Professor Jayashri Kulkarni is one of Australia's leading psychiatrists and researchers specialising in women's mental health. She is the Director of the HER Centre Australia (Health, Education and Research for Women's Mental Health) and Professor of Psychiatry at Monash University. Professor Kulkarni's work focuses on mood disorders across the female lifespan, with particular attention to the neurochemical effects of reproductive hormones on brain function, including perimenopausal depression and premenstrual dysphoric disorder. She is the developer of the Meno-D rating scale and has been a leading voice for over four decades in the recognition and treatment of hormone-related mental illness in women.

This is a supplementary episode to the GP Show's earlier conversation with Professor Kulkarni on perimenopausal mental health, recorded in the same sitting. In it, Dr. Ashlea Broomfield and Professor Kulkarni discuss what distinguishes PMDD from common premenstrual symptoms; the emerging evidence on trauma and gene expression as a mechanism for PMDD onset; why current guidelines default to SSRIs rather than hormonal treatment and the limitations of that approach; practical hormonal treatment options for women with and without contraceptive needs; and the role of trauma therapy alongside hormonal management. Professor Kulkarni argues that PMDD is a brain condition, not an end-organ condition, and that treatment should reflect that.

Dr Peter Brukner OAM is a world renowned sport and exercise physician who has played a leading role in the development of sports medicine both in Australia and internationally. Peter is the founding partner of the Olympic Park Sports Medicine Centre in Melbourne and Professor of Sports Medicine at LaTrobe University. He is a founding Executive Member of the Australasian College of Sports Physicians, where he played a key role in establishing sports medicine as a medical specialty in Australia. He has been recognised with a Medal of the Order of Australia in 2006, and the recent 2025 Senior Australian of the Year for Victoria. More recently Peter has become a passionate advocate of improving nutrition particularly in those with chronic disease such as type 2 diabetes. Host Dr Sam Manger is joined by Emeritus Professor Richard Hays, a well known clinical and academic GP serving rural and regional Australia. In the 1990s he became the Professor of General Practice and Rural Health at the newly established North Queensland Clinical School (NQCS) which allowed medical students to complete their training in rural and remote Queensland, setting the stage for what would later become the Rural Clinical Schools, introduced by the Federal Government in the early 2000s. Richard was appointed the Foundation Dean of the James Cook University medical school, becoming the first GP to hold such a position in an Australasian medical school. Host Dr Sam Manger is joined by Dr Johanna Lynch, an Australian General Practitioner who has spent the last 15 years working as a psychotherapist with adult survivors of childhood trauma and neglect. She founded a transdisciplinary trauma-informed clinic in Brisbane named Integrate Place which operated from 2009-2013 offering psychology, social work, trauma-informed yoga, art therapy and mental health nurse support. In 2019 she completed a PhD, in whole person approaches to distress, championing the sophisticated craft of generalist approaches to distress – integrating social science and biomedicine. This PhD, entitled Sense of Safety: a whole person approach to distress in primary care sought to integrate trauma-informed care into primary care. Dr Johanna Lynch works as a Senior Lecturer at The University of Queensland, teaching medical students about trauma-informed whole person care. She is an advisor to BlueKnot foundation, trains rural GPs as a Fellow of the Australian Society for Psychological Medicine and The Royal College of General Practitioners.

Dr Tim Senior has worked as a GP at Tharawal Aboriginal Corporation, the Aboriginal Community Controlled Health Service in Campbelltown, South-West Sydney for the last 18 years. Prior to this he has worked in Central Australia, the Torres Strait and the UK, each time in low-income communities. Tim combines his clinical work with work as the Medical Advisor for the Royal Australian College of General Practitioners (RACGP) in Aboriginal and Torres Strait Islander Health and has been heavily involved in the development of guidelines and resources for all GPs working with First Nations Australians. He established two specific interest groups within the RACGP, one on poverty, and one on environmental impacts in general practice. In 2023 he completed his Churchill Fellowship on how to investigate how to provide high quality primary care to disadvantaged communities in Australia.

Dr Zac Seidler is a clinical psychologist, researcher and leading men’s mental health expert. He currently holds dual roles as Global Director of Men’s Health Research at Movember and Senior Research Fellow with Orygen at the University of Melbourne. Through his research and PhD, Zac has advocated for a change in the way we think about treating men’s distress. As part of his research, he has uniquely focused on creating mental health services that account for masculinity. Professor Michael Berk is a NHMRC Senior Principal Research Fellow and is Alfred Deakin Chair of Psychiatry at Deakin University and Barwon Health. Dr Mark Horowitz is a training psychiatrist, split between UK and Australia, working in London as a Clinical Research Fellow in the NHS and an Honorary Clinical Research Fellow at University College London. He has completed a PhD in the neurobiology of depression and the pharmacology of antidepressants at the Institute of Psychiatry, Psychology and Neuroscience at King’s College London and for this received two prizes from the British Association of Psychopharmacology. Professor Judy Lowthian leads a multidisciplinary research team as Head of Research and Principal Research Fellow at Bolton Clarke, a national not-for-profit aged and community care provider. She is an Adjunct Professor with University of Queensland’s Faculty of Health and Associate Professor with Monash University’s School of Public Health & Preventive Medicine. Her early professional career was as a speech pathologist in rehabilitation following acquired brain injury, and in management of interdisciplinary child and adolescent health services. Judy is internationally recognised in the field of gerontological health services research in emergency care, hospital-to-home care transitions, frailty, cognitive impairment and dementia, loneliness, palliative care, and implementation of new models of care with a focus on patient and family experience.

Dr Alana Gall is a proud Truwulway woman, a Pakana (Tasmanian Aboriginal) from the north-east coast of Lutruwita (Tasmania). Alana is passionate about Indigenous peoples' holistic health and wellbeing globally. She has a background with degrees in nutritional medicine, Master of Research and a PhD. Alana is a Postdoctoral Research Fellow in the National Centre for Naturopathic Medicine at Southern Cross University, and an Honorary Research Fellow at both the University of Queensland and Menzies School of Health Research. She is the Co-Vice Chair of the World Federation of Public Health Associations Indigenous Working Group, and the Vice President-Elect for Aboriginal and Torres Strait Islander Health at the Public Health Association of Australia. Alana pioneered the use of individual yarns with a think-aloud component, called the ‘think-aloud yarn’, and co-developed the Key Principles to Co-Design with First Nations peoples, which have informed the development of Cancer Australia's Australian Cancer Plan and will underpin its implementation. She leads a research program that centres around First Nations Australians traditional medicines and healing practices, with the aims of protecting and preserving these medicines for future generations and improving accessibility for all First Nations communities across Australia.

Professor Julia Rucklidge is a Professor of Clinical Psychology in the School of Psychology, Speech and Hearing at the University of Canterbury and the Director of Te Puna Toiora, the Mental Health and Nutrition Research Lab. Professor Rucklidge has been researching the interface of nutrition and mental health for 20 years and has published over 150 empirical papers. She received her doctorate from Berkeley, and is currently an Associate Professor in Psychology at the University of Texas. Dr Christine Ball is an anaesthetist at the Alfred Hospital in Melbourne and co-manages a Master of Medicine (Perioperative) at Monash University. She is the 2020–2024 Wood Library-Museum Laureate of the History of Anesthesiology. She has been an honorary curator at the Geoffrey Kaye Museum of Anaesthetic History for thirty years.

Today we discuss her 2021 book "The Chloroformist", a fascinating read about the history of anaesthesia and detailed biographies of its pioneers including Dr Joseph Clover. Her book is available at all bookstores including Melbourne University Press.

Dr Alex Gyani is the Director of Research and Methodology for the Asia Pacific region at the Behavioural Insights Team, also known as the Nudge Unit. He has worked in a diverse set of fields around the world from domestic violence, energy regulation, health system efficiency, obesity, financial regulation, employment services and education. His PhD focused on encouraging people with anxiety and depression to seek treatment using a mobile phone app and encouraging psychological therapists to use evidence-based treatments. Associate Professor Kristin Neff is widely recognized as one of the world’s leading experts on self-compassion, being the first one to operationally define and measure the construct almost twenty years ago. Professor Sir Muir Gray, CBE, is a British physician who has held some of the most senior positions in public health, evidence based medicine and value in healthcare. Sir Muir has worked for the National Health Service in England since 1972. He founded the National Library for Health, and was the Director of the National Knowledge Service. After posts at The University of Oxford, UK, and the University of Sydney, Lorimer was appointed Professor of Clinical Neuroscience and Chair in Physiotherapy at the University of South Australia. Professor Sir Michael Marmot is Professor of Epidemiology at University College London, Director of the UCL Institute of Health Equity, and Past President of the World Medical Association.

He is the author of The Health Gap: the challenge of an unequal world (Bloomsbury: 2015) and Status Syndrome: how your place on the social gradient directly affects your health (Bloomsbury: 2004). Professor Marmot holds the Harvard Lown Professorship for 2014-2017 and is the recipient of the Prince Mahidol Award for Public Health 2015. He has been awarded honorary doctorates from 18 universities.

Professor Marmot has led research groups on health inequalities for over 40 years. He chairs the Commission on Equity and Health Inequalities in the Americas, set up in 2015 by the World Health Organizations’ Pan-American Health Organization (PAHO/ WHO). He was Chair of the Commission on Social Determinants of Health (CSDH), which was set up by the World Health Organization in 2005, and produced the report entitled: ‘Closing the Gap in a Generation’ in August 2008. Professor Marmot served as President of the British Medical Association (BMA) in 2010-2011, and is President of the British Lung Foundation.

In 2000 he was knighted by Her Majesty The Queen, for services to epidemiology and the understanding of health inequalities.

Dr Cathy Kezelman AM is a medical practitioner, GP, mental health consumer advocate and President of the Blue Knot Foundation National Centre of Excellence for Complex Trauma. She is also the past director of the Mental Health Coordinating Council (MHCC). In 2015 she was awarded a Member of the Order of Australia (AM) for her work in this important area.  She is an author of many publications in research but also the author of a memoir chronicling her journey of recovery from child sexual abuse: Innocence Revisited.

Dr Valter Longo is the Edna M. Jones Professor of Gerontology and Biological Sciences and Director of the Longevity Institute at the University of Southern California –Leonard Davis School of Gerontology, Los Angeles, one of the leading centres for research on ageing and age-related disease. Dr Longo is also the Director of the Longevity and Cancer Program at the IFOM Institute of Molecular Oncology in Milan, Italy. The Longo laboratory published key findings on a 5-day periodic dietary intervention called Fasting Mimicking Diet (FMD), and showed in randomized clinical trials that FMD reduces the risk factors and markers associated with ageing and diseases. Dr Longo’s most recent studies focus on the use of FMD interventions to activate stem cell-based regeneration to promote longevity. Professor Felice Jacka is Director of the Food & Mood Centre at Deakin University. She is also founder and president of the International Society for Nutritional Psychiatry Research (ISNPR) and immediate past president of the Australian Alliance for the Prevention of Mental Disorders. Professor Jacka’s current research focuses closely on the links between diet, gut health, and mental and brain health. Professor Lorimer Moseley is a clinical scientist investigating pain in humans. After posts at The University of Oxford, UK, and the University of Sydney, Lorimer was appointed Professor of Clinical Neuroscience and Chair in Physiotherapy at the University of South Australia. He is also Senior Principal Research Fellow at NeuRA and an NHMRC Principal Research Fellow. He leads the Body in Mind Research Group, based at the University of South Australia which investigates the role of the brain and mind in chronic pain. He has published over 310 papers, six books for clinicians and patient workbooks, and numerous book chapters/. He is a strategic adviser for PAIN, associate Editor for the Journal of Pain, the European Journal of Pain and the British Journal of Sports Medicine.

Professor Sir Michael Marmot is Professor of Epidemiology at University College London, Director of the UCL Institute of Health Equity, and Past President of the World Medical Association.

He is the author of The Health Gap: the challenge of an unequal world (Bloomsbury: 2015) and Status Syndrome: how your place on the social gradient directly affects your health (Bloomsbury: 2004). Professor Marmot holds the Harvard Lown Professorship for 2014-2017 and is the recipient of the Prince Mahidol Award for Public Health 2015. He has been awarded honorary doctorates from 18 universities.

Professor Marmot has led research groups on health inequalities for over 40 years. He chairs the Commission on Equity and Health Inequalities in the Americas, set up in 2015 by the World Health Organizations’ Pan-American Health Organization (PAHO/ WHO). He was Chair of the Commission on Social Determinants of Health (CSDH), which was set up by the World Health Organization in 2005, and produced the report entitled: ‘Closing the Gap in a Generation’ in August 2008. Professor Marmot served as President of the British Medical Association (BMA) in 2010-2011, and is President of the British Lung Foundation.

In 2000 he was knighted by Her Majesty The Queen, for services to epidemiology and the understanding of health inequalities.

Dr David Unwin is a GP based in the UK. He is a Royal College of General Practitioners (RCGP) clinical expert in diabetes, RCGP National Champion for Collaborative Care and Support Planning in Obesity & Diabetes, Ambassador for the All Party Parliamentary Group on Diabetes, Founder member of The Public Health Collaboration Senior Collaborator NNEdPro at Cambridge (Need for Nutrition Education / Innovation Programme) and NHS Innovator of the year 2016. He has published multiple times on his low carb cohort and in 2017/18 his practice saved £57,000 on drugs for type 2 diabetes, hypertension and other conditions by offering patients a dietary alternative to medications.

Professor Gregory Phillips is the CEO of Abstarr consulting and Professor of First People’s Health in the School of Medicine at Griffith University. Gregory is from the Waanyi and Jaru Aboriginal Australian peoples and comes from Cloncurry and Mount Isa. He is a leading change maker, thought leader and medical anthropologist. He developed an accredited Indigenous health curriculum for all medical schools in Australia and New Zealand, founded the Leaders in Indigenous Medical Education (LIME) Network, and co-wrote a national Indigenous health workforce strategy. He established the Aboriginal and Torres Strait Islander Healing Foundation Ltd in the wake of the federal apology to Indigenous Australians, has advised federal ministers on Indigenous health inequality and was honoured in 2011 with an ADC Australian Leadership Forum Award.

Dr Joseph Firth completed his PhD at The University of Manchester in the United Kingdom. His initial research projects investigated the physiological and psychological effects of physical activity in young people with mental illness. Since then, his research has expanded to cover multiple aspects of health behaviours (including physical activity, sedentary behaviour, sleep, technology and diet) and the interaction this has with mental illness and brain health - particularly in young people. Professor David Hunter is a rheumatology clinician-researcher whose main research focus has been clinical and translational research in osteoarthritis (OA). He is the Florance and Cope Chair of Rheumatology and Professor of Medicine at the University of Sydney and the Royal North Shore Hospital and Consultant Rheumatologist at North Sydney Orthopaedic and Sports Medicine Centre. He is ranked as the world’s leading expert in osteoarthritis on Expertscape.com since 2014. He is on the editorial board for Arthritis and Rheumatology, Osteoarthritis and Cartilage, Arthritis Care and Research and part of the review committee for OA for the American College of Rheumatology, EULAR and OARSI scientific meetings. Dr Hunter has over 400 peer-reviewed publications in international journals, numerous book chapters, is the section editor for UpToDate Osteoarthritis and has co-authored a number of books. He is also Co-Chair of the recent RACGP Osteoarthritis guidelines.

This was a thoroughly enjoyable and informative discussion around osteoarthritis pathophysiology, imaging, prevention and treatment. There was a bit of mic rustling when David speaks but this stops at ~11:25mins (to be exact!).

Dr Peter Tenni is widely recognised as one of Australia's leading clinical pharmacy practitioners and trainers with over 40 years’ experience. He is a clinical senior lecturer at the University of Tasmania, a Director of Medscope and Director of Consultant Pharmacy Services.  He is the lead author of the deprescribing guides from the PHN Primary Health Tasmania which we are going to discuss today and will be updated on the website after May 9th.  These cover a range of principles and specific medications from opioids, benzo, PPIs, statins, aspirin, antihypertensives, antihyperglycaemics and many more. 

I am very grateful for these because there is no shortage of guidelines on how to prescribe more medication but very few on how to get people safely and beneficially off medication in an evidence-based manner.

Professor Boyd Swinburn is Professor of Population Nutrition and Global Health in the School of Population Health, University of Auckland. He trained as an endocrinologist but is now a public health physician and conducts research on community and policy actions to prevent childhood and adolescent obesity, and reduce, what he has coined, the ‘obesogenic’ food environment.

He is Co-Chair of World Obesity's Policy & Prevention section and Co-Chair of the Lancet Commission on Obesity. He established the World Health Organisation's first Collaborating Centre for Obesity Prevention, at Deakin University in Melbourne and has also contributed to over 30 WHO consultations and reports on obesity. He leads an international network (INFORMAS) in over 30 countries to monitor and benchmark the healthiness of food environments and the implementation of food policies and actions to reduce obesity.

He is also the patron of the Australasian Society of Lifestyle Medicine and will be speaking at the upcoming June ASLM conference in Auckland.

A listener asked me to do an episode on "what do we do if patients refuse their blood pressure and cholesterol medication?". We live in an interesting time where medicine is rapidly evolving for lots of reasons and often discussions around lifestyle, nutrition and supplements are met with ideology and emotional reactivity. But we need to have a mature, calm and open dialogue about these things. We need to accept we all (myself included!) need to continue learning and the only way to do that is to be humble and curious yet sensible and look at the evidence critically. You may agree or you may disagree with some of these points, but appreciate it for what it is.

I must admit though I failed to raise the Mediterranean diet here and it should be noted (as we discussed in the diabetes episode) that this is a great whole foods way of life that is easy to stick to and enjoyable, and good at reducing CVD risk. Though what we are really discussing in this episode are diet approaches designed to reverse existing coronary artery disease.
