Sex after prostate cancer treatment can be a goal for some men and not a priority for others, but recovery is all connected. Taking time to address sexual health concerns will simultaneously improve urinary incontinence. It’s time to revitalize your pleasure.

In this special 4-part series featuring Dr. William Mills, PT, DPT and Dr. Susie Gronski, PT, DPT we delve into insights and solutions centered on the treatment and rehabilitation of men's pelvic and sexual health following prostate cancer.

Dr. William Mills, PT, DPT is a physical therapist who specializes in male pelvic rehabilitation, particularly post-prostatectomy and chronic pelvic pain. With a forte in manual therapy and corrective exercises, he enhances outcomes, enabling men to regain urinary control, bowel function, erectile function, and overcome pain. Together with his colleagues at Alinea Performance, he is committed to assisting men of all ages in enhancing their quality of life.

00:00 Introduction 03:30 How erections are good for your health 05:22 Using a penis pump after prostate cancer treatment 07:50 Focusing on pleasure over performance 09:31 Becoming comfortable with new sexual experiences 13:00 It’s all connected! 16:20 Shock wave therapy and vibration stimulation 18:20 Holistic approach to recovery

Reframing Sex Post-Prostatectomy

I’m not going to sugar coat it: things are different than they were, but your capacity for pleasure is limitless! Connect with your pleasure rather than the performance. While using something new to help you get erect may seem unfamiliar, getting comfortable with it is only a temporary hurdle. Once you’re comfortable, you’re free to keep swinging.

Strategies For Getting Hard

At this phase, you want to focus on blood flow. Tools like the penis pump, medications like Cialis or Viagra, and techniques like physical touch can help you get and maintain an erection. But remember: it’s not just starting in body, you also need to get your brain involved and retrain your neural pathways with mental arousal.

Your Concerns Are Interconnected

Recovery is a package deal. There’s research that supports using a penis pump can improve urinary incontinence. Focusing on all things that have changed will bring your recovery faster and farther.

How Pelvic Floor Physical Therapy Can Help

Pelvic floor physical therapists think outside the box, considering your personal experiences, physical health, a hands-on exam, and what matters to you most. We look to see what treatments are on the table for you and which to try first. Everyone is unique and some particular modalities or treatments may be more or less beneficial for some.

Many men experience urinary incontinence after prostate cancer. It’s common to delay treatment, thinking that it’ll go away on its own, but there are many strategies you can start using right away to stop peeing and start living again.

Our episode covers topics including:

00:00 Introduction
01:00 Bladder retraining for post prostatectomy
01:26 Pelvic floor contractions in treating urinary incontinence
02:23 Cueing pelvic floor contractions with Star Wars
03:42 Applying pelvic floor contractions to improve daily activities

Intake and Output

Start by taking note of how much water you currently drink a day: the goal you should aim for is ~half your body weight in ounces of water. Also note how often you go to the bathroom: the goal you should aim for is creating a bladder schedule where you pee every 2-3 hours. If you’re nowhere close to that 2-3 hours, don’t panic and don’t force yourself to hold it. A pelvic floor physical therapist can plan out a treatment path to get you where you’re at to where you want to be.

Pelvic Floor Contractions

Pelvic floor contractions are a coordinated effort with key three areas: simultaneously pulling in and lifting back of the pelvic floor, lifting the testicles upwards, and lifting the rectum towards your belly button. If you’re struggling with these movements, you’re not alone. Many men feel more confident with contraction exercises after working with a physical therapist.

After prostate cancer, men often grapple with urinary incontinence, sexual dysfunction, and other pelvic floor challenges. The significance of pelvic floor rehabilitation is that it can mitigate symptoms after prostate cancer and help you quickly get back to enjoying the activities you love. Whether it's regaining intimacy or achieving better bladder control, getting an early start can set you years ahead on your recovery path.

While it's expected to face new and persistent symptoms, it’s empowering to know that with tailored rehabilitation methods, these challenges can be temporary.

Our episode covers topics including:

00:00 Introduction 00:35 Deciding on radiation or prostatectomy for prostate cancer 02:01 Urinary incontinence post-prostatectomy 04:26 Sexual function challenges post-prostatectomy 06:04 Viagra and Cialis post-prostatectomy 07:03 Treatment approaches for post-prostatectomy 11:44 Should I go to the doctor for post-prostatectomy symptoms?

Delaying Treatment: You’re not alone. Many men experience sexual difficulties and urinary incontinence post prostatectomy. When concerns are not addressed early on, they may become more difficult to treat as time goes on, leading to unnecessary long-term suffering and reduced quality of life. There’s help and there’s hope.

Prostate Cancer Rehabilitation Options: A comprehensive understanding of post-treatment rehabilitation for prostate cancer is key. While the key decision may feel like deciding between radiation and prostatectomy, there are many decisions for after-cancer care.

Intimacy After Prostate Treatment: Some men experience reduced sensation in their penis and erectile dysfunction, feeling less connected to physical stimuli and their sexual selves. Nerve injury and diminished blood circulation, which can be a result of prostate cancer treatment like prostatectomy, can lead to reduced penis sensation and erectile dysfunction. Penile vibratory therapy, a penis pump, and medications are just a few treatment options that can help in improving blood circulation and erectile function. An individualized approach is best.

Urinary Incontinence After Prostate Treatment: By addressing bladder habits, such as hydration and reducing potential bladder irritants, and with functional pelvic floor exercises, recovery can be expedited and post-treatment symptoms like incontinence can be mitigated. Proactively seeking knowledge and tailored rehabilitation strategies enables you to regain urinary control, greatly enhancing your quality of life outside the bathroom.

Need for More Expert Male Pelvic Health Therapists:

Everyone has a pelvic floor, including you, and it plays a pivotal role in concerns such as erectile dysfunction and urinary incontinence. Unfortunately, due to societal stigmas and narrowly focused marketing, there's a disconnect in the availability of pelvic health therapists specializing in men's care. Addressing this educational and accessibility gap is crucial for providing comprehensive healthcare to those who need it most.

Leading Voices in Care for Male Pelvic Health:

Dr. Susie G and Dr. Jo Milios are at the forefront of efforts to educate both patients like you and physicians about male pelvic health while improving access to necessary care. Dr. Susie is the founder of a pelvic and sexual health clinic exclusively for men, serving patients in-person in Asheville, North Carolina and globally online. Her clinic brings together the expertise of pelvic floor physical therapy, pelvic floor occupational therapy, nutrition, sex therapy and mental health. Meanwhile, Dr. Milios's research promotes sexual recovery and continence control through tailored pelvic rehabilitation using pelvic specific exercises for men and therapeutic ultrasound for Peyronie’s Disease that can be adopted to improve therapeutic outcomes post prostatectomy.

Confronting Chronic Challenges in Post-Prostate Cancer Care:

Issues accompanying prostate cancer, such as erectile dysfunction and urinary incontinence, can severely hinder your quality of life. These conditions extend beyond physical repercussions, also taxing mental and emotional health. It's essential to understand that for many who have accepted these issues as the new norm, there are avenues to alleviate or even completely overcome these concerns.

It’s human nature to get excited by the latest and greatest treatments, from outer space to muscle tone pelvic pain, we’re captivated by scientific advancements. But this enthusiasm has a dark side, causing many clinics and patients to exchange simple and effective strategies for flashy new treatments that come with a hashtag.

Dr. Rachel Worman is a licensed physical therapist (PT) who has experience in acute care physical therapy and extensive dual training, clinical and teaching experience in musculoskeletal and pelvic health physical therapy. After 13 years of clinical practice, Dr. Rachel earned her PhD with a focus on pelvic health outcome measures and male pelvic health.

We cover topics such as:

1:15 What to call it? Hypertonic pelvic floor, overactive pelvic floor, tight or non-relaxing pelvic floor muscles?

8:55 How and who treats pelvic floor dysfunction

10:30 Educating patients with pelvic pain and choice of language

16:37 Uncertainty and the biopsychosocial model

17:40 Pain and hypertonicity

20:00 ICS and reasoning scale

36:00 EMG biofeedback for pelvic floor physical therapists

45:00 Internal and manual therapy for male pelvic pain

51:30 Psychosexual measures and sleep, hormonal, or musculoskeletal factors

55:00 Bridging research with clinicians on social media

Pelvic floor physical therapists and patients have to be mindful of the influence that media (including social media and speakers at conferences) has on trending treatment ideas and how common it is for a 10-second video on social media to hijack a patient’s treatment plan, simply because it wasn’t a good fit.

There’s no one-size-fits-all treatment strategy for pelvic pain, which is why there many treatment strategies. Through critical evaluation, clinicians must determine the most appropriate and effective treatment strategy, or combination of strategies, for that single patient. Dr. Rachel shares how important it is for clinicians to be flexible, individualized, and science-based, and to avoid getting caught up in clinical habits of using one hallmark treatment for all patients.

In such a difficult field, where all the measures we have for tone are problematic, these  generalizations and trending treatment ideas, while good for marketing, ultimately delays effective treatment and harms patients. It’s necessary to prioritize the knowledge of evidence-based practices and personalized care to ensure optimal outcomes for patients.

Gain self-confidence and dating confidence by knowing that you’re presenting yourself exactly the way you want to be perceived.

Mariko is a coach, businesswoman, thought leader, and lightworker. She coaches clients to do whatever it is they truly love whilst thriving financially; by guiding them lovingly and playfully in their exploration, between the material world and the spiritual world; the light and the dark; vanilla and non-vanilla.

We cover topics like:

  • 2:18 sexual consent in online dating
  • 9:24 why is confidence attractive
  • 13:01 why shadow work is important
  • 19:54 first message tips for dating app
  • 33:21 what to do when ghosted in dating

A common fear in dating is that your actions or messages will be misinterpreted, and that you won’t be seen for who you truly are. Many guys worry about being labeled as the "creepy guy," the “nice guy,” or other stereotypes that don't represent who you truly are.

Uncertainty can kill the mood. It's also a sign of lacking confidence in yourself and how you come across to others. When you show up as your authentic self, there’s no uncertainty or confusion: you feel confident in who you are and understand how you'll be perceived.

Becoming confident in who you are can help you overcome stressors not just in dating, but in other relationships as well, like work or family. Shadow work, a transformative self-exploration technique, delves into unraveling the stories we create in our minds that make situations seem heavier than they should be. Often, these stories revolve around untrue narratives.

Shadow work helps you tackle the parts of yourself that are often tucked away in the dark. When you’ve done the work to foster self-awareness, confidence, and the ability to navigate emotional terrain, dating can be a fun and fulfilling experience.

Here's Mariko’s best online dating tip for Hinge users: genuine interest trumps self-promotion. Your opening line should reflect that you've taken the time to read their profile.

Penis implant surgery provides hope for those unable to get an erection and have penetrative intimacy with a partner. Without the worry of not being able to get it up, an implant can help you stop thinking about erectile dysfunction and focus on your pleasure and relationship instead. A penile implant is not a cosmetic surgery that helps you to last longer, but a tool to regain intimacy.

Dr. Jonathan Clavell is a board certified urologist and men's sexual health expert in Houston, Texas. His practice focuses on the treatment of erectile dysfunction, low testosterone BPH and Peyronie's disease. He is one of the highest volume surgeons in penile implant surgery, and currently trains doctors both nationally and internationally when it comes to this surgery.

We cover topics like:

  • 2:40 What is a penile implant
  • 6:10 Who needs penile implant surgery and can a penile implant be removed
  • 9:20 When erectile dysfunction is psychological
  • 14:40 If a penile implant won’t work for you and disadvantages
  • 18:54 What a penile implant will and won’t change
  • 21:40 Can a penile implant make you bigger (or smaller?)
  • 27:30 Using sex toys and sexual satisfaction with a penile implant
  • 33:10 Erectile dysfunction after prostate cancer and prostatectomy
  • 37:20 Penile implant for gender-affirming surgery
  • 38:55 Who performs penile implant surgery
  • 42:37 Avoiding complications with penile implant surgery
  • 47:50 When to start using a penile implant after surgery

What is a penile implant? A penile implant is a device that can give you an erection: there are inflatable and malleable devices, with the inflatable device being the most common in the US.

If you’re struggling with erectile dysfunction, it’s common to wonder which erectile dysfunction treatment is best and if a penile implant is right for you? For many men and their partners where erectile dysfunction is a concern, a penile implant was the perfect option because it’s very effective at helping you achieve an erection and be able to have penetrative sex.

Let’s take a step back and look at the physiological cause of erectile dysfunction: caused by blood not being able to fill or remain in the penis, causing the penis to remain flaccid. For anyone experiencing erectile dysfunction, the first line of treatment is not considering a penile implant. Regardless of age, the first line of treatment for erectile dysfunction is to start with medication. If you experience erectile dysfunction from stress, you could also benefit from mental health or physical and occupational therapy.

For vasculogenic erectile dysfunction, when Viagra doesn't work (or any of the other common medications like Levitra, Staxyn, Cialis, or Stendra) a penile implant becomes an available treatment option.

Penile implants work so well because it gets your penis hard. But it’s important to note that a penile implant can not engorge the head of the penis, can not increase penis sensitivity, can not impact orgasms, and can not impact ejaculation (your partner can still get pregnant if you have a penile implant).

Is a penile implant right for you? If you’re considering a penile implant: look for a surgeon who has experience: who has done a penile implant recently and does them regularly and who is also comfortable with doing the procedure and comfortable managing complications. A great surgeon also knows how to prep you for surgery in order to get the results you want, and what post-op care to recommend.

Dr. Matos shares the core skill guys lack that are necessary in having a successful date after matching on an app. These skills also create a strong foundation for a long-term relationship and having passionate sex for many years to come.

Dr. Greg Matos is a board-certified couple and family psychologist in Los Angeles, California. He is the founder of the charity abetterloveproject that helps people start healthier relationships from the beginning. Dr. Greg is a former Marine Corps Sergeant and recipient of the Bronze Star with Combat Valor.

We cover topics like:

  • 1:52 Why younger men are lonelier than younger women - 4:20 It’s hard to hear this advice - 6:00 Dating app tips for guys - 6:33 Core relationship skills that guys usually struggle with - 8:40 3 practical scripts to easily connect with your partner - 13:30 Tips for your dating app profile - 18:56 Dating with chronic illness, sexual pain, or sexual dysfunction - 23:55 Relationship goals - 27:40 Discussing something difficult with your partner

Dr. Matos shares 3 scripts for healthy communication habits:

1) How to create a deep bond with your partner:

  • Ask your partner “what was the most stressful part of your day today?” - Listen and follow-up with a clarifying question “tell me more about (that part)” - Sharing what you think you understood from listening - Validate their experience by saying “that makes sense to me”

2) How to peek into your partner's inner world:

Asking: “what are you most grateful for today,” and: “what are you looking forward to tomorrow?”

  • Being curious about their inner world, validating their experiences, and being able to provide support

3) How to start a challenging conversation:

“I feel _____ about _____, I need/want/prefer _____”

  • When you start a conversation with “I feel,” you’re firing up your partner’s mirror neurons - “about” allows you to share a specific event, action, or experience - “I need/want/prefer” allows you to communicate what your needs are that your partner can fulfill

Finally, here's Dr. Matos' #1 dating app tip: put time and effort into nailing the first impression when you match or like someone's profile.

Hinge is the best dating app for a serious relationship: you can ❤️ a prompt and add a comment, instantly improving your chances of creating a lasting first impression.

Men who are aware of female sexual anatomy relax more around sex and enjoy their sexual pleasure more. Why? Because they don’t have to work so hard (no pun intended) to “give” their partner an orgasm.

Research shows that even in relationships, 85% of men will orgasm during sex while only 65% of women will orgasm during sex. Only 4% of women experience orgasm from only penetration, and a lot of women don’t have their first orgasm until they try a vibrator.

While the entertainment industry has culturally taught us that men only orgasm from thrusting fast and hard, there are more pleasurable experiences to be discovered. There’s so much sexual freedom that comes from learning about female sexual anatomy!

We cover topics like:

2:14: Why women have fewer orgasms than men and how it’s not your job to give your partner an orgasm (but tips to help you do it!)

4:30 How men can understand female anatomy to have mutual orgasms during sex

Chronic pain, muscle tightness, and fatigue are common complaints for those with Ehlers–Danlos syndrome (EDS); an inherited condition that can take over 10 years to diagnose.

In this episode, Libby shares why the specialty model of medical care leaves many health concerns undiagnosed. Multi-system conditions, like Ehlers–Danlos syndrome, are hard to diagnose not only because many physicians are not educated about them, but also because people with Ehlers–Danlos syndrome experience vague clusters of symptoms that don’t fit within a specific medical specialty, leaving the majority of concerns undiagnosed or left with random and overlapping diagnoses.

Libby Hinsley is a Doctor of Physical Therapy and Certified Yoga Therapist specializing in the treatment of chronic pain, hypermobility disorders, and yoga-related injuries. She has taught yoga since 2005 and has trained yoga teachers for over a decade.

We cover topics like:

2:33 Hypermobility and Ehlers-Danlos syndrome, symptomatic hypermobility, hypermobility spectrum disorders, and connective tissue disorders

7:19 Diagnosing Ehlers-Danlos syndrome

10:10 Common symptoms of Ehlers-Danlos syndrome

15:05 Impacts on the urogenital tract and muscular tightness

22:30 How to manage chronic pain capabilities with Ehlers-Danlos syndrome

29:10 Hypermobility and Ehlers-Danlos syndrome in men

35:00 Yoga for bendy people

40:00 Tips on finding a healthcare professional for Ehlers-Danlos syndrome

I love how Libby reminds us why getting a diagnosis is freeing: because when you understand your physiology, you can learn how to approach your life without fear of the “what if.”

Ehlers-Danlos syndrome involves a genetic anomaly in collagen and connective-tissue synthesis and structure. This is important to recognize when considering concerns in your pants, because the pelvic region and urogenital system are made up of collagen-rich tissues (like the bladder and pelvic ligaments).

With EDS, wherever there’s connective tissue, instead of having taught tissues, the tissue sags and creates an opportunity for pelvic floor prolapse, urinary and fecal incontinence, anal fissures, vesicoureteral reflux, recurrent urinary tract infections, cryptorchidism, hypogonadism, testicular torsion, and tight foreskin.

Because muscle tightness in Ehlers-Danlos syndrome is a compensatory mechanism for a lack in joint stability and muscle strength, treatment plans start with simultaneously unwinding tension while building a foundation of functional strength. Before training and strengthening muscles, it’s important to allow the muscles to relax first, since an already activated and contracted muscle can’t contract anymore.

How and why to seek a diagnosis for Ehlers-Danlos syndrome:

On average, it takes 10 years, leaving many suffering for a prolonged time. There’s a wide spectrum of symptom severity for people with Ehlers-Danlos syndrome. For some, the symptoms magnify quickly after surgery, viral infection, or changes in hormones (like during or after pregnancy).

There are 14 types of Ehlers-Danlos syndrome, with Hypermobility Ehlers-Danlos syndrome (hEDS) being the most common. 13 of the types are diagnosable with a blood test, while the hypermobile type is diagnosed with a clinical evaluation.

For healthcare providers and practitioners, rare conditions like Ehlers-Danlos syndrome are a reminder to check assumptions when considering a default diagnosis and to think outside your patterns of diagnosing by really considering all the symptoms in front of you.

Refer to the Ehlers-Danlos Society checklist: https://www.ehlers-danlos.com/heds-diagnostic-checklist/ and the signs and symptoms: https://www.ehlers-danlos.com/what-is-eds/

There are many conditions that are commonly diagnosed with those who have Ehlers-Danlos syndrome. The EDS classification provides more information about these conditions https://www.ehlers-danlos.com/2017-eds-classification-non-experts/

Urinary incontinence is not only expensive, but also robs you of fun in life for fear of having an accident.

Today’s guest, Fiona Rogers has researched the safe and highly effective results of using TENS (transcutaneous electrical nerve stimulation) on bladder pain and urinary incontinence. Fiona shares her knowledge with other health professionals and with us on the show.

We cover topics like:

3:36 How bladder concerns impact your quality of life

5:40 Using TENS for urinary incontinence

10:10 Using TENS for bladder pain and pelvic pain

12:00 TENS unit recommendations

14:10 How to use a TENS for bladder concerns

16:30 TENS or neuromuscular electrical stimulation

20:00 Research on the effectiveness of TENS and how long it can take to see a change

27:30 Which health professional can get you started with TENS?

35:30 Upcoming courses for health professionals on TENS

TENS works by communicating with the neural pathways that communicate with bladder spinal reflexes and your brain. Electrical nerve stimulation via the skin stimulates sensory nerves that can help calm an extra sensitive bladder. When used as part of a treatment plan, Fiona saw how using TENS for bladder concerns was very effective.

How fast will you know if TENS working for you? For an overactive bladder, you’ll typically see results in a few weeks (episodes aren’t as intense, more time between episodes…) and should use a TENS unit for at least 12 weeks.

For an overactive bladder: Fiona recommends placing the TENS pads on the ankle over the tibial nerve. You’ll know you’re on the nerve if you turn up the pulse width and see your toes start to wiggle. Start with 10 Hz and 200 pulse width, increasing the pulse width until you start to notice the sensations. Use for about 30-minutes daily.

For bladder pain: Fiona recommends placing the TENS pads over the tibial nerve or sacrum. You can either start with a low frequency 1-4 Hz or a high frequency 80-150 Hz. Use for about 30-minutes daily.

When purchasing a TENS unit, make sure you can adjust the frequency (measured in Hertz), pulse rate (pulses you feel in one second), and pulse width (the amount of stimulation you feel, you want to be able to change the pulse width in small increments) to give you the most control and personalization over the unit.

There are many, many challenges with pelvic pain that don’t happen if you sprain an ankle or have a shoulder injury that you can point to and talk about with your co-workers.

With pelvic pain, it may feel hard to show others where it hurts— even a health professional, awkward to talk about it with friends or someone you love, that your morals or faith will be questioned because of religious beliefs, or that the pain will never stop.

Dr. Sandy Hilton is a pain and pelvic health physical therapist currently in Chicago at Entropy Physiotherapy. Sandy works in the clinic and online to help people with pain recover and is an international invited speaker and instructor of other health professionals. Sandy teaches classes on manual therapy and sensory integration techniques that are purposefully non-painful and help recover tolerance to sitting, bike riding, sexual function and more!

Dr. Sandy shares strategies to move through challenges with pelvic pain:

  • Using your imagination to identify what can trigger pain and how to unhook your thoughts from the pain
  • Using graded exposure (from sports psychologists and used in Olympic-level recovery strategies) to start doing something challenging again
  • Gaging your progress in relation to what else is happening in your life— a flareup doesn’t mean your progress is slipping away
  • Making it a priority to do something you love every single day

I love how Dr. Sandy reminds us that even though the pain can become the center of your life, life is not about dealing with the pain; managing pain is about how you approach living your life beyond the pain and making your happiness a priority.

Even if the body and pain is complex, the strategies we can use to recover from pain doesn’t have to be!

We’re going on a “Magic School Bus” ride (cue 1994 PBS nostalgia) to our cellular systems on the nanomolecular level. Jump to 34:40 for simple biohacks to reset a hypersensitive biological system that happens during chronic pain.

“Change is inevitable, how you change is not.” Huge things are possible when we know how our body works and actively use that knowledge to make changes to our cellular systems. Mark reminds us that without trust, transformation in finding relief from chronic pain is really hard.

Professor Mark Hutchinson is the Director of the ARC Centre of Excellence for Nanoscale BioPhotonics (CNBP), an Australian Research Council Future Fellow, a Professor within the Adelaide Medical School at the University of Adelaide, and President of Science and Technology Australia, the peak body in Australia that represents 90,000 scientists. Mark explores the “other brain” and has pioneered research which has led to the discovery of novel drug activity at innate immune receptors.

We cover topics like:

  • Why we experience pain
  • Strategies that speak the same language as our cellular systems
  • Why our cells remember pain

We have hundreds of cellular systems that turn on and off hundreds of switches in our body to stay in control and keep us safe. These switches determine why, when, and how much pain we experience in response to stimuli.

When we feel a feather against our skin or step on a Lego, we’re experiencing nociception. Over time, our baseline of what turns these switches on and off becomes more sensitive, resulting in hypernociception and allodynia that are caused from stimuli that normally wouldn’t cause pain, but now does.

Strategies that work with how our cellular systems function include:

  • Eating well (eating plants borrows their defense mechanisms!)
  • Going on a walk outside
  • Playing in nature
  • Having deep meaningful conversations (talking actually changes the structure of the central nervous system)
  • Creating a positive and supportive environment
  • Drinking hydrogenated water
  • Mindfulness practices, “thinking well” breathing practices like Wim Hof supercharges your immune response and resilience

When exploring strategies, your mindset matters! Having an expectation that the strategies can work influences the outcomes. It’s critical to have a positive mindset that you can shift your pain, even it’s only by a little bit.

Doing a self-examination often can help you to detect Peyronie’s disease as well as other concerns (like testicular cancer). The earlier you can detect Peyronie’s, the easier it is to treat it as a soft tissue injury.

Take one minute while in the shower to:

  • Check your balls for lumps, bumps, and changes in shape
  • Check your penis for any bending/curvatures, rotation, or hard or calcified skin

Dr. Jo Milios is a Titled Clinical Researcher and Men’s Health Physiotherapist (MACP) who has a special interest in Prostate Cancer. Dr. Jo has presented the research findings from her PhD at many international conferences including the World Confederation of Physical Therapy (WCPT), Asia-Pacific Prostate Cancer Conferences (APCC), Australian (APA) & American Physiotherapy Association’s (APTA).

We cover topics like:

  • Why Peyronie's is commonly thought to be caused by penile injury or a buckling effect, yet only ~10% of men recall an incident
  • Connection between Pyronie’s and treatment for prostate cancer or other pelvic surgery (like hernia repairs and bowel surgery)
  • Connection between Peyronie’s and cardiovascular changes (like heart disease and diabetes)
  • Injuries during sex that have caused Peyronie’s in younger men
  • The low down on treatments you should be considering if you’ve been diagnosed: therapeutic ultrasound, pumps, traction devices, medications, and more.

If you’ve been diagnosed, what are the treatment options for Peyronie’s? In addition to pumps, traction devices, and medications, Dr. Jo’s research on the effectiveness of therapeutic ultrasound has been shown to be an extremely effective and gentle treatment for Peyronie’s that any physical therapist can use.

Refer to her thesis for all the nuts & bolts of ultrasound therapy for Peyronie’s, erectile dysfunction, and urinary incontinence:

Pain is just neuroscience. Your brain is calculating information 24/7 to keep you safe and alive; when it calculates the threat of danger, pain becomes a protective measure.

This information is biological (like recognizing a broken bone, torn ligament, or a cut), but there are other factors the brain is calculating to make a decision on whether or not you’re in danger and need pain to rescue you.

Knowing the information that both produces and reduces pain levels allows your brain to start to process pain differently. Dr. Rachel shared many of these factors and their impacts, including:

  • Cognition (the way you think)
  • Mood and emotions (the way you feel)
  • Behaviors (what you do)
  • Context and environment (where you are and who you’re with)
  • Beliefs around how you feel
  • What other stressors you may already have (like income, available resources, and social support you have access to)
  • Fears (uncertainty and unknowing)
  • Attention (what you’re focusing on)
  • History of experiences (what has happened previously when you’ve had pain

Dr. Rachel Zoffness is a leading global pain expert, international speaker, author, and thought-leader in medicine revolutionizing the way we understand and treat pain. She is a pain psychologist, Assistant Clinical Professor at UCSF, and lectures at Stanford. Dr. Zoffness is the author of The Pain Management Workbook and consults on the development of integrative pain programs around the world. She is a regular guest on popular podcasts like Ologies, Jordan Harbinger and ZDoggMD, and her episodes have over 5 million downloads.

We cover topics like:

  • Breaking the stress, anxiety, fear, increased pain cycle
  • Science behind how pain is emotional and physical
  • Tips and exercises on recovering from chronic pain

I love Dr. Denise’s focus on restoring quality of life and normalizing that there’s no normal in sexual function and desires for men. Instead of a disease-centered approach that puts a diagnosis in the spotlight, focusing on getting back to enjoying life with a personal approach.

Dr. Denise Asafu-Adjei is a urologist with subspecialty training in sexual dysfunction and male infertility. She completed a Fellowship in Andrology at the University of California Los Angeles (UCLA). She will be joining the Urology Department at Loyola University in Chicago as the Medical Director of Male Reproductive Medicine.

We cover topics like:

  • The reason why men don’t seek out healthcare as often as women do
  • Penis-performance and other cultural expectations
  • Ultrasound and traction device therapy for Peyronie's disease

When you have a concern, the first step is finding a provider you trust to get reliable and accurate answers to your questions. Men tend to be less proactive in scheduling routine appointments with physicians and face social and cultural messages that can be constraining, misleading, and create fear around going to the doctor for sexual health concerns.

Because it’s not openly discussed, concerns and underlying causes may go untreated for years.

In my practice I see a gap in access to care, trust in the medical community, and not knowing when and who to schedule an appointment with when sexual health concerns arise. In Dr. Denise’s practice, she has seen patients struggling to talk about their sexual health and that many physicians aren’t comfortable talking about sex.

I admire and look to Dr. McCarthy as a mentor for his work to empower men to enjoy their sexuality, freedom of choice, and to challenge the pressures men have to experience sex in a specific way.

Dr. McCarthy shares how leaning into sex as sharing pleasure between partners, and letting go of the emotional need for total predictability and control, can deepen your own feelings of pleasure and allow you to continue to be sexual and playful with your partner.

Vulnerabilities and concerns do not define your worth, they encourage you to deepen your relationships with yourself and your partner. Living authentically is through recognizing sexual freedom is about having choice and feeling free to engage or disengage- the choice to say yes and no to sex or stimulation and to know your veto is accepted and respected.

I am so proud to share Dr. McCarthy’s wisdom with you in this episode.

Dr. Barry McCarthy is a board-certified clinical psychologist and diplomate in sex therapy who practiced for 42 years and taught human sexuality for 50 years. His latest book is Contemporary Male Sexuality (which I highly recommend!)

We cover topics like:

  • Male sexuality and challenge the current myths
  • When and how to talk sex with your partner
  • Sexual equity and finding synchronicity in sex
  • Vulnerabilities and male sexual pain
  • Hot facts about sexuality after age 60

The moments in life that bring you the most joy in your day (the things you would not skip or live without) is what grounds us and makes us feel powerful. In my conversation with Dr. Sakshi Tickoo, founder of Sex, Love, And OT, and a queer Occupational Therapist and Counselor actively working in the areas of sexuality, intimacy, relationships and mental health across lifespan, we’re reminded how there is no normal. From sex to relaxation, what does your personal pleasure practice look like? Maybe it’s your morning coffee or conversing with your partner over dinner; recognizing that pleasure is not only synonymous with sex opens up a world of possibility, play, and happiness. Dr. Sakshi also reminds us that the conversations we have with others validates our experiences, helps us become more comfortable talking about sex and sexuality, and challenges our assumptions on pleasure. Prioritizing building the intimacy and connection you have with yourself and your partner(s) is an exploration that you can do anywhere and everywhere with the five love languages. ☕️ Questions you want to ask, options you want to sort through, or deciding on what you should do? We explore religious lenses on sexuality and pleasure. From giving ourselves permission to feel pleasurable to the fierce and genderless expression of sexuality, your body is a precious gift.Your body is asking permission to be a body and to be appreciated and loved in various expressions. We talking about the Bible and what the Catholic Church says about pleasure and sexuality. “Am I enough?” is a social and sexual anxiety we ruminate about. Yet, how did we get to this place of having abandoned our unconditional self-acceptance? Deepak put it beautifully when he said that: “we are at war with our own bodies and it’s desires.”
Eroticism is part of breaking free from our binary thinking: of true/false ideas. Erotic intelligence, similar to emotional intelligence, is the awareness of and accepting of your emotions, what brings you physical pleasure, what brings others physical pleasure, and the connection you experience with yourself and with others.
Deepak shares how you can be present as a nonjudgemental observer: starting by reflecting and noticing what you’re drawn to in the world and what repels you.
Listen, and you’ll hear that you are more than enough.

Dr. Murray uncovers how and when the shift happens when men say they don’t feel “as much as a man as I used to,” and worry if their partner is thinking the same thing. Delving into the challenges of how men can be authentic in expressing their sexual desires when the chasm of expectations and standards keeps them hiding their true sexual health and concerns. Her favorite finding from the research was discovering how deeply men wanted to feel sexually desired, physically cared for, close to someone, and romantically wanted, despite being part of a culture that minimizes these sexual experiences, vulnerabilities, and intimate desires. Remembering that sex can be much more than penetration can open up that trust, emotional connection, and broader conversation to shift your sexual encounter into the experience you truly crave.2:00 Pivotal moments that sparked Dr. Murray research male sexual desire

Pooping, peeing, and sex (our body’s basic physiological functions!) are just as important for our well-being as good nutrition and daily movement are for a healthy body Everything’s connected: nerves from the spinal cord and lower back connect with the pelvic floor and impact the penis, bladder, and bowel. Addressing our basic physiological functions allow the nervous system to dial down, and allow us to feel confident and safe: just like eating a chunk of broccoli nourishes our body with essential nutrients. We uncover the research that links lumbar spine conditions together with pelvic pain syndrome. The most up-to-date information on pain science from the researcher! Taking new research advancements and implementing them in the clinic. Researchers found that TENS increases the body’s inhibitory pathways and reduces their excitability in the nervous system. After years of research and understanding how it works: researches tested it to find out if TENS can help reduce pain in movement. TENS is a safe way to activate your opioid system (using your own body’s pain killers), and is what Dr. Kathleen Suka recommends is in everyone's medicine cabinet. Everything in the nervous system is moldable, and once you have a concern going on, it doesn’t mean it’s set it stone! Pain can change

If you’re looking for one physical cause to your pain, you’ll never find it. Pain is influenced by how you feel about it, as much as it’s influenced by biomechanical, physiological, or pathological factors. There’s a lot of research on the mechanisms of pain, on rating pain, and strategies for treating pain, but research has widely ignored the impacts of persistent pain on a person’s life. Why is this missing puzzle piece so important? Because Tom Astill’s QoL study on pelvic pain connects the dots between how pelvic pain impacts your life and where you can begin to explore your attitudes, beliefs, and values to start feeling better. We’re zooming out and taking a look at everything with a wide scope. It’s all connected.
