The long and short of it is that the three of us started getting together every Wednesday morning to drink coffee, take a break and figure out how we were going to make an impact (and how we could work together to do so). We called it 'Solving the World's Problems Coffee Hour'. We chatted about all kinds of ideas: research, new workouts, combining things, and crazy ideas we heard on podcasts. Then it dawned on us. We should start a podcast.
Yep, that is how we thought we might be able to make a dent into the worlds problems (it all starts with a dent, right?) So here we are now. Several months of planning, brainstorming and politely asking Kyle's friend Justin (the awesome AV guy) if he could help us out.
So what it's all about is that Clinically Pressed is going to be a traveling podcast and we will be recording on location. We are going to interview experts around the country from all health/nutrition/performance professions as well as include round table discussions covering major topics and questions. Our goal is to provide as much information as we can that is useful to viewers/listeners, and do it all at no cost.
We want to make the complicated simple and make it something you can apply to your life. We hope you enjoy! Support this podcast: https://anchor.fm/clinicallypressed/support

Shin splints are a real pain in the tibia. If you have ever taken up running as a new hobby or you have jumped back into activity a little too soon you have probably had this soreness along the front or inside of your lower leg. While it is a frustrating injury it is also one you can address in a preventative fashion and you can correct. It takes some time and patience but if you address all the factors it will be well worth it in the end. Focus on form, technique and rehab and you'll be good to go in helping correct this extremely annoying injury.POSE Running Book: https://amzn.to/2Ou3adBSubscribe: https://www.youtube.com/c/clinicallypressed?sub_confirmation=1#complicatedsimple #resultsthatgiveback
Being in administration doesn’t automatically make you a leader. Being a leader doesn’t always mean you’re in administration. An administrative position doesn’t necessarily mean a position of leadership and this can be key in how you communicate with your team and those who report to you. Busy seems to come with every work territory in this day and age. There are times when things can become so much it produces overload and different people handle that differently. Andy has worked in some extremely productive lab settings where there are constantly multiple research projects and associated writing occurring. Joel oversees two departments that help the athletic department achieve its goals. In this episode, we talk about what we do to manage when things might get overwhelming and things we would do differently if we might go back and be in the same situation. This research review looks at a study that utilized “research snacks” to see the effects on insulin, glucose and free fatty acids after meals throughout the day. These “snacks” consisted of ascending 3 flights of stairs as fast as possible every hour throughout the day. Each time would take between 10-20s. The results showed that for an overweight and obese group there was a decrease in insulin and free fatty acid levels, no changes in these levels occurred in a healthy group.Interesting concept to help control blood levels with a minimal amount of effort throughout the day. Weight certification processes have been in place in wrestling for quite a while and are there to try and help ensure the safety of the athletes that have to go through it. While the focus is safety there are still a lot of things that can go on that can put athletes at risk in the weight cutting process. This is becoming even more apparent as women’s wrestling is beginning to explode across the country.
This episode is the first of many featuring Dr. Eric Twohey a current fellow in sports medicine and our very own Dr. Andrew Jagim. This episode could have gone on for hours. Reflecting back on so many different things that I could have done differently in my career is almost daunting, but worth it to try and learn and shape a better career in the future. The lessons learned were not only clinical (but there were a lot of those). There were so many lessons that I should have learned earlier about interaction with patients, coaches, colleagues and others that could have made my life easier and provided better care for everyone. My hope with this one is to provide insight for any level of AT, but especially younger ATs so that they can identify areas that they can learn from earlier to help advance their career and knowledge faster than I did my own. I feel 5-7 years behind and if I can help others pick up their pace, it helps me do it with my own and makes the whole system better. The goal of any practitioner should be to continue to evolve and grow in their craft throughout their career. If you’re treating/training the exact same way you were 5-10 years ago you aren’t evolving and that could be a problem. In this mini-series I’ll be going through some of the things I “Wish I knew” earlier in my career and how I hope to get ahead of that in the future. The first subject is, strength and conditioning. I discuss both my own personal, wish I knew when it came to my own training and how much time was mis-used along the way and also the professional aspect of my strength and conditioning career. I get the distinct feeling I’m 5-10 YEARS behind where I could be and am in the process of catching up to ensure I’m doing it the best I can. Nutrition is the foundation of everything when it comes to performance and recovery. It is really hard to build a high performing athlete if they aren’t fueled in a way that allows them to do what they need to do.
In this episode, Rusty Murphy asks a lot of the questions he had as he was coming up as an athlete and what he has heard in his current work as a coach. Our very own Andrew Jagim PhD breaks down some of the key components that youth and high school athlete should focus on. We specifically don’t get into supplementation (future episode) but talk about why nutrition is the foundation from which an athlete is built upon. Check it out and let us know if you have any questions.

Performance training at any level can be complex, but it doesn’t need to be. With youth and high school athletes, there is often a feeling of wanting to do more as that can put you ahead. Hard work is definitely part of the process but it has to be smart as well.
In this episode, we discuss what traying could look like and why more isn’t necessarily better. A lot can be accomplished by sticking to the basics a lot can be accomplished in not only maximizing performance but also helping reduce injury risk. We discuss in depth what that might look like and how you can work it into your training.

In this research review we are highlighting what Greg Nuckols has put together around the impact of sleep (or lack of it) on strength. The two studies below look at how sleep impacts strength and the findings weren’t quite as big as you might have thought. We discuss what this means, how it applies and how you still want to focus on sleep to make sure it is a priority but also that you can still get work done if you fall short on any given night.1. You often see baseball/softball players warming up with a weight on their bat or just a heavier bat and trying to time out swings. The thought is too warm up and get everything moving and potentially swing their regular bat faster. But does it actually accomplish that? In this research review they look at the weight of a warm-up bat and how it might actually impact actual bat speed. The results might be surprising.Reference:Montoya, B. S., Brown, L. E., Coburn, J. In this Reserach Review, Dr. Jagim talks about split vs. full body training in a group of non-trained women that assessed the changes in some performance metrics at the beginning and at the end. The volume/amount of exercises were equalized over the course of the week with the split group having shorter workouts but more frequency compared to longer training sesssions but fewer for the full body group. The results found that both groups got stronger to the same extent, so ultimately what they found is that both types of workouts were effective. From this study it appears you can utilize both types of training and still focus on getting the results that you want.

Is training to failure the best way to train? Is going hard all the time truly going to help you get the changes that you are desiring? Dr. Andrew Jagim takes a look at a study that tested just that. They looked at how trainign to only having 1 rep left in reserve vs. 4-6 reps in reserve and how that caused changes in strength. The results may be surprising to some who live by the “go hard or go home” style of training. We are hoping to bring some potential solutions through Clinically Pressed but these are so key for both people trying to get health and fit but also for the people that serve them. We did a small mini-review to help identify and discuss them.

We had the privilege of talking to Dr. Guy Jodarski of Organic Valley. We cover so much information about nutrition in this episode. We talk organic vs. conventional, grass-fed vs. not, all the way down to soil quality. If you have ever thought about organic but couldn't make up your mind for multiple reasons we suggest you check this one out because the information is great and backed by science.
The hope is some of these ideas will help others get to that point of understanding sooner than I did.

The results showed that for an overweight and obese group there was a decrease in insulin and free fatty acid levels, no changes in these levels occurred in a healthy group.

Being in administration doesn’t automatically make you a leader. Being a leader doesn’t always mean you’re in administration. An administrative position doesn’t necessarily mean a position of leadership and this can be key in how you communicate with your team and those who report to you. Being able to remain humble and continue to focus on your team and developing their human capital will be a game changer in how you interact with staff and the people you serve.
We discuss some of the ideas around how you refer to yourself and your interaction with your team. Check it out.

This concept came up in a recent episode of Barbell Shrugged and it is the theory of nutritional triage. The basic premise is that if there is a deficiency in micronutrients the body will triage and move micronutrient usage to short-term needs while neglecting long-term requirements of that nutrition. This can be good in short bouts but over the long-term these sub clinical deficiencies can cause serious problems that may go unnoticed unless looked for specifically or until they become a major problem. This article highlights the importance of vitamin K.

It can be hard to get people to do things, but are there ways to try and get people more likely to do things? This book takes a look at that, can be ‘nudge’ people to make decisions that can help them and beyond. Opt in vs opt out, anchoring, fear of loss, systems and regression to the norm. In this short we talk about how these concepts can apply to health, fitness, and performance to help get better results and more buy in.

Plantar fascia pain is a real pain in the foot and can be difficult to manage. This study looks at how treatment of trigger points in the calf musculature can impact the thickness of the plantar fascia as well as the pain the patient is feeling. We discuss what this means and how you can apply to your own care and management of plantar fascia pain. Lots of good ideas coming out to this one that you can practically apply.

In this Clinical Insight we take a shallow dive into the Radiculo-Neuropathic Myofascial Pain Model. This model was heavily influenced by Dr. Chan Gunn. It is a very solid model in the evaluation and treatment of chronic pain as it takes into consideration a lot of key components that play a role in the production of chronic pain. We talk through how the combination of Cannon’s Law, denervation and other factors play a role in chronic pain based on how this model describes it. At the end we discuss what are some practical recommendations that you can try immediately to see if there is something you can do if you are dealing with chronic pain.
Check it out, leave a comment and we will continue the discussion as well as we can to keep helping make the complicated simple.

Greg is in the middle of his Master's degree to continue to increase his knowledge of how research works and how to best utilize it to get the results we are looking for.
We talk about how it is hard to fully understand research and even when you think that you do understand it there is no promise that how the statistics were run that what you are going in the conclusion is correct. Greg talks bout a lot of the ways to get around these pitfalls and what he and his team are trying to do in order to help set a better standard when it comes to the science of getting strong and performing well. Trying to convince someone that a certain technique, item or program is the most effective can be a daunting task. There are many ways in which to communicate the information but which way is most effective? Telling stories and testimonials? Or getting them the hard facts and statistics about why something works? In this research review we looks at that very question and provides some insight on when and where to use different types of information to help make your case.

Tim Ferriss breaks down many areas of health and fitness and how you can implement those ideas with the minimal amount of time required, i.e. the 4 hour body. Whether you are looking to drop a few pounds or gain a few pounds of muscle, there is a way to try and do it with the minimal effective dose. It is important to identify not only barriers to health and fitness but also the motivators that help people get to where they want to be. We are hoping to bring some potential solutions through Clinically Pressed but these are so key for both people trying to get health and fit but also for the people that serve them. We did a small mini-review to help identify and discuss them.
Motivators:
+Improve health/energy/body image/lose excess weight
+Social influences
+Reinforcement/assistance/motivational interviewing/problem solving communication
+Belief in benefit
Barriers:
+Time/No leisure time/motivation
+Lack of experience
+Unaffordable indulgence
+Environmental factors

Very little is new in the worlds of strength & conditioning/nutrition/fitness/performance yet, more and more seems to be coming out. How is that? Well most of it is in the interpretation, consolidation, and ultimately implementation of this information that can make it new and effective. It is ok for practitioners not to discover the “next big thing” if you can collect ideas and thoughts and ultimately apply it to the individuals you work with.
This can make you highly adaptable and useful to those you serve (or you should look for this in someone if searching for services). Being able to take different ideas and utilize them with the people you work with can make you most effective as you don’t force something on to someone when it doesn’t fit. We discuss in this Clinical Insight.

This research review looks at a study that utilized “research snacks” to see the effects on insulin, glucose and free fatty acids after melas throughout the day. These “snacks” consisted of ascending 3 flights of stairs as fast as possible every hour throughout the day. Each time would take between 10-20s. The results showed that for an overweight and obese group there was a decrease in insulin and free fatty acid levels, no changes in these levels occurred in a healthy group.
Interesting concept to help control blood levels with a minimal amount of effort throughout the day. Check it out.

The book focuses on a ‘growth’ vs. ‘fixed’ mindset and how they can impact your life. Fixed can keep you stuck and make the stakes seem higher than they need to be. A growth mindset allows for continued development and allows for adjustments to your approaches to continue to evolve.
Again, highly recommend reading this book to apply it to your life.

Di’kademe/: a division or contrast between two things that are or are represented as being opposed or entirely different.
Moving to one extreme or the other can be hard to balance, especially if you try and make two seemingly opposing views/perspectives try and mix together and champion both of them. This is where dichotomy comes from and the concept that you have to find balance. There are times to be aggressive but you have to be able to double back with caution because you can’t run yourself in only one direction and then immediately make a switch.
Recognizing and embracing the dichotomy of ideas is important to all professions but can have some serious implications in athletic training. It is important to recognize the wide range of differing ideas but also be able to balance those in order to be maximally effective. We discussing this episode.

In this part of the ‘Wish I Knew’ series we discuss nutrition. This topic can get in depth so quickly and there are so many facets of it that it could go on for hours, we tried to keep it short.
Nutrition can get really complex, very quickly and that makes it even harder to apply good principles to your life and to others you serve. Some of the major take aways I’ve learned over the years is you have to start with the basics and try and cover as much of those as you can to start. From there you can start looking into the complex but you have to remember “there is no free lunch in physiology”. Anything you choose as an intervention is going to impact other things.
Here is what I’ve learned.

Muscle stiffness can be correlated with muscle pain. There are many ways to try and lessen it and in this study, researchers looked at how using TpDN would impact muscle stiffness of three muscle sites (infraspinatus, erector spinae and gastroc). What they found was that the reduction in muscle stiff was immediate but transitory. It happened to reduce immediate after intervention but increased again at 24 hours.
This leads to potential impacts on intervention and when/where TpDN or other soft tissue work modalities could be useful in helping reduce muscle stiffness. It doesn’t lend to the idea that only these interventions will help maintain those changes. Interesting study and while it has its limitations (stated by authors) it opens up some interesting ideas for future treatment options.

There is still time to sign up for the 2nd Annual Youth Sports Symposium (check out our FB page or Mayo Clinic Health System's FB event page). This is an episode we recorded leading up to the initial one two years ago. The importance of Youth Sports Injury and how to hopefully prevent as many as possible.
Youth sports injuries happen, it's part of the game. Many of those injuries can be reduced or prevented with smart training and monitoring training loads. In this episode we sat down with Dr. Jake Erickson, a sports medicine physician in La Crosse, WI. Dr. Erickson details how overuse in youth injuries can have significant negative effects on a growing body. Dr. Erickson gives some simple and practical steps that both youth and parents can use to help protect youth athletes. These steps include basic, solid training along with monitoring how many hours athletes are being active.

Youth sports has taken front and center in the conversation of sports safety. Early specialization, endless hours of practice and how that can affect a growing body. Recovery is key for all athletes but especially for this group and nutrition plays a key role in not only helping fuel youth athletes but helping them recovery and stay health. We talk with Andrew Jagim PhD about some best practices that can be implemented when it comes to youth sports nutrition. Baseball players use all types of methods to warm up before they get up to bat. It is still common to see weighted donuts and other implements in the on deck circle as well as players swinging multiple bats at the same time. But does all of this add up to swinging the bat faster?
This study takes a look how swinging different weighted bats prior to a normal bat impacts the velocity of the swing. The results may seem logically counterintuitive but it makes sense if you really think about it. Check it out.

I hope there are some great takeaways from this one and it generates so future learning and discussion.

The hope is some of these ideas will help others get to that point of understanding sooner than I did.

In this episode, Andrew Jagim PhD breaks down what it looks like to eat like an athlete. A common pitfall to training at the highest levels (whatever they may be) is not consuming enough to fuel your body to achieve what you need. Dr. Jagim digs in deep here and helps lay a great framework for consuming enough calories to enhance both recovery and performance. From there, many options exist to help build off this foundation.

This was a game changer episode as it took us outside of the normal treatment and approaches we typically had done and stretched our though of comprehension. So insightful and after talking with Shannon, have to consider something more out there that helps shape all of us.
On this episode we have Shannon Amberg PhD and Reiki Master. This is the first of two episodes with Shannon as we try and scratch the surface fo what Reiki is and how energy healing works. We get into asking a lot of questions as Joel is relatively skeptical (even when trying not to be) when it comes to the body and energy and someones ability to actually read that energy.
Shannon does a great job breaking down what it is all about and how it can benefit everyone in one way or another. There are some extremely interesting connections and ideas that you can come away with when you really listen to the ideas behind energy and how it runs our worlds. Check it out.

In a world of extremes in health and fitness it can be a good reminder to mind the middle. There are so many things that can work but they don’t work for everyone, no matter what anyone says. If there was a one size fix all, we’d all be doing it and that would be the end. That hasn’t occurred yet. This is a friendly reminder of looking at due diligence that is necessary to make sure you are finding what works for you and not spending a fortune on it in the process.

There is so much uncertainty in the world right now. Being home, not working, scared of being sick. There are so many things that can bring stress especially if money is tight for the foreseeable future. With that, we have seen a lot of “scams” out there claiming immune boosts to fight COVID-19. While some of the things can help with your immune system there is no quick fix and trying to get people to buy things strictly on the fact that it can combat the virus is well…..wrong.
This can be supplements, workouts, specific foods and diets. We understand people need to make money for small businesses and that is all good. What we do ask is, don’t do it on the back of a worldwide pandemic. Be better.

When we got to talk with Riverside Corporate Wellness they highly recommended that we talk with their dietitian, Jordan Murray. Jordan focuses on customizing nutrition plans based on the wants and needs of his clients. We discuss how he juggles the latest trends in nutrition with sound practices and what he believes to be effective.
We also get into eating disorders and their prevalence. We were surprised to hear how common they are in the ‘real world’ and the varying degrees to which they can manifest. Jordan had so much great information and a lot of take aways from this episode. Check it out.

Fitness Lying Down takes a very unique approach to improving your overall fitness and they make it fun at the same time. The focus of FLD is movement first. Gone are a lot of the traditional movements with barbells and dumbbells (you won't find them in their facility) and those are replaced with bands, kettle bells and most importantly, the ultimate sand bag. Anthony and Cory have spent a lot of time thinking about what they wanted to accomplish with FLD and it shows and is highly effective.
