After the episode on chandler and his ketamine related death, Wip asked JP questions about ketamine and tolerance based on his past history of personal use. While there are differences in the use if ketamine for depression vs addiction, for some people, self titration to effect is not realistic. Medicine also has history of iatrogenic harms caused by prescribed drugs. He is the Interim Vice-Chair of Research for the Department of Family and Community Medicine and holds the Dr. Barnett & Beverley Giblon Professorship in Family Medicine Research at the University of Toronto. He is a Clinician Scientist and Director of the Mental Health and Addictions Division in the Department of Family and Community Medicine. He is also a Professor in the Department of Psychiatry and in the Dalla Lana School of Public Health at the University of Toronto. He is also a full member of the School of Graduate Studies through the Dalla Lana School of Public Health and the Institute of Medical Sciences at the University of Toronto.Areas of ResearchDr. Selby's research focus is innovative methods to understand and treat addictive behaviours and their comorbidities. He also uses technology to combine clinical medicine and public health methods to scale up and test health interventions. His cohort of 280,000+ treated smokers in Ontario is an example. His most recent program of research utilizes a Learning Health Systems approach to investigate how technology enabled collaborative care can enhance the delivery of evidence-based interventions to the patient while providing a more satisfying experience of care for both patient and provider.Dr. Selby has received grant funding totaling over 100 million dollars from CIHR, NIH, and Ministry of Health, and has published 38 research reports prepared for the government.Dr. Selby mentors residents and faculty from a variety of disciplines. ‘Detective Joseph Aliberti from the NYPD has charged Sophia Marks for knowingly distributing and possessing narcotics in violation of Title 21, United States Code, Section 841. The narcotics included fentanyl and alprazolam. Aliberti's knowledge comes from personal investigation, examination of reports and records, and conversations with other law enforcement officers. The main evidence is a text conversation between Marks and a victim, in which Marks discusses selling counterfeit oxycodone pills and Xanax. After purchasing these drugs, the victim was found dead of a suspected overdose. Further evidence includes surveillance video of the drug delivery, and lab tests confirming the presence of fentanyl and alprazolam in the drugs found at the victim's death scene. The victim's toxicology screen was positive for cocaine, fentanyl, and benzodiazepines. Another sale of fentanyl to an undercover officer was also documented.’

Dr. Wiplove Lamba and Jean-Paul Michael went through Matthew Perry Audiobook, ‘friends, lovers, and the big terrible thing’ on his recovery journey. It was a moving and powerful experience. They learned about early attachment trauma, substance use, and the internal and extremely dangerous life threatening struggles he has faced. They also learned about his experience in various treatment programs he attended and his near death experience. While both Wip and JP do not work in private treatment centers, some of the items brought up were extremely interesting and relevant to addiction practice in general. Addiction medicine, harm reduction and safer supply has been in the news of late to the point that it has become conservative talking points. New media have talked a lot about misinformation and disinformation , and JP convinced Wiplove that it would be worth breaking down a promotional video.

Dr. Sanjeev Arora developed project ECHO (extending community health outcomes) in New Mexico to help the generalist to pick up the skills to treat Hepatitis C. This is a very low barrier treatment for both the therapist or peer support worker to learn AND for the patient to get access to. How do I learn more???CRA (community reinforcement approach) and CRAFT (community reinforcement approach and family training) are the two best therapies for people who use drugs and their family members respectively. Maybe a little bit of bias in that statement lol. The outcomes are solid, but it hasn’t had the marketing you see in regular CBT and other psychotherapies. We have always admired strong women. Wip’s Grandmothers and his mother were incredibly inspiring to him, and now he learns great life lessons through his wife and daughter on a regular basis. Addictions case manager and former addict Jean-Paul (JP) Michael and addiction psychiatrist Dr. Wiplove (Wip) Lamba discus the perspectives of psychiatry in more detail. This includes shifting the conversation from how the perspectives of psychiatry model is into how would you actually use it in practice. Wip is not able to fully answer the questions and for clarity, both of them decide the next best step is a mock demonstration of what a psychiatric assessment looks like with a mock patient or a standardized patient.
The key point is that a traditional psychiatric assessment is how it is. And the perspectives of psychiatry approach allows for a unique and complementary formulation.

At John Hopkins, McHugh and Slavney developed the perspectives approach to psychiatry. Addiction psychiatrist Dr. Wiplove (Wip) Lamba regularly uses it in his clinical practice. In this episode, Wip and addictions case manager and former addict Jean-Paul (JP) Michael discuss the model, the different quadrants and how they help us understand the person and match to the appropriate treatment. Wip believes this allows for a broader understanding of the person than a DSM diagnosis alone, and it is a useful complement to whichever clinical approach any of you use. It divides compliments of the person into 4 areas: disease, dimensional, behavior and life story. Disease is what a patient has, dimensional is what the person is, behavior is what the person does and life story is what the person encounters.

Dr. Wiplove (Wip) Lamba has come closing questions for addictions case manager and former addict Jean-Paul (JP) Michael. A lot of things were not discussed.
Wip does ask however what it was like to have had a recent history of drug use and attend a training on chemsex for health care providers, how he can do his job better, and JP gives Wip a few ideas of what to put up into his office to make the environment more inviting.
JP also makes it clear that we need to have the conversations to get these experiences out in the open.

Community Case Manager Jean-Paul Michael In 2015, Jean-Paul ended his two-decade long entanglement with substance use. He has attempted to put that lived experience to a useful purpose. He joined St. Michael’s, Hospital Addiction Service, as one of their first Peer Support Specialists. Today, as a Community Case Manager he provides intensive case management for some of the most marginalized patients. When not working directly with patients, Jean-Paul’s other passion is as an harm reduction educator and as a consultant. Primarily providing guidance on how to better support Persons Who Inject Drugs (PWID). Prior to healthcare, Jean-Paul was a lifelong entrepreneur. At 19, he co-founded Northern Cards, which ultimately grew into becoming Canada’s largest greeting card company.

Addiction Psychiatrist Dr. Wiplove Lamba & Addictions Case Manager and former addict Jean-Paul (JP) Michael have learnt a lot about how to help men who “party with drugs” (PNP) from Dr. David Stuart. Dr. Stuart has been a beacon of light in trying to help health care providers help those who party with drugs. In this session, Wip has to prepare a talk for Monday where he teaches on “Party and Play”. Addiction Psychiatrist Dr. Wiplove (Wip) Lamba and Addictions Case Manager and former addict Jean-Paul (JP) Michael are in Saskatoon, Saskatchewan for the Canadian Society of Addiction Medicine (CSAM) annual conference. Wip loves this conference. When he started doing addiction medicine, he felt like he was in a bubble. Providing psychiatric care to people who use drugs or addiction medicine care to psychiatric patients may seem like commonplace, but usually those two fields are siloed. CSAM connected Wip to a community of health care providers that were really trying to figure out how to best serve people who use drugs. It's JP's very proud to be presenting for second time at CSAM, although this is his first in-person opportunity. For those of you in health care, it may be worth considering joining as well.
