Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Gregory DiFelice, an orthopedic surgeon at Hospital for Special Surgery who has spent close to 20 years repairing ACLs rather than replacing them.

He lays out Preservation First, his algorithm for sorting ACL tears by tear type and tissue quality, and the four lanes it runs through: repair, BEAR, augmentation, and reconstruction as a last resort. He also explains why the field abandoned repair decades ago, what the 1993 paper that buried it actually said, and why he thinks the current direction of ACL surgery is big, bigger, and biggest.

The second half is built for sports PTs. Dr. DiFelice drains every patient's knee at the first visit, shares the data his group presented on what that does to range of motion and quad firing within 10 minutes, and explains why he moved his protocols off the calendar and onto milestones. He is also candid about the cohort where his own published failure rate was high and what he changed because of it.

If you rehab ACLs, this conversation will change what you think you are treating. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Kevin Johnson, the trainer behind Team Ascension in Virginia, whose client list runs from middle school athletes to All-Pro NFL talent.

Kevin's path into the profession is unlike any guest we have had on. He walks through the two and a half years he spent incarcerated, where a thousand push-ups a day and a deck of cards turned into a training system and a following of inmates who wanted in. That became the blueprint for everything he built afterward.

The conversation gets into how he landed his first pro client in a Chuck E. Cheese, why he told that athlete no when he showed up late, and how holding that line earned the respect that built the business. Kevin and Yoni compare notes on setting a precedent with your first big-name client, the gratitude mindset behind training someone in a cave with two rocks, and how Kevin assesses force absorption before he ever lets an athlete chase speed.

If you are a clinician, coach, or trainer trying to build something real, this one is worth your time. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Justin Farnsworth, founder of Rebuilt Strength in Rochester, New York, a physical therapist who spent over a decade coaching in the fitness industry before PT school.

Justin makes the case that rehab is training, and that most clinicians were never taught to program. They get into the characteristics rehab tends to skip: power, which lives at 3 to 5 reps rather than 3 sets of 10, strength, which requires real strain and not just fatigue, and hypertrophy, which comes down to mechanical tension and proximity to failure. He also rethinks activation, breaking down yielding and overcoming isometrics and where potentiation belongs in a session.

The conversation covers his pre-fatigue hack for return to sport testing, built with a leg press, a tape measure, and his eyes rather than a $10,000 machine. It closes on the business of physical therapy, and why understanding math is not the opposite of altruism but the thing that keeps the lights on.

If you are a clinician, coach, or new grad who wants a bigger toolbox, this one is full of gold. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Vikash Sharma, DPT, OCS, COMT, CFSC, founder of Perfect Stride Physical Therapy in Manhattan, a board-certified orthopedic clinician, certified strength coach, and run coach who has spent over a decade building an out-of-network practice niched entirely around runners.

They dig into the patient population that intimidates so many clinicians: the high-level marathoner who feels great until mile 23. Vikash explains why his first question is almost always about nutrition and fueling, not the tissue, and how a training block can expose a problem that was there all along.

The conversation also covers what "tendons love load" actually means in practice, from desensitizing an acute Achilles to progressing through heavy isometrics, eccentrics, plyometrics, and back to running. They get into strength programming across all three planes of motion, and the hard-won business lesson of working on the practice instead of in it.

If you treat runners, coach them, or are building a clinic of your own, this one is full of gold. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down for the third time with Jack Rasmussen, founder of Caddix Cleats, the company behind the Smart Studs that move with the athlete and decrease force up the kinetic chain.

Jack traces the whole arc: from making beds as a housekeeper and watching a player go down on an open field, to building a cleat now worn by nearly 100 NFL players, Team USA Lacrosse, and Olympic athletes. He and Yoni dig into the decisions PTs face too, including why he turned down $80 million in funding and a $20 million buyout on gut feeling alone.

It also covers the science a clinician can stand behind: the Rush University testing that showed reduced peak force at the ankle, knee, and hip, plus reduced hip adduction and knee valgus, the first step of an ACL tear. Jack is careful about that claim, and the precision is the point. They also get into why athletes are breaking down earlier than ever.

If you are a sports PT, a clinic owner weighing your next big decision, or building something from scratch, this one carries lessons on both the business and the biomechanics. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Marc Pietropaoli, MD, orthopedic surgeon out of Syracuse, NY, founder of Victory in Motion, and author of Repair Not Replace: Natural Joint Relief Without Surgery or Pills.

Dr. P walks through the regenerative medicine philosophy he built after training under Dr. James Andrews and Kevin Wilk: treat the body as something that heals itself, and only operate when nothing else will work. They get into the PRP literature, why dose matters more than almost any other variable, the difference between leukocyte-rich and leukocyte-poor preparations, and the case for treating the knee as an organ instead of injecting only intra-articularly.

The conversation also covers Dr. Hernigot's landmark bone marrow study (82% of severely arthritic knees never needed replacement at 15-year follow-up), how the Victory Method's Clarity Day combines physical exam, ultrasound, MRI, and a full hour with V Motion Fit, and the long-game mission to eliminate knee replacements by 2043 through prevention.

If you are a sports PT working alongside orthopedic referrals or fielding patient questions about PRP, biologics, and surgical alternatives, this one will sharpen how you vet providers and how you talk about regenerative care. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Odell Miller, former NFL athlete, businessman turned physical therapist, and owner of Off the Bench Physical Therapy.

Odell unpacks the cash versus in-network debate, why he chose a premium cash model from day one, and how he hit $51K months as a new grad inside six months. They get into the art of the handoff (how to position a clinic so it is not built around the founder), Gary Vee's "hire fast, fire faster" approach, and what to actually look for in an interview beyond the rehearsed answers.

The conversation also dives into Myron Golden's implementation ladder (implementation, management, communication, imagination) and why staying small and treating every patient yourself is doing the world a disservice. Odell shares his content philosophy (documentation over creation), how he lives in his van to reinvest cash into the business, the mentorship lens he uses to pick coaches, and why building leaders beats managing people.

If you are a PT student, new grad, or clinic owner thinking about scale, this one will fire you up. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Kevin Lynch, a brand new UMB graduate, former Division I lacrosse captain at the University of Delaware, NLL Philadelphia Wings alum, and the newest staff PT at the True Sports Bare Hills location.

Kevin walks through how he prepped for and passed the NPTE on the first try, why he believes the CSCS exam was more applicable to outpatient MSK than the boards themselves, and the gaps in PT school curriculum around movement deviations, exercise prescription, and the business of physical therapy.

The conversation also unpacks how to build and maintain professional relationships from PT student to first job, what truly separates a good outpatient PT from a great sports PT, and why coaching youth athletes is one of the fastest ways to level up your clinical communication. Kev closes with the Eric Cressey lightning round covering The Hard Hat, the Daily Stoic, and the PTs he watches outside True Sports.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Zack Atwood, owner of Primal Recovery PT and Performance, founder of the ACL Institute, and the creator behind The Football Doc on Instagram.

Zack walks through how he thinks about graft selection across age and sport demands, when an LET (lateral extra-articular tenodesis) earns its place in a primary repair, and why retear rates may be tracking better with modern hamstring + LET combinations. He explains arthrogenic muscle inhibition, why the calves get neglected in ACL rehab, and how focal joint cooling, NMES, and BFR change what is possible in the first 6 to 8 weeks.

The conversation also unpacks why limb symmetry index can mislead clinicians and parents, how peak torque to body weight gives a far truer picture of readiness, and why Derek Hansen's 10x10 protocol is replacing traditional run-walk progressions for field sport athletes returning to sprint.

If you are a sports PT, student, or clinic owner who wants to level up ACL rehab and return-to-sport decision making, this conversation will sharpen every phase of your program. Tune in.

Welcome to the True Sports Physical Therapy Podcast. Today's episode is a rewind of one of Dr. Yoni Rosenblatt's favorite conversations: Dr. Frank Benedetto, founder of the Honey Badger Project, a business incubator for healthcare clinicians modeled after Y Combinator.

Frank breaks down why grad schools fail clinicians on business, the consolidation and fragmentation cycle playing out in PT right now, and the lean startup principles that make bank loans obsolete for most new practices. He explains why your gym choice dictates your income ceiling more than any other variable, and how to progress from basic HEP apps into hardware-software integrated member journeys.

The conversation also digs into the money-versus-mission problem baked into PT education: why clinicians are taught that business is dirty, why that framing costs patients, and why a lean, ethical practice is the vehicle for impact rather than the obstacle to it.

If you're a startup clinician, a cash-based practice owner, or an established operator looking to add revenue streams, this episode will reframe how you think about business as the vehicle for mission. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Brent Brookbush, founder of the Brookbush Institute, longtime educator, and one of the most outspoken voices in rehab education.

Brent breaks down why so much continuing education feels evidence-based without actually being built on comprehensive research review. He explains what outcome-driven education really means, why original research matters more than recycled summaries, and how clinicians should think about expected value when choosing interventions.

The conversation also digs into the profession's bigger structural problems: why outdated board content survives for years, why many so-called experts are not reading research or measuring outcomes, and how that disconnect shapes what students and clinicians are taught.

If you're a sports PT, clinic owner, student, or educator trying to think more clearly about evidence, outcomes, and clinical decision-making, this episode will challenge how you evaluate rehab information. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Brett Morse, a sports performance coach out of upstate New York and three-time All-American from Cortland University.

Brett breaks down how to diagnose whether an athlete is force-dominant or elastic-dominant using vertical jump comparisons, split times, and reactive strength indices. The conversation covers how to train each bucket, when to prioritize hypertrophy versus max strength, and why overcoming isometrics are underused in both performance and rehab settings.

The discussion also dives into rotational power for overhead athletes, the link between lower body strength and throwing velocity, and how med ball work bridges the gap between weight room capacity and sport skill.

Whether you are a sports PT or a performance coach, this conversation will sharpen how you assess and program for speed and power. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Philippe Schafer, a male pelvic floor specialist who owns Schafer Physical Therapy in Union Square, Manhattan. Philippe is a University of Maryland DPT graduate, former Georgetown strength and conditioning coach, and former standup comedian.

Philippe makes the case for why sports PTs should care about the male pelvic floor. He walks through his full evaluation process from the base of the neck to the feet, explains the obturator internus connection between the hip and pelvic floor, and shares how he has resolved persistent groin and hip cases by looking at pelvic floor involvement. He covers strengthening progressions from Kegels to sumo squats to single-leg work, mobility approaches including low-load long-duration stretching and weighted RDLs, and why defaulting to Kegels can actually make things worse.

The episode also covers Philippe's journey opening a cash-based practice in Manhattan, the grind of knocking on 70+ physician doors, and the key lesson from standup comedy that every clinician should hear: complete composure, no matter what the patient says. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Austin Current, CSCS, CISSN, strength coach, personal trainer, and author of The Science of Strength Training, for a conversation that bridges the gap between personal training and sports physical therapy.

Austin breaks down his approach to calf and gastroc hypertrophy using undulating intensity across the training week: heavy low-rep days, high-volume endurance sets, and moderate hypertrophy ranges all targeting the same tissue through different stimuli. He explains why fatigue-resistant muscles like calves and delts need more loading variance than most practitioners give them, and shares specific coaching cues (inside of the foot, barefoot setup, driving the ankle forward) that unlock end-range plantar flexion for athletes rehabbing post-Achilles.

The conversation also tackles mind-muscle connection, where Austin lands between the two extremes and why setup matters more than sensation for most trainees. He closes with his take on what makes a great PT from a trainer's perspective: strength equipment in the clinic, frequent two-way communication, and treating every session like a podcast with your client.

Whether you're a sports PT trying to grow a post-op gastroc, a strength coach refining your calf programming, or a clinician looking to build stronger referral relationships with trainers, this one delivers. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Dr. Sean Sheridan, the clinician behind Return to Sport DPT, for a deep dive into what return to sport actually looks like when you strip away the fancy equipment and wide-open turf.

Sean breaks down his movement taxonomy for field and court sport athletes: acceleration, sprinting, deceleration, change of direction, and jumping/landing. He walks through how each pattern can be trained in a 500 square foot clinic using bands, medicine balls, metronomes, and creative positioning. The conversation covers his four-position framework for distilling athletic movement into limb relationships, Derek Hansen's A-run progressions for mimicking top-end speed in place, and how band resistance replicates deceleration forces without needing a runway.

One of the most practical sections tackles objective testing on a budget: why a $200 dynamometer pays for itself over a career, how to use Planet Fitness for 10 RM limb symmetry data, and which functional tests actually require nothing more than a tape measure and a phone.

Whether you're a new grad intimidated by return to sport, a travel PT working in general clinics, or a clinic director outfitting a space on a tight budget, this episode delivers. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with two True Sports clinicians, Dr. Zach Adams and Dr. Sydney Cope, for a master class on what actually makes a physical therapy session great.

Zach and Sydney break down the specific habits that separate average sessions from elite ones: how micro assessments during every exercise drive better outcomes, why celebrating small victories changes an athlete's trajectory, and what it looks like to be fully locked in with the person in front of you. The conversation also covers how to handle patients who walk in wanting the wrong treatment, from manual therapy requests when the research points to loading, to navigating the line between patient buy-in and clinical integrity.

One of the most practical sections digs into where students and new hires struggle most: mimicking their CI instead of developing their own clinical identity, going too personal too fast, and letting body language undermine credibility before a word is spoken.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Greyson Daviau, an athletic trainer at York College of Pennsylvania who brings a collaborative, patient-first approach to how ATs and PTs work together.

Greyson walks through the real overlap and divergence between athletic trainers and physical therapists: where scopes align in rehab, where ATs bring acute care and daily athlete access that PTs simply don't have, and why the grind of 60-70 hour weeks in a training room builds a different kind of clinical readiness. The conversation also covers how to manage multiple athletes at different stages of recovery simultaneously, and why front-loading education and autonomy is the key to making that work.

One of the most honest moments is the discussion around gatekeeping on both sides. Greyson breaks down why ATs sometimes view PTs as too conservative and why PTs sometimes feel shut out, then explains how direct clinician-to-clinician conversations at True Sports York have cut through those narratives entirely.

Whether you're a sports PT trying to build better relationships with athletic trainers, an AT navigating the evolving profession, or a clinic owner thinking about interdisciplinary collaboration, this one delivers. Tune in.

Welcome to the True Sports Physical Therapy Podcast. In this episode, Dr. Yoni Rosenblatt sits down with Andrew Coates, a veteran personal trainer with over 15 years on the gym floor and an interesting background that shapes how he builds client relationships, referral networks, and a sustainable in-person business.

Andrew shares why the human element of coaching matters more than social media clout, how being visible and engaged on the gym floor compounds into a referral tree that feeds your business for years, and why he believes the industry's push toward online coaching leaves many trainers worse off. The conversation also dives into cross-education training: the research-backed phenomenon where training a healthy limb reduces atrophy in an injured, immobilized one.

One of the standout moments is Andrew's breakdown of how to program around injuries using safety squat bars, single-leg machines, and creative sled variations to keep clients training through recovery rather than sitting out entirely.

Whether you're a personal trainer, physical therapist, or clinic owner trying to build a practice that runs on relationships rather than algorithms, this episode delivers. Tune in and rethink how you show up for the people in front of you.

In this episode, we sit down with Joe DeFranco, legendary strength and conditioning coach, founder of DeFranco’s Gym, and creator of CPPS and Team Forever Strong. Widely regarded as one of the most influential coaches in modern performance training, Joe joins Dr. Yoni Rosenblatt for a wide-ranging, honest conversation on efficiency, longevity, and what truly drives success in training, rehab, and business.

Joe shares his remarkable journey from training athletes out of a 500-square-foot storage closet to building one of the most recognizable brands in strength and conditioning. Along the way, he reflects on the lessons learned from personal injury, business mistakes, and decades of coaching elite athletes including NFL veterans, professional wrestlers, and lifelong clients who have trained with him for over 20 years.

In this episode, we cover Joe’s obsession with efficiency and minimal effective dose, why warm-ups should blend seamlessly into training sessions, and how listening, not talking, is the most underrated skill in coaching and physical therapy. Joe breaks down his philosophy on programming for longevity, why two high-quality sets may be enough for hypertrophy, and how overdoing volume often sabotages recovery, especially in rehab populations.

The conversation also dives into the blurred line between rehab and performance, highlighting why sports PTs and strength coaches must live in the “middle ground.” Joe discusses common mistakes PTs make when transitioning athletes back to strength training, why football conditioning tests are outdated, and what truly contributes to ACL and Achilles injuries at both the youth and professional levels.

Whether you’re a sports physical therapist, strength coach, clinic owner, or performance professional, this episode delivers timeless lessons on coaching, communication, programming, and putting the athlete first.

In this episode, we sit down with Dr. Tim Stone, Chief Operating Officer at True Sports Physical Therapy, former NCAA National Champion lacrosse player, and one of the most respected voices in ACL rehabilitation and coaching within sports physical therapy.

Dr. Stone shares his unique journey from collegiate athletics to clinical leadership, detailing how he balances executive-level operations across multiple clinics while still delivering elite-level care to athletes. This conversation centers on the art of coaching: what separates simply instructing exercises from truly coaching athletes through high-stakes rehab.

In this episode, we dive deep into how great clinicians create exceptional sessions by combining preparation, adaptability, and communication. Tim explains how to structure rehab sessions with clear goals, how to regress and progress exercises in real time, and how to read the room to meet athletes where they are emotionally and physically.

In this episode of the True Sports Physical Therapy Podcast, Dr. Yoni Rosenblatt sits down with renowned strength coach Tony Gentilcore to break down one of the most essential, and misunderstood lifts in the gym: the deadlift. From his early days of discovering the lift post-college to his evolution as a coach at Cressey Sports Performance and now founder of Core Collective, Tony offers a goldmine of insight into how to teach, modify, and scale the deadlift for everyone from elite athletes to reluctant gen pop clients.

Tony walks us through his progression strategy, starting with kettlebells and eventually building toward trap bars and straight bar deadlifts, if they even get there. He dives into assessment strategies like the hip scour, ankle dorsiflexion checks, and thoracic spine mobility screens to determine what each client is ready for. But beyond the biomechanical nitty-gritty, Tony hammers home what separates great coaches from average ones: the ability to meet the client where they are, read the room, and stop over-coaching.

The episode also tackles the mistakes physical therapists and strength coaches often make when integrating the deadlift into rehab or performance programs. Tony argues for the value of loading early, even imperfectly, and discusses why certain coaching cues (like “squeeze the orange in your armpits”) are more effective than overloading the athlete with anatomy jargon. He even gives us his favorite underrated accessory exercises, his thoughts on spinal flexion under load, and how to coach "just enough" to allow natural movement correction.

Finally, Tony gives us a behind-the-scenes look at Core Collective, his innovative Boston-based gym that combines strength training with physical therapy, mental health, and other holistic services. He shares what went into designing a facility that blends coaching with comprehensive care, and offers a preview of his upcoming fall seminar. This episode is a must-listen for PTs, coaches, and anyone serious about helping others train smarter, not harder.

What makes a great PT from a surgeon’s perspective? Dr. Hugh House, a veteran orthopedic surgeon who operates across the upper and lower extremity, shares his answer in this candid and insightful episode. Spoiler: It’s not where you went to school or how many techniques you know. It’s how much you care, how clearly you communicate, and how well you teach.

Dr. House shares exactly what he looks for in post-op therapy, what red flags tell him a PT may not be pushing hard enough, and how small changes early in rehab can create big differences in range of motion and long-term outcomes.

He also explains why many surgeries are avoidable with the right lifestyle choices, and how orthopedics needs to evolve beyond the OR toward collaboration with PTs, strength coaches, nutritionists, and even psychologists. We touch on return-to-play protocols for total joints, the science behind early loading after trauma, and how to support patients struggling with weight, motivation, or fear of movement.

If you’re a therapist who wants to understand the surgeon’s mindset, this is one of the most valuable conversations you’ll hear.
