On the list is a 22 year old woman who requires a CT-guided biopsy of a left renal mass that was discovered incidentally on abdominal imaging. The patient was admitted today for the procedure and is accompanied by her parents who are her legal guardians and carers. A postoperative bed has been booked for the patient.

Severe cerebral palsy
- spastic quadriplegia
- moderate intellectual disability

Recurrent aspiration pneumonia
- severe gastro-oesophageal reflux
- oropharyngeal dysphagia
- gastrostomy feeding tube (feeds ceased for six hours)

Anxiety
- distress with medical procedures

Epilepsy

Medications
- Baclofen 10 mg tds
- Levetiracetam 1000 mg bd
- Pantoprazole 40 mg once daily

Allergies
- Nil known

Observations
- height 155 cm
- estimated weight 40 kg estimated
- (BMI approx. 16.6 kg/m2)
- HR 90 bpm
- BP 95/55 mmHg
- SpO2 96% on room air

Preoperative Investigations

You are the duty anaesthetist at a busy tertiary hospital. Your provisional fellow calls you for
urgent assistance with an airway emergency.

They have performed a rapid sequence induction and attempted to intubate a 22-year-old man
for a Per-Oral Endoscopic Myotomy (POEM) procedure for type 2 achalasia in the main
theatres.

Intubation was attempted with a videolaryngoscope and hyperangulated blade. The percentage
of glottic opening visible was 10% and they were unable to pass the endotracheal tube or a
gum elastic bougie. They were then unable to bag-mask ventilate the patient but successfully
placed a second generation supraglottic airway. Ventilation has been restored, and green
tinged fluid has been noted in the gastric port.

Current observations are:

HR 95 bpm
BP 97/56 mmHg
SpO2 90% on FiO2 1.0
ETCO2 41 mmHg (5.47 kPa)
TV 400 mL with a small air leak

Weight 169 kg
Height 199 cm
BMI 42 kg/m2

Past Medical History

Type 2 achalasia
Class III obesity
Attention deficit hyperactivity disorder
Mild developmental delay
Severe anxiety

Medication

Lisdexamfetamine 70 mg once daily
Melatonin 4 mg nocte
Diazepam 10 mg was given orally preoperatively

Allergies
Nil known

She is a Fellow of the Royal Historical Society and the current Wood-Library Museum Laureate of the History of Anesthesiology.

You are a resident on an anaesthesia rotation and have started to anaesthetise a 50-year-old female for an elective laparoscopic cholecystectomy. She has a BMI of 40 but is otherwise well.

You are an intern on evening ward call. You attend a MET call and when you arrive, you're the first responder. The ward nurse reports that the patient is 70 years old, had a vaginal hysterectomy 2 days ago and is now tachycardic and hypotensive 80/55

Conclusion: The estimated EC(95) of remifentanil Ce for smooth nasotracheal fibreoptic intubation with conscious sedation was 3.38 (95% CI 2.90-3.46) ng·mL-1 when used in combination with midazolam and topical lidocaine. Remifentanil TCI may provide a tolerable experience of awake fibreoptic intubation despite the high incidence of recall.

Fat embolism syndrome:Distinct pattern of clinical symptoms and signs following fat emboli,
- more frequent in closed fractures
- mostly associated with fractures of long bones. (1-3% single long bone or 33% bilateral femoral)
- overall mortality 5 – 15%
- typically presents 24 – 72 hours post injury (rarely 12 hrs to 2 weeks)
- classic triad: respiratory changes, neurological abnormalities, petechial rash (not always present)
- other clinical features include pyrexia, tachycardia, right heart strain, myocardial depression coagulopathy & renal changes

Bone cement implantation syndrome:· No agreed definition.
· Characterised by hypoxia, hypotension, arrhythmias, increased PVR & cardiac arrest.
· Usually occurs at one of 5 stages:
Femoral reaming
Acetabular and femoral cement implantation, insertion of prosthesis, or joint reduction
· Fat embolism seems to be the most likely aetiology

The CaseYou are called down to review a patient for the evening emergency list in your tertiary hospital.

60yo female with SBO for laparotomy, looks unwell

A: speaking

B RR 30 sats 92% on RA 98% 6L HM O2.

C 2 16G iv. HR 130, BP 100, 2L CSL given.

D GCS 15. In 7/10 pain. Total 10 of morphine iv given by paramedics and ED in last 2 hours. Temp 38.5deg. Broad spec antibx given.

E. 80kg. 150cm. BMI 35. Large goitre.

How do you treat this?

This is very uncommon and I would always consult endocrinology to advise!

o Supportive

o Cool patient

o Hydration

o Oxygen and ventilation

Reduction of synthesis of thyroid hormone

o Dexamethasone 4mg iv q8hr or hydrocortisone 100mg iv q6-8h

o Iodine: sodium iodide (1 g IV or Lugol's solution 4–8 drops orally q6–8h) or supersaturated potassium iodide solution (5 drops orally q6h)

o Iv methimazole (20mg 4-6hrly) carbimazole 20-30 enterally tds

o Propylthiouracil (200-400 mg o q6-8hrs

Reduction of peripheral conversion of T4 to t3

o Propylthiouracil

o Hydrocortisone

o propranolol 80mg o 6hrly or 5mg iv. Over 10mins repeat 3x prn.

Decrease metabolic effects of thyroid

o Beta blockers

o Propranolol, esmolol

o Caution in COPD.

o Maybe iv diltiazem

o plasma exchange
