On January 31st, 2023, Drs. John Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook livestream entitled, "Is Preimplantation Genetic Testing (PGT) Right for me?"
At the Fertility Institute of Hawaii, we offer a genetic carrier screening panel that checks for more than 250 inherited conditions that could be passed on to a baby. If both partners are carriers of the same abnormal gene, there is a risk of having a child with the condition. PGT is an option for couples who test positive for the same abnormal gene, where embryos are created and tested for the condition before being implanted in the uterus.

On November 290, 2022, Drs. Frattarelli, Karmon and Goulet hosted a Facebook LIvestream entitled, “Optimizing Male Fertility.
November is men’s health awareness month. Did you know that men can suffer from infertility issues, and that it is not just women? According to ASRM, approximately 40% of couples will have the male as the primary cause or a contributing cause to infertility. Topics that were discussed include the following:
- Male fertility evaluation and the use of a semen analysis, and what the results mean
- Risk factors that affect male fertility include weight, smoking, the use of certain supplements, avoiding extreme heat that can affect scrotal temperature, and the importance of maintaining a healthy diet
- The ability to preserve male fertility and what this means

Endometriosis is a condition in which the tissue that lines the uterus, “the endometrium,” grows outside of the uterus. This tissue can be found on the ovaries, the fallopian tubes, as well as other organs. It can cause inflammation, pain, formation of scar tissue, and adhesions, possibly leading to fertility problems.

During this session, the Doctors talked about: what is Unexplained Infertility, potential causes, tests, and treatments. Unexplained infertility is fairly common among infertility patients. After a full work up of both the male and female partner, approximately 15-30% of infertility is unexplained.
Various tests and treatments are involved in diagnosing unexplained infertility, such as Ultrasound, bloodwork, semen analysis, ovulation induction, intrauterine insemination, in vitro fertilization, etc.

On October 18th 2021, The Fertility Institute of Hawaii’s mid-level providers: Tricia, Anna, Lyndsey, and Jae hosted a livestream entitled: ”The FIH Team Approach to Care.”

On June 29th, 2021, Drs. Anatte Karmon and Emily Goulet hosted a Facebook Live Session entitled: “Donor Egg and Donor Embryo IVF – Information for Donors and Recipients,” with guest speaker Tiare Brown. Tiare Brown is the Third party Coordinator at FIH.
During this session, the doctors talked about the process and perspective for donors and recipients and answered questions such as “What is a donor egg cycle?” and “When does someone come in for a donor egg cycle?”
A donor egg cycle is when someone opts to use the eggs of someone else. Eggs are extracted, fertilized, and transferred into the patient or a gestational carrier.
They also explained and discussed: the reasons for a donor egg cycle, the difference between a fresh vs frozen egg cycle, the typical length of time to complete an donor egg cycle, common side effects, risks, costs, if donor eggs are covered by insurance, and also the anticipated success rate for pregnancy.

On July 23, 2021 Drs. John Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook live session entitled: “What happens in the Andrology and Embryology Laboratory,” with guest speaker Pauline Garrison. Pauline Garrison is the Embryology Lab Manager and Technical Supervisor of the IVF Laboratory at the Fertility Institute of Hawaii.
During this session, the doctors discussed the importance of “World Embryologist Day,” celebrated on July 25th, which was created to honor and recognize scientists in the field of reproductive medicine and is also the birthday of the first IVF baby, Louise Brown, born in 1978.
Pauline shared her typical day in the life of an embryologist and talked about the changes and trends in embryology since she started in 2003.

On May 26th, 2021, Drs. John L. Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook Live Session entitled: “IVF- What Do I Need to Know Before Starting?” During this session, the doctors answered questions and explained that In Vitro Fertilization is commonly referred to as IVF. IVF is the process of fertilization by manually combining an egg and sperm in a laboratory dish. When the IVF procedure is successful, the process is combined with a procedure known as embryo transfer, which is used to physically place the embryo in the uterus. They also explained and discussed: What are the reasons for IVF, the typical length of time to complete an IVF cycle, common side effects, risks, if IVF is covered by insurance, talked about what an embryo transfer is, and also the anticipated success rate for pregnancy. On March 24, 2021 Drs. John L. Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook Live Session about Endometriosis and Infertility. During this session, the doctors answered questions and explained that Endometriosis is a condition in which endometrial tissue (tissue that lines the inside of the uterus) grows outside the uterus. This tissue can be found on the ovaries, the fallopian tubes, the intestines, as well as other organs. It is frequently associated with painful menstrual cycles. Since tissue outside of the uterus goes through the same growth and breakdown cycles as the endometrium in the uterus, it can cause inflammation, pain, formation of scar tissue, and adhesions possibly leading to fertility problems. On January 22, 2021 Drs. John Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook Live Session about the Covid-19 vaccine. During this session, the doctors answered questions and explained the Covid-19 Vaccine in regards to fertility. Question such as: Can this vaccine affect future pregnancy? When should someone get the Covid vaccine and which vaccine? Can you still get Covid after getting the vaccine? What are common side effects after receiving the vaccine? Any effects on men (ie sperm count/quality) after getting the vaccine? What routine vaccines are recommended in regards to fertility? Many patients have asked if the Covid-19 vaccine is safe prior pregnancy. The Covid-19 vaccines by Pfizer and Moderna are novel mRNA vaccines with an efficacy rate of 95 percent. It is important to note that mRNA is not a live vaccine, it does not replicate or change your DNA, nor is it expected to have any long term genetic consequences. In addition, the mRNA vaccine does not harm fertility or reproduction, such as sperm and egg quality or count, and is safe to use if you are trying to conceive. The common side effects from the vaccine are pain at the injection site and flu like symptoms. Please talk to your physician to see if getting the vaccine is recommended for you. On December 16, 2020 Drs. John L. Frattarelli, Anatte Karmon, and Emily Goulet hosted a Facebook Live Session about Ovulation Induction. During this session, the doctors answered questions and explained that ovulation induction is when fertility medications are used to stimulate the release of one or more eggs from the ovary. Common medications used for an ovulation induction cycle include Clomiphene Citrate, better known as Clomid, as well as Letrozole. Injectable medications like a trigger shot, amongst others, are often used as well and may be best suited for certain patients. Side effects are typically minimal and it is only for a short course of treatment. This could include things like headache, hot flashes, or sometimes the development of a cyst. In other cases, there may be no ovarian or uterine response to medications, and treatment is then adjusted. The typical success rate for an ovulation induction cycle population-wide is 20%, always remembering that the process is a journey. Many times, this will be the first step in the family-building quest. Lastly, this segment also explains which patients specifically can benefit from this treatment and if it is the correct option for them, as well as the cost. Fertility preservation is more successful now than ever before. With the recent advances in technology, the Fertility Institute of Hawaii is able to cryopreserve embryos, eggs, or sperm for patients in Hawaii. On September 9, 2020, Drs. John L. Frattarelli and Anatte Karmon hosted a Live Session about Recurrent Pregnancy Loss, with guest speaker Dr. Greigh Hirata. Early pregnancy loss is a devastating but common complication that occurs in 15% of clinically recognized pregnancies. Approximately 2% of women experience two consecutive pregnancy losses, but the overwhelming majority of these women will have a subsequent live birth. Although most women will go on to have a baby after having a miscarriage, there are some tests and treatments that may diminish the risk of an additional pregnancy loss, which were discussed in great detail.
