Around 25% of women will experience a longer than anticipated early labour phase. Some maternity care providers might explain this as ‘false’ labour… but I can assure you it’s not false, it’s called ‘prodromal labour’ or prolonged early labour. You are in labour, but for some reason your body is not making visible signs of progress towards birth. Why does this happen? There are lots of reasons why you could be having a long early phase of labour… it can be completely normal and there is lots you can do to resource yourself for this labour marathon. The first two weeks after birth have a lot going on… So I'm doing 2 episodes on it. Here you will learn what recovery can look like for both mother and baby. I discuss the wide range of postpartum experiences, the care gap that can exist after birth, and how support from midwives, nurses, GPs, partners, family, friends, and the wider community can help with feeding, recovery, and emotional wellbeing.

I’m also going to explain matrescence, the transition into motherhood, and the physical, emotional, and identity changes that can happen after birth. This episode covers practical early postpartum advice, including skin-to-skin contact, feeding, rest, wound healing, bleeding, swelling, afterbirth pains, and what to watch for if there are signs of infection or heavier-than-expected blood loss.

I give guidance on the first wee and first poo postpartum, as well as the importance of food, fluids, and support in the early days. The first days can feel intense, but we adjust and things usually become more manageable.

If you get a swollen cervix during labour, your care provider might recommend an epidural or caesarean section. There are many other things you can try before resorting to such extreme measures and this episode covers them all. Interestingly, there are no research papers AT ALL to substantiate any of the strategies to remedy a swollen cervix so it’s worth trying them all to see what works, if you want to avoid an epidural or caesarean just because of your swollen cervix

Precipitous labour and birth can be a real surprise if you were mentally preparing for a longer labour. I came up with this Mastitis protocol early in my career and it’s just always worked. For this reason I have had no inclination to update or change it, because it has proved it to be effective. The umbilical has a purpose in the minutes following your baby’s birth. Leaving it intact enhances the health of your baby and assists in the transition to breathing air. A rebellion can take many forms, for Diane Lockhart her brand of rebellion involved her reluctantly and somewhat accidentally opening a birth center in a Ugandan slum. Amani Birth Center has now been running for 10 years and Diane shares how they have managed to care for women without the loss of a single mother or baby at the birth center. The UK used to be held up as an idyllic maternity care system worth replicating, but in the past 10 years, the caesarean section rate has increased to over 45% and poor maternal outcomes are rising as a result. Midwives are leaving in droves, birth services that once facilitate physiological birth are closing and the whole system is becoming hostile toward normal birth. There are people petitioning to put a stop to midwifery education from focusing on physiological birth which will fundamentally change midwifery in the UK relegating midwives to obstetric nursing roles. Unfortunately though many women are coached by their care provider to push a particular way. It doesn’t have to be like that. The evidence shows that you and your baby would be better off without a coach guiding your pushing or breathing during the pushing phase. On Aug 19th 2025 the world farewelled legendary Obstetrician Michel Odent. He spent his career trying to warn us of the generational impact of interrupting labour and birth and gave timeless advice on the practice of authentic midwifery and obstetrics. Today in this Ode to Odent I give a broad sweeping overview on the work of Odent that I believe should be preserved and passed on to this generation. A new research paper is causing concern amongst the birthing community this week after a local newspaper said that the study shows that outcomes are better in private obstetric-led care compared to the public maternity care system. Routine vaginal examinations can cause more harm than good. Sometimes they are needed but most of the time they are not. Vaginal examinations are offered as routine surveillance during most births, but you have a choice. The RSV vaccine during pregnancy has just been approved for use in Australia and the Australian government is providing it for free to all pregnant women. Mel goes over the research about the vaccine's effectiveness and the results of the 3 studies done so far. There is limited short term data but no long term data since this is a new vaccine.

A birth keeper is a vague and obscure term used to describe some unregulated birth workers who attend births. No one except the birth keeper themselves really knows the full scope of what they do or what they are capable of because they prefer to work in secret and away from the gaze of society. In this episode Mel invites journalist Charlotte King to explore the stories of two families who hired birthkeepers and who now, having experienced the care of a birthkeeper, want to alert the public to their experiences. Charlotte makes clear that the families sought her out and asked her to report on and share their stories and her article for the ABC was written in collaboration with the families.

A birth keeper is different from a doula and different from a registered Midwife. This episode seeks to communicate the differences. We thank the families in this story for their bravery and resilience in sharing their experiences and seek to share these with respect to both the families and birth keepers involved.

In this episode B is back and we are talking about Pre-labour rupture (release) of membranes (PROM). This is when your waters break/release when you are full term (37 weeks or more), but your labour doesn’t start. We discuss the likelihood of you going into labour, the routine management of PROM in hospital vs at home with a private midwife. In this episode B and I talk about the place and purpose of vaginal examinations during pregnancy and birth. Almost all birthing women will be offered vaginal examinations but the research doesn't support routine vaginal examinations. Most women are offered vaginal examinations for the benefit of the system and not for themselves or their babies.

We discuss why it is a bacteria found normally in 20% of the female population, the difference between colonisation and infection, the risks during, pregnancy, birth and the first week of your babies life. This week, along with our guest Prof. Hannah Dahlen, Mel and B talk about how anyone (maternity care providers, women, families, birth workers, etc) can find good quality research without having to pay for it. We give secret hacks about getting access to research papers that might help you better negotiate your care options and we discuss how to interpret and use it once you've found it. As a bonus, Hannah breaks down the ARRIVE trial to help us understand why it is a useless research paper in the real world when deciding if induction of labour is right for you. Due dates - this week we talk about due dates, how they are calculated, how the timing of pregnancy has been medicalised, the shortfalls of 'estimated due dates' and the importance of knowing your due date in a world where 'post-dates' pregnancy is taboo. We also discuss how women who go beyond 41 weeks of pregnancy are 'cared for' by the system and navigate the pressure that women feel to have an induction of labour. We learn about the history of full term pregnancy and catch the bloopers at the end where Mel educates B on a few farm facts about animals and their due dates.
