In Europe, over 4 million cataract surgeries are performed each year. A major cause of suboptimal vision following cataract surgery is cystoid macular edema (CME). In patients with CME, fluid accumulates in the central retinal area, due to leakage of blood vessels. Multiple cyst-like (cystoid) areas of fluid appear in the retina and cause retinal swelling or edema. Symptoms of CME are blurred and decreased vision. Anti-inflammatory eye drops are used after cataract surgery to prevent CME. However, noncompliance with eye drops is a major issue and may compromise the effectiveness of treatment. Noncompliance is often unintentional and related to forgetfulness or incorrect instillation, particularly in the elderly cataract surgery population. Because many patients have difficulty with self-instillation of eye drops, formal or informal care is often required. Administrating anti-inflammatory drugs with an injection in or around the eye during cataract surgery may obviate the need for eye drops after surgery. To investigate the effectiveness of anti-inflammatory drug delivery in or around the eye during cataract surgery, we propose to conduct a study in which 540 patients scheduled for cataract surgery will participate. Patients will be randomly divided over 3 treatment groups. One group will receive standard anti-inflammatory eye drops after cataract surgery. The other groups will receive a single or a combination of two anti-inflammatory drugs by injection in or around the eye during cataract surgery. Before and after cataract surgery we will measure the thickness of the central area of the retina, and compare this between groups. In addition, visual acuity and quality of life will be evaluated.

We expect that administration of anti-inflammatory drugs during cataract surgery improves vision and comfort for patients and decreases costs. It may also alleviate the burden on homecare organizations.

The overall goal of the vRESPOND study is to provide a clinically feasible and cost-effective method to manage negative dysphotopsia (ND). The study aim to use MRI-based patient-specific eye-models to uncover the underlying optical origin of ND through ray-tracing simulations. They subsequently aim to develop a Virtual Refractive Surgery application for the prevention and treatment of ND.

In terms of specific research aims:

  • comprehensive clinical evaluation will be made of 60 ND patients and 60 pseudophakic controls both in terms of objective ophthalmic measurements and standardized questionnaires.
  • Based on these data, ray-tracing analyses will be performed using the patient-specific, MRI-based, eye-models to:
    1. elucidate the mechanism for ND
    2. determine the key aspects of IOL design to prevent ND
    3. develop the Virtual Refractive Surgery model to preoperatively select high-risk patients and guide to the optimal IOL-type to prevent ND for these high-risk patients

Microbial keratitis are one of the most severe emergencies in ophthalmology and a world leading cause of preventable blindness. Diagnosis is challenging due to the multitude of possible pathogens mimicking similar clinical presentations. Sensitivity of traditional culture-based assays usually ranges from 40 to 70 %.

Next Generation Sequencing (NGS) technology will be used to study the microbial communities directly in their natural environment, through whole genome sequencing, switching from cultured to uncultured genetic-guided diagnosis is the more appropriate diagnostic approach towards microbial keratitis. Samples will be obtained using scrapes, swabs and impression membranes and will be sent to the laboratory for NGS and CDM. The metagenomic tests will be performed on the samples using high-throughput platforms, such as Illumina HiSeq2500 and MiSeq.

Clinical impact will be revolutionary, because ophthalmologists will have information on the precise microbiota/microbiome of all samples rather than the 40% of those from which an organism has been isolated.

This study aims, for the first time, to identify changes in neural activity patterns after multifocal IOL implantation overtime and their relation with objective and qualitative aspects of visual function. Such information will provide valuable knowledge on the identification of therapeutic targets and of intraocular lens characteristics that are more likely to trigger neuroadaptation circuits effectively and, hence, lead to practical clinical use.
