a new player that brings innovative solutions to address patients’ needs with the technical processes they already know and manage well. Just like their well-known service, Uber Health counts on independent earners that can transport patients or deliver medications to them, an option that can benefit people like seniors, patients with complex chronic conditions, and employees who work for self-insured employers. Caitlin discusses Uber Health’s approach to overcoming care barriers and addressing patients’ social determinants of health by designing benefits for them and aligning incentives across healthcare. She also speaks about the three different ways they help employers facilitate access to care for their employees, and why it’s important to scale solutions while still meeting individual needs.

There’s an unspoken assumption when you go to see a doctor: the doctor knows your medical story and is making decisions based on that story. But the reality frequently falls short. Medical records vanish when we switch doctors. Critical details of life-saving treatment plans get lost in muddled electronic charts. The doctors we see change according to specialty, hospital shifts, or an insurer’s whims.

Stanford physician Ilana Yurkiewicz calls this fragmentation, and, she reveals, it’s the central failure of health care today. In this gripping narrative from medicine’s front lines, she shows how a system that doesn’t talk to itself forces doctors, patients, and their loved ones to go to heroic lengths to bridge the gaps. With lives at stake and little other choice, we all do so―but the system is hanging by a thread, and we need better solutions. Radiantly humane, empowering, and ultimately hopeful, Fragmented is a prescription for what really needs fixing in modern medicine.

On PopHealth Week we contiune to discuss topline COVID-19 pandemic news which unfortunately shows no sign of slowing. In fact positivity rates, hospital admissions, deaths are all accelerating, a growing pool of 'long haulers' report a range of long term health consequences as part of their recovery, while a vocial minority downplay or discount the threat to the public at large, ie, 'much to do about nothing'.

On today's edition of our COVID-19 update series we dive a little deeper into some of the headlines from the COVID-19 dashboard to the current state of vaccine development and comment on the likely agenda of the soon to be installed Biden/Harris administration.

So first up is the U.S. COVID-19 dashboard where we've passed the grim milestone of 10.5 mil cases with death counts just shy of 1/4 million 240,857.

Most troubling is positivity rates and hospital admissions are all trending up putting health systems are at increased risk in terms of both capacity (beds) & staffing.

In the second installment of PopHealth Week's Covid-19 update (listen to first here), my guests included Nick van Terheyden, MD (@drnic1), Fred Goldstein, MS (@fsgoldstein) and Gregg Masters MPH (@2healthguru).

We provide an update on the Covid-19 pandemic including a novel approach to safe re-opens for business and educational institutions.

We also discuss the global and US specific stats including confirmed cases, deaths and where we stand on regional and national geo-cluster containment, and mitigation efforts including an update on the treatment and prevention (vaccine) horizon.

Dashboard via Center for Systems Science and Engineering at Johns Hopkins University:

Global Total Confirmed (as of 8/11/20)

Cases: 18,585,774

Deaths: 701,665

United States Total Confirmed

Cases: 4,775,621 25.6% share

Deaths: 156,906 22.3% share

This amidst a Covid-19 ‘balancing act’ factoring in the opposing forces of: Outcome 1: Stop the pandemic (get R0 below 1.0 via usual public health measures to contain or mitigate spread).Outcome 2: Keep economy going (nobody working takes its toll on health and wellbeing)Outcome 3: Protect the hospital system from overwhelm (flatten the curve, our clinicians are exhausted). We've a long way to go to see how this 'novel' coroniavirus chooses ultimately to run its course be be confronted by either effective treatment and/or prevention via one or more vaccines.

Stay tuned!

On This Week in Health Innovation my guest is Charles Gaba the publisher of 'ACA Sign-Ups' a go to blog tracking the launch and continued operations of health plan offerings associated with the Affordable Care Act (ACA). Charles weighs in on the current stat of the ACA and re-emerging conversations on single payer as well as the general climate for health policies driving U.S. healthcare reform. For his latest, check out: 'A REALLY DEEP DIVE into the updated "Medicare for America" bill'.

In 2013, when neither the government nor the mainstream media provided consistent, reliable reporting of enrollment data for the Affordable Care Act, Charles Gaba launched ACASignups.net  as a hobby, using crowdsourcing to track enrollments in real time.

His work soon caught the attention of major media outlets, and has been cited and used as a media resource ever since spanning the ideological spectrum including the Washington Post, Forbes, Bloomberg News, Vox.com, MSNBC, the New Republic, USA Today, the CATO Institute, National Review Online and The New York Times, as well as prominent medical journals including the New England Journal of Medicine and The Lancet.

In this episode Jane reveals the insights reflected in her new book 'HealthConsuming: From Health Consumer to Health Citizen' and unbundles the seemingly subtle but profound difference between 'consumption' and prident navigation and access of the health system as a 'citizen' with rights and obligations.

We'll discuss Jane's thesis of how health consumers in the U.S. might or can emerge as 'health citizens' where healthcare is a civil right, with privacy protections for personal health information (PHI), and the digital health innovations supporting a healthcare everywhere (and for everyone) model.

On PopHealth Week our guest on PopHealth Week is David Klebonis, Chief Operating Officer of Palm Beach ACO.

High performing and low revenue ACO Chief Operating Office David Klebonis weighs in on accountable care and Palm Beach ACOs strategic glidepath

David Klebonis serves as Chief Operating Officer, for both the Palm Beach Accountable Care Organization and the South Florida Accountable Care Organization'.

'Recruited as part of the original executive team, David helped lead PBACO and its affiliates to save Medicare $45.2M in performance year 1. In performance year 2, PBACO increased savings by an additional 9.8% per beneficiary while improving quality by an average of 13.5% per measure. He currently leads ACO operations including government rules and opportunities, network development, clinical programs and outcome based contracts. Prior to joining PBACO, David was president of Medical Insight Partners, an EHR reseller and healthcare business and technology consulting company. His team specialized in helping physicians maximize their income through implementing technology, achieving government and payer incentives and optimizing workflow.'

More about Palm Beach ACO (PBACO)

PBACO is a wholly physician-owned and operated organization. According to the Centers for Medicare and Medicaid Services entities like PBACO perform better (both cost reduction and care of care) than 'institiionally led' ACOs where a hospital or health system serves as parent or general partner of enterprise directing the mission of the organization. Recently characterized a 'low revenue' vs. 'high revenue' performaing participants in the ACO program whether as MSSP, NextGen or Advanced Payment Models, physician driven ACOs seem to be outperforming the group.

This was promoted to "convene a growing community of innovation leaders to spur industry-wide knowledge sharing. The event will be the first in a series of discussions that explore the ways that innovation can drive systematic improvement of the health care system.'

In this segment, Richard Gilfillan, MD, acting director of the ACA enabled CMS Innovation Center aka CMMI, who currently serves as Chief Executive Officer of Trinity Health, a $17.6 billion Catholic health system that serves communities in 22 states.  Dr. Gilfillan weighed in with his insights on real 'innovation' for the healthcare ecosystem - which he emphasized is not the next new shiney thing whether medical device, drug, platform or app, but a business model that makes 'sustainable sense' by advancing the three part triple aim - better experience of care, better outcomes at lower per capita costs.

Wednesday September 20th our special guest was Mike Biselli the visionary founder of Denver based Catalyst HTI.

More about Mike:

After being named 1st Team All PAC-10 Kicker in 1999 and playing in the Rose Bowl with the Stanford Cardinal the following year – all while earning his BA and MA – Mike Biselli hung up his cleats and brought his unwavering determination and collaborative spirit to the field of healthcare. During his tenure with several industry-leading medical device companies in Nevada and Colorado, Mike quickly recognized the potential of digital health technology to significantly improve medical outcomes, and decided to join the growing movement of entrepreneurs, technologists, and clinicians who were reimagining healthcare through innovation. MedPassage, a digital health startup that connected the buyers and sellers of medical implants via an online platform, represented his first attempt at using technology to address the US healthcare system’s pervasive inefficiencies. After a private equity group acquired MedPassage in 2013, Mike began collaborating with a wide range of individuals and organizations dedicated to accelerating the current transformation of the US healthcare industry through digital health innovation.

As Board Member of Prime Health and a Senior Advisor to 10.10.10 that Mike witnessed firsthand the tremendous power a determined community of clinicians, executives, technologists, entrepreneurs, academics, and investors could wield. Realizing just how much more could be accomplished if that power were harnessed, Mike began developing the industry integrator concept, a completely new feature of the innovation economy that would allow the healthcare industry to be integrated at the point of innovation by housing an entire health-tech ecosystem in a single location.
