As Dr. Bob Barkley once said in the 1970s, "The goal of dentistry is to make the patient worse at the slowest possible rate". Dentistry is not a perfect science. Any time we have to touch a tooth, all we are doing is slowing down a train already in motion. So the #1 mistake you can make as a dentist is to be a bad communicator. You will always fail someone's expectations IF you haven't properly managed them. This podcast is my letter to anyone who needs to hear this, and can hopefully save them from learning the hard way as many dental professionals do.

I recently received a text from a good friend who was hearing from other colleagues in her area that her friends are "having trouble with the economy" which has led to their chairs being empty. I gave this a lot of thought and decided it could make a great podcast. Because while the economy can play a factor of the equation, it is never the whole picture. I believe that if you look at what you can control, you might be shocked to find that there are many things you can do to keep your chairs full that have nothing to do with the economy. Most of us do not suffer from having too few clients, the problem is one of quality not quantity. And we will sometimes attempt to compensate by adding more clients.
If we are to build a lucrative clinic with patients who appreciate our work, we need to work at creating great patient relationships. These bonds are formed when we educate and find out what the patient cares about. If the patient doesn’t care about a problem, chances are you haven't engaged them enough. This approach does a couple of things:
1. Find more of the treatments that patients need.
2. Find patients who want treatment.
3. Save time when the Dr is in the room - patient already engaged - has co-identified and “sees the problem”.
4. Build trust and rapport with patients.
5. Utilize the value of the technology we have invested financially in, in order to help more patients.

These types of clinics are on the rise in rural areas so that these communities can attact various health services that they would otherwise not be able to draw in. Probably one of the most common cosmetic issues I see that can throw off the harmony of a smile is uneven gum tissue. It's often hard for a patient to identify as the root cause which leads to them being disappointed in their smiles even after undergoing Invisalign or previous bonding. Nothing feels worse than spending hours meticulously creating beautiful cosmetic dentistry for your patient, sitting them up, and not getting the reaction you had hoped for. Even though I have been doing DSD for years, my AHA moment was only 5 months a go when I skipped my smile mock-up after a patient finished Invisalign and had the worst reaction to a smile make-over I have ever experienced in my career. While we were able to save the appointment and the patient ended up loving her smile on her adjustment appointment, it really highlighted for me the need to prepare patients with a mock-up ahead of time. So, in light of my mistakes, I will be discussing:
- My experiences with different styles of trial smiles.
- The non-DSD method for showing a patient their future smile.
- The advantages of taking this approach for you and your patient.

A happy patient means a happy day, and long term, a happy career. The dentistry itself is not the game….managing the patient expectations is the game. In fact, no amount of good dentistry will solve a poor patient understanding.
Biggest problem in dentistry is emotional responses to situations and the communication (or lack thereof) with our patient and our team.

This episode is a real example of how important is it to consider the whole patient and really test their understanding of what they know BEFORE we do treatment, especially in the esthetic zone. I use this example because within our practice we did it both wrong (initially), and then, right! I think it’s a worthwhile lesson for all!
And this topic is not just important for the Doctors but invaluable for your entire team too! As much as we want to help solve problems as they arise, patients need to understand their health and take ownership.
From the disgruntled call at the front desk, to the complaints to the manager, and even the loss of patients, much of it can be avoided if we learn to step back and clearly listen to one another.
This does many things:

Probably one of the biggest decisions I’ve had to make about my clinic in a long time was whether or not to rebrand. It’s a difficult decision for many dentists because as you change and grow in your profession, your values and passions change as well. It’s very important that your brand represents the atmosphere and patients that you want to connect with. Choosing to rebrand is a struggle because you want your brand to speak to who you are as a dentist and what you want to promise your patient but without losing the brand loyalty you have already created. Today we are going to talk about consultations for children. Gaining their trust and their interest is always harder than a typical consult. Using analogies and simple words and terms they understand is extremely helpful.
The key I’ve found is that including the child in treatment discussions leads to better success in the long run, no matter what the treatment.
It also helps the parents trust you as a provider when they see you explaining things at their child's level and sometimes even helps them better understand treatment too.
So in today's episode I will go over the verbiage and dialog I used in a recently performed Invisalign consult for a 9 year old girl who was with her mom. Her name was Olivia and she was definitely on the shy side.
Although mom already trusted me, I really wanted to make sure I had the trust of Olivia too. Clinically she was a Class 2 Division 1 case with excessive overjet, blocked out canines, narrow maxilla, and a history of snoring and puffer use.
This was DEFINITELY a case requiring early treatment , and especially because she was female, we wanted to get ahead of her growth upcoming spurt. Convincing mom and child to go ahead with treatment now was especially important when timing was critical.

Myofacial Pain patients can be the most challenging types of patients to treat and many doctors refer them to oral surgeons and other specialists not knowing what to do. The reality is, we do not receive a lot of experience in this area in dental school except to recommend a “bite plane”, which, after years of treating pain patients, is now what I barely consider a ‘bandaid at best’.

Lately, there have been a few times when I have either been unfairly judged or could have unfairly judged someone else. This is always a tricky problem to navigate especially when you think you know what’s best for the patient and that your next steps could ‘hail you as a hero’…..or does it?
Is it really good practice to not be too quick to judge another’s work, particularly when you don’t have all the information, and when there may be more at play than what is currently before you?
Although it’s really tough to not jump on another colleague’s past dental treatment, at the end of the day, what you say to that patient is super crucial in how the public sees us as professionals. Unless there is a pattern of repeated negligence or harm, how can one doctor be judged based on one case?
Frankly, I don’t believe anyone has a flawless track record, seriously, no one. This episode considers helps us consider how to be kinder to each other and protect the image of the profession at the same time. Let me know your thoughts!

It is my absolute pleasure to introduce and interview my (now 7 months) associate, Dr. Penny Dhaliwal (@dr.dhaliwaldds on IG), and her experience finding me and my clinic.
It’s been a joy to have Penny in the clinic and we recorded this on the fly because we were hanging out on the weekend practicing our Taekwondo (yes, I roped her into becoming a ninja in real life, not just in the clinic ), and the subject came up about a recent interview she did to help new grads looking for associateships. April is that time of year when new grads are desperately looking to find the ‘practice of their dreams. Associating prior to owning is a crucial first step into the real world of dentistry, and as if that wasn’t scary enough, new grads receive no coaching on how to go about it. What I didn’t know was how much work and thought Penny put into finding the right practice to begin her career. After questioning her about it, I realized how this can be an overwhelming and difficult decision for many 4th-year dental students, not to mention for the principal dentists as well! So we wanted to help! By sharing our experiences on what we both went through, we feel listeners will gain great insight on where to start, what to look for, and key ways to help guide the decision-making. We believe this episode (although longer than my usual episodes) is suited for both new grads and hiring principle dentists. So enjoy! And let us know what you think! Ideas of topics to ask the principal dentist during an interview include:
- How much mentorship can they provide?
- What materials and workflows are currently in use?
- What is the clinic’s team culture (this can be also interpreted by asking to speak with some of the existing employees or associates of that clinic).
- What did they learn from having a previous associate and would they do anything differently?
- How digital and modern is the clinic? (This can also be inferred from the presence of a good website).
- What type of business philosophy do they have and where are they in their career? Such as are planning to stay for a bit and how much time do they think they can contribute to mentorship, especially in the first year).
- In what ways are the clinic marketed and what are their expectations to gain new patients?
- What type of payment will they receive? (eg. Collections vs Production).
- What expectations do they have in the first few months as far as speed and production?
- Will they have to do dental hygiene therapy and if so, how much.

Commonly referred to as ‘the hot tooth’, a patient who presents with one can never be totally trusted to make the decision on treatment or even diagnosis.
It is not as uncommon as we would expect that sometimes the patient is just in too much pain to be logical in their decisions or even correct about which tooth it is.

Today I share my experience with one of dentistry’s newest (and coolest) technologies, known as Dental Monitoring (DM).
We all know that consumer buying behaviour and expectations are changing. Patients want more convenient appointments and care that is customized. For over a year now, DM has allowed me to do just that! Dental Monitoring is a smartphone app that utilizes artificial intelligence (AI) technology, allowing doctors to monitor their patients' orthodontic progress remotely, with more detail and accuracy than even possible with the naked eye during an in-office visit. As a bonus, patients get shorter treatment times, weekly monitoring, and will not miss a beat, no matter how far they live or how busy they get.
Incorporating DM into my Invisalign workflow that has made a major positive impact in my ability to care for more patients, scale my case load and more effectively provide a level of care that is modern, and rewarding, for all parties. I'm not going to lie, going digital has it's downsides, but I wouldn't enjoy dentistry nearly as much if it weren't for the technology in my clinic. I felt it was important to share some of both the wins, and pitfalls, of digital dentistry, and I hope you can even take away some great tips that may help you embrace digital as well.
