This two part episode explores:

  1. Using the SPIKES model as a useful tool to format conversations around the Autism evaluation and eligibility process within the educational setting
  2. Systems-wide changes SLPs need to support families through this process

As it stands now, the Autism evaluation process in the preschool and school-age population is overwhelming for parents to navigate. What doesn't help? As SLPs, graduate school didn't prepare many of us for these conversations, and many of us don't have the supports we need at work to truly spend the time we need educating and supporting families through this process.

This episode provides a framework for structuring conversations with families (SPIKES protocol) with examples tailored towards the educational eligibility process, and ideas for how workplaces can better support SLPs through the pre-assessment, evaluation, and eligibility process so we (as SLPs) have the time and resources we need to better support families through this evaluation and eligibility process.

As speech language pathologists, many of us got into this work to help people. This can make it difficult when we receive criticism and feedback - especially from the families of the students we work with.

In this episode, you’ll learn 7 tips for navigating difficult feedback conversations with grace and compassion. These are things as SLPs we didn’t learn in graduate school (but should have!).

Listening to parent perspectives is essential for understanding a child’s life, so how can we do that without allowing that feedback to dictate our treatment plans? How can we listen to feedback that may be difficult and hard to hear without taking it personally, listening for key topics or issues that may be just below the surface?

While there is no way to “conflict proof” these conversations (all communication with humans will inherently involve conflict, since as humans we have differing desires and needs), these ideas may help you get through your next IEP meeting or parent conversation moving forward in a more positive direction.

5 Tips for Building a Sustainable SLP Career

  1. Communicate with caregivers, don’t put pressure for progress all on your own shoulders.
  2. Identify what “good enough” work for you means and what that looks like.
  3. Separate your identity from your job, your title isn’t who you are. Think about your strengths and your values - who you are isn’t only your professional role.

In this episode, Sarah walks you through three steps for getting started with a NLA Stage 1 student she worked with - including where to start, what information to gather, and what gestalts she decided to model.

Gestalt Language Processor = GLP(s)

Full Show Notes:www.slphappyhour.com/show-notes/142

Key Takeaways:

  • Take your time to provide a generous onramp to speech therapy for GLPs to get to know them
  • What data do we take for GLPs? The data is the language sample (Sarah takes hers on a Boogie Board and transfers it to her SOAP note when she has time)
  • Determine the “just right” amount of stimulation - students may mean more or less stimulation in sessions, incorporate sensory play and movement as needed
  • After observing play, use the strategy of “add one more” to add a step into a play routine they already have going or add one more toy (ex - another figurine or action) into the play
  • Sarah walks you through several play scenarios she used with this student - including a Fisher Price Frozen Castle, Potato Head, and a fabric tunnel
  • Summary: three overall steps to get started working with Gestalt Language Processors (GLPs) Get ongoing language samples for your new student: observe in play, gather language samples, prepare the parent to understand gestalt language development and play-based sessions

In this episode, Sarah interviews Sarah Cruz, an SLP in Northern California who shares about her experience implementing a high-teach AAC program in ECSE classrooms in California.

Key Points

  • Start with a specific offer of help to show AAC in Action
  • Show first then tell - show AAC by teaching a lesson with students and in classrooms/settings the students are in so staff can see AAC implementation is possible in their classroom
  • Interpret questions as curiosity (not as push back) and be someone that staff can ask questions to as they learn
  • Show AAC implementation in the classroom and make it simple - use routines staff already have and materials they already have access to
  • Let it take time - nothing changes overnight

As of this recording, a new year is upon us and while the whole world seems to be screaming that we can do more, better, and faster I want to remind you to invite in some self kindness. It’s OK to feel excited about the new year, or sad about the new year - or anything in between. Reflection and intentions or goals shouldn’t be a way to shame ourselves into doing more, but rather a chance for a check-in filled with self kindness and grace. And if you are listening to this episode and it’s later in the year - keep listening; the reflection questions in this episode are for anyone wanting to clarify what they want to focus on and what they don’t want to focus on moving forward - no matter what time of the year it is.

In this episode, Sarah gets coached by Rachel Madel, SLP on a case where she'd like to move beyond requesting with a student who uses AAC.

Key Takeaways

  • Involve the whole team, but within your treatment session time
  • It's takes a team to work with students, and you aren't the only member of that team
  • If a student has something they love, consider working on increasing independence with that requesting with different people and in different environments
  • A visual schedule can be an asset (not a detriment) to students as long as it is flexible and has options the child chooses - this helps them know what's happening next

In this solo episode, Sarah shares the 4 biggest mistakes students make when trying to produce the /r/ sound, and how to cue those misarticulations into “strong r” productions.

Discussed in this episode:

What to do when students:

Clenching their jaw

Rounding their lips

Distorted “throat sound” that sounds like gargling

Hybrid of any of the above or doing the “same sound”

Remember - it can take a long time to establish the /r/ sound and that doesn’t mean you are doing something wrong as an SLP.

If you are ready for sentences and readings, check out this Bloodsuckers Unit which is full of sentences and nonfiction readings on some “icky and fun” (so say my students) animals and insects. Here are a few units I recommend for students who are already at the sentence level or above (these include vocabulary and comprehension questions and are perfect for groups):

/r/ readings with a landforms theme targeting science standards bundle: https://www.teacherspayteachers.com/Product/R-in-Readings-Bundle-Nonfiction-Reading-Passages-6343345

r readings bloodsuckers bundle: https://www.teacherspayteachers.com/Product/R-Articulation-Readings-R-Articulation-Sentences-BLOODSUCKERS-6423675

Tips discussed in this episode:

By completing a thorough speech sample, we learn more about how and when (and if!) a student is using the /r/ sound

Completing a /r/ probe before we work on production will also make /r/ articulation speech therapy sessions be a lot less frustrating and more productive

Use consistent terms with your students when describing the /r/ sound (“lip sound” “tongue sound” and a “strong /r/” are some of my favorites)

Get your students lips out of the way by asking them to smile or by using some of the tips above

Practice discrimination and see if your student can discriminate your productions as well as their own productions, either as old way /r/ sounds or strong /r/ sounds

Working on the /r/ sound is one of the trickiest sounds to work on in speech therapy. Remember to give it time. If it’s taking time, chances are - you are doing it right, it’s just going to take longer than you think it will.

Why rest is important and what they are doing to get more of it

Plans for a special one-year of podcasting episode

Goals for the Winter (Sarah: create a clearer vision for the podcast, map out all the topics for 2019, figure out systems to work less at her private practice clinic; Sarie: spend more time with family and friends)

What they really think when listening back to the first few episodes (hint: we are a little embarrassed)

Sarah and Sarie do a special “what’s up, what’s down” segment on their highlights and lowlights of their first year podcasting

What it’s really like to do interviews (hint: Sarah wants to hug everyone)

How to find creativity in your work life

Sarah shares some personal news about her year and why the podcast has been so important to her in moving forward

How to “own your nerdliness” (according to Sarie)

We also share LOTS of outtakes at the end of the show, so keep on listening!

This episode, we talk about using the love languages at home and work, what we think of them and if they are really important.

Sarie also shared the most important things she's learned from the podcast to make it through her tenure as a CF:
1. Check email only twice a day (Episode 6)
2. Having difficult conversations, and emotionally charged conversations with parents, families, and caregivers (Episode 1 & Episode 9)
3. Saying No (Episode 3)
We mention the book Option B, and something Sarah learned about in this book about empathy and the Danish and how empathy is related to happiness. Sarah took this one step further and has been researching teaching empathy in schools.

This episode, we talk about working weekends. Do you do it? Do you feel like it helps or hurts you? We’ll share what we do, and feel free to chime in on the conversation.

We also answer a listener question - what do you say when people ask you what you do for a living? If you are an SLP, you are used to blank stares or odd comments when you tell people what you do for a living. Sarah and Sarie share the strangest things they’ve been asked at parties after revealing they are SLPs, and you are going to want to hear these stories.

The bulk of this episode discusses client success factors. Do you work with a variety of clients? Do some clients seem to have a bad attitude or get upset easily? Do you have other clients who seem interested in getting service and then seem to disappear, never to be seen again? Well, you might be ready for your clients - but you need to be able to answer the question - are your clients ready to work with you? The discussion is based on the research of Prochaska & DiClemente, 1983; Prochaska, DiClemente, & Norcross 1992.

We’ll also propose a self care challenge that’ll get you knowing your clients even better, and using your time even more effectively in your speech room.

On this podcast, we answer a listener question - How to do you connect with difficult-to-reach clients? We share our thoughts on everything from building rapport with teenagers, to establishing rapport with adult clients to forming a relationship with young clients with Autism and identifying what they will work for and creating sessions so clients will want to work with us.

We also talk about what is new in our speech room - which is being chronically late this time of year (because we are over booked?) and what we are doing about it.

Sarah shares a lesson gone wrong and what her new nickname is according to a new client. We won't put it in the show notes, but listen in if you are curious.

This episode answers a listener question:

The job of being an SLP is so busy and I feel like there is always more to do. How do you know when you just need to keep your nose to the grindstone and hustle away at the work, and when it’s time to stop, step away, and take a break?

If you find yourself powering through a season of overwork or deadlines, this is the episode for you. Sarah shares how powering through is sometimes helpful and when she’s had to do it. Sarie shares that powering through is the name of the game for a lot of the CF year. The discussion then moves along to how to have more boundaries and time and when it’s time to not hustle.

We also talk about - what do you do when you are out sick (for more than a day or two) as an SLP?
We also discuss difficult conversations with families we work with. What do you do when you need to share news about another possible diagnosis? What do you do when a client doesn't hear what you have to say?

We also discuss negative mantras: what they are, and how to catch yourself in one.

We end with a self care challenge to flip your perspective when times get tough.
Self Care Challenge

To find something you are feeling negative about, and to catch your negative work mantra.

Then, if you are telling yourself a negative story about a work situation nip it in the bud. Look for evidence that your negative mantra isn’t true. Maybe you’ll find it, maybe you won’t. Lastly, reassess your negative mantra to find out if it’s true and if it’s serving you.

This episode, we talk about some SLP advice that has helped us lately, perfectionism in the SLP world, and advice for your first year as an SLP.

Whether you are an SLP student, a new SLP, or someone who has been in the game for a long time, we hope you enjoy this conversation and pick up some tips along the way for managing some common pitfalls in the workplace.

This episode is perfect for anyone who is battling perfectionism as an SLP (aren’t we all?) or who is starting a new job or is starting to work in a new setting and wants to learn some tips for making it through the transition.
What NOT to do in your CF year. Don’t:

  1. Be isolated

  2. Do it all yourself / figure it out yourself

  3. Accept SLP guilt

  4. Think you know it all OR think you have to know it all

  5. Feel like your CF mentor has to be your primary mentor

  6. Surprise a caregiver, family, or parent at a meeting

  7. Only share the negative

  8. Eat lunch in the break room or office

  9. Reply to the emotional conversation right away

  10. Continue to deal with a difficult parent - or coworker - without asking for help

This week, we are digging in to ways that work email can make you miserable and actionable steps about what to do about it.
We also answer a listener question: what work settings do we work in?

If you ever find yourself shuddering over that next email ding, overwhelmed by your inbox, or even repeatedly checking your email, this episode is for you! We’ll talk about how you may not be using your email well and how to make it work for you.

We shared six email tips:

  1. Turn off all notifications
  2. Have scheduled check-in times for email. Stick to it!
  3. Scan for urgent emails first and leave the rest.
  4. Keep emotional conversations out of your inbox.
  5. When you check your email, have your to-do list in front of you, and slot in other’s requests in order of priority - don’t just take care of email tasks first.
  6. Get off the email response carousel (to keep out of your inbox: scheduling, emotional conversations, task management)
This week, we mentioned several project management systems, including:
