In this episode, we are joined once again by Jeff Abraham, CEO of Absorption Pharmaceuticals, the company behind Promescent — the first FDA-compliant, clinically supported delay spray for premature ejaculation. Jeff has spent more than a decade helping change the conversation around men’s sexual wellness, reducing the shame around premature ejaculation, and advocating for science-backed solutions for a problem that many men suffer through in silence.

Premature ejaculation is one of the most common sexual concerns men face, yet it remains one of the hardest topics for men to talk about. Shame, embarrassment, unrealistic expectations from porn, and myths about “mental weakness” often stop men from getting the help they need. In this conversation, Mark and Jeff break down why PE is not about being selfish, weak, or uncaring — and why many men who struggle with it would do anything to last longer and feel more confident with their partner.

In this episode, we are joined once again by Philippe Schafer, a doctor of physical therapy practicing in New York City who specializes in men's pelvic health. After his powerful first appearance on the show — where we explored the surprising role the pelvic floor plays in erectile dysfunction — Philippe returns to take us even deeper into territory that affects far more men than anyone talks about. With advanced certifications from the Herman & Wallace Pelvic Rehabilitation Institute and a background in strength and conditioning coaching, Philippe continues to bring the same rigorous, whole-body, root-cause approach to one of the most silently suffered sexual challenges men face: premature ejaculation.

Premature ejaculation is one of the most shame-ridden and chronically underreported conditions in men's sexual health — and yet, as Philippe explains, it is also one of the most fixable. This conversation breaks down what PE actually is, why so many men suffer in silence, and how pelvic floor dysfunction sits at the center of a problem most clinicians treat as purely psychological or neurological. Philippe walks us through his comprehensive head-to-toe physical assessment — covering the thoracic spine, lumbar spine, hips, core, and the three layers of pelvic floor muscles — and explains why tightness, weakness, poor breathing mechanics, and overall conditioning all play a direct role in ejaculatory control.

Erectile dysfunction (ED) and performance anxiety affect countless men, yet many suffer in silence—feeling ashamed, confused, or alone. As a certified sex therapist, I frequently hear the same core concerns: “Is this normal?”, “Is it in my head or my body?”, “Can I talk to my partner about this?”, and “Will I ever feel like myself again?”

The truth is, ED is incredibly common and often caused by a mix of physical and psychological factors. Even when a doctor says everything looks fine physically, stress, shame, and fear can disrupt sexual function. That’s where therapy comes in—not just to talk, but to understand the root causes and offer real, evidence-based tools for healing. Opening up to a partner can be tough, but silence only adds to the tension. Therapy can also help with communication strategies that bring couples closer together. The good news: ED is treatable. With the right support, men can rebuild confidence, restore intimacy, and redefine what "normal" means in their sex lives.

In this special episode of the Erectile Dysfunction Radio Podcast, host and certified sex therapist Mark Goldberg opens up about how he began helping men with psychogenic erectile dysfunction (ED) and premature ejaculation. Originally trained as a couples therapist, Mark began to notice a recurring, often unspoken pattern in his sessions—men quietly struggling with sexual performance, not due to a physical issue, but because of anxiety, pressure, and disconnection.

These conversations revealed a critical gap in support: while many men had seen doctors and tried medication, they still felt lost and ashamed. Mark realized that the traditional medical approach wasn’t enough. What was needed was a space where men could talk openly and safely about their experiences, and where emotional, psychological, and relational factors could be explored without judgment.

Sexual satisfaction is vital in relationships, but differing expectations can create challenges. Certified sex therapist Mark Goldberg discusses two key approaches: Good Enough Sx and Expansive Sx. Understanding these can enhance intimacy and address sexual dysfunction.

Good Enough Sex

This approach emphasizes routine and predictability. Individuals who prefer good enough sex find comfort in familiar experiences, leading to reliability and efficiency in intimacy. While this can strengthen a couple’s bond over time, it may also lead to sexual stagnation, boredom, and decreased arousal if variety is lacking.

Expansive Sex

In contrast, expansive sex involves exploration and novelty. Those with an expansive approach seek new experiences to maintain excitement and psychological engagement. While this can enhance pleasure and connection, it may also create pressure, especially if one partner struggles to keep up with evolving desires. This pressure can contribute to performance anxiety and even erectile dysfunction.

Today, we are joined by Rafi Billick, a couples therapist based in Baltimore and author of "Couples Communication Handbook: The Skills You Never Learned for the Marriage You Always Wanted." With years of experience helping couples at every stage of their relationships, from newly engaged to decades-married, Billick emphasizes that the foundation of all successful relationships lies in empathy. Many relationship experts believe that empathy lies at the heart of every successful relationship. Through years of clinical observation, therapists have discovered that the ability to truly understand and feel what a partner is experiencing creates the foundation for lasting connections.

As a sex therapist, I’ve seen how political differences can create stress between couples. When partners don’t agree on politics, it can lead to arguments and distance. That’s why empathy—being able to understand and respect how someone else feels—is so important. Even if you don’t agree, showing kindness and care can help keep your connection strong.

Mental health is deeply connected to sxual health. Challenges like anxiety and depression can directly impact sxual function, while unresolved s*xual health issues often exacerbate mental health struggles. The quicker individuals seek help, the better their prognosis. Prioritizing mental well-being is not just about avoiding catastrophic outcomes but enhancing overall quality of life.

Globally, healthcare systems face challenges in providing affordable, accessible, and comprehensive s*xual health support. For example, treatments for erectile dysfunction (ED) like PDE5 inhibitors (e.g., Viagra) are rarely covered by insurance, despite being widely recognized as essential for many. This lack of coverage creates barriers for individuals seeking treatment and highlights systemic inequities.

Patrick and Cameron are passionate about empowering men to explore their health and wellness beyond prescriptions and diagnoses. Together, they delve into the critical connection between mental health and sexual function, offering unique insights and actionable advice.

Mental health is deeply intertwined with s*xual well-being, particularly for men. The unresolved mental health issues such as depression, anxiety, and even stress from daily life can significantly impact not only a man’s ability to perform but also his overall desire and connection during intimacy. Today’s digital world can dull emotional and physical responses, making it harder for men to fully engage with their partners.

Erctile dysfunction (ED) can have various origins, including psychological and physical factors, yet many treatments tend to focus on the physical aspect alone. In clinical practice, shifting focus from achieving a “perfect” erction to enjoying intimacy can relieve performance anxiety and often leads to better s*xual experiences for both individuals and couples.

In this episode, we dive into Seasonal Affective Disorder (SAD), a form of depression that many experience, particularly during the winter months. This condition can bring on low moods, irritability, fatigue, and changes in sleep or appetite due to decreased sunlight and shorter days. For those affected, SAD often leads to a feeling of "hibernation," where energy levels drop, and motivation wanes.

The consequences of SAD are not limited to mood; they can also extend to sexual health. Depression and fatigue can significantly impact libido, lowering sexual interest and activity. It’s common for men to experience a decline in libido or even erectile dysfunction as energy and mood diminish. Furthermore, SAD doesn’t just affect individuals—it can impact relationships too. Partners may notice changes in sexual routine or emotional responsiveness, and this shift can strain intimacy.

Peyronie's disease, which affects up to 1 in 10 men, is often misunderstood and underdiagnosed. This condition causes penile curvature and deformity, but its impact goes far beyond the physical. Many men feel shame or inadequacy, even with mild cases. Interestingly, partners are usually less concerned than the affected individuals.

A growth mindset helps people see challenges as opportunities to learn and grow, even in difficult times. Instead of feeling defeated, those with this mindset look for ways to adapt and solve problems. This is especially important for men dealing with erectile dysfunction (ED), as it can prevent feelings of helplessness and frustration.

In contrast, a fixed mindset can lead to negative emotions like self-doubt and hopelessness. Men with ED who have a fixed mindset may believe nothing can improve their situation, which only adds to their anxiety and stress. This cycle of stress can make ED even harder to manage.

Today, we have the pleasure of speaking with Dr. Marah Hehemann, an assistant professor of urology and the Associate Program Director of Andrology at the University of Washington in Seattle. Dr. Hehemann specializes in male sexual function and fertility and also practices clinically at the university.

Hair loss can be a distressing experience for many men, leading them to seek out treatments like minoxidil (Rogaine) and finasteride (Propecia). However, these popular medications may come with potential side effects that impact sexual function. Minoxidil is unlikely to significantly affect erectile function when applied topically, as only about 1% of the medication is absorbed into the bloodstream. On the other hand, finasteride, which blocks the conversion of testosterone into dihydrotestosterone (DHT), has been associated with a 5-10% chance of negatively impacting sexual function, including reduced libido, erectile dysfunction, ejaculatory problems, and decreased sexual pleasure. These effects have been reported with both the higher 5mg dose used for benign prostatic hyperplasia (BPH) and the lower 1mg dose for male pattern baldness.

Today, we're joined by Natalie Finegood Goldberg, an LMFT, licensed marriage and family therapist, and certified sex therapist based in Los Angeles. With her extensive experience in resolving sexual dysfunctions, including erectile dysfunction (ED), Natalie sheds light on the intricate relationship between performance anxiety and ED.

Performance anxiety triggers a cascade of panic hormones, diverting blood flow away from the penis due to the fight or flight response. This physiological reaction can hinder erectile function, exacerbating the anxiety loop. Addressing performance anxiety involves unpacking false beliefs, broadening the definition of sex, and cultivating a healthy mind-body connection.

Today we are privileged to have Doctor Kevin Chu with us. He is a private practice urologist and men's health specialist in Los Angeles, California. With roots in Pittsburgh, Pennsylvania, he pursued his undergraduate studies at UC Berkeley before attending medical school at the esteemed New York Medical College. Dr. Chu honed his expertise in urology through rigorous training at the University of Miami.

Shockwave therapy, originally developed for kidney stones, utilizes acoustic waves to induce a response in targeted tissues. While the mechanism for kidney stones involves breaking them up, shockwave therapy for ED operates differently. It stimulates the body's natural healing response, promoting neovascularization and tissue regeneration in the penis.

Addressing common concerns, Dr. Chu clarifies that shockwave therapy sessions are non-invasive and generally well-tolerated, with minimal side effects such as mild tingling or bruising. However, he emphasizes the importance of patient selection, cautioning that not all individuals may benefit equally from this treatment.

Today we are joined by Dr. Ariel Moradzadeh, he is a board certified urologist practicing in Beverly Hills, California. He specializes in men's sexual health and sexual issues. He will enlighten us on the intricacies of vasectomy, vasectomy reversal, and their potential impact on sexual function.

A vasectomy, a brief office-based procedure taking 15 to 20 minutes, involves cutting, clipping, and cauterizing the vas deferens, the tube carrying sperm from the testicle. Contrary to common misconceptions, a vasectomy does not impact sexual function, erection issues, testosterone levels, or cause long-term testicular pain. Some individuals, after having a vasectomy, decide to undergo a vasectomy reversal. This is often due to remarriage or a desire to have more children with a new partner. The reversal process is a delicate four-hour operation conducted under general anesthesia and requires precise microscopic techniques. The success rate varies, diminishing over time from the original vasectomy.

These include the missionary position, straddling with the partner on top, doggy style, side lying or spooning, and various standing and sitting positions. Each of these positions can impact the male partner differently, depending on the physical demands and pressure on specific body parts.

The way the medical system makes decisions has changed in a noticeable way over time. The days when people just did what their doctors told them to do are over. Instead, there has been a change in the way people think about making decisions. This change is caused by a number of things, such as the fact that there are many ways to treat a given condition and the realization that each person's wants and preferences must be taken into account. Patients with erectile dysfunction can now choose from a wide range of treatments, each of which has its own pros and cons. Dr. Noble is a pelvic floor therapist who works with all types of people to resolve sexual function challenges. She has specific expertise in helping people address erection and ejaculatory issues. Pelvic floor is a group of muscles and tissues that support the pelvic organs, including the bladder, uterus, and bowel.

One of the important roles of pelvic floor is to contribute to the process of achieving and maintaining strong erections. During sexual arousal, the pelvic floor muscles contract, which increases blood flow to the penis and helps trap blood within the erectile tissue, resulting in a firm and sustained erection. However, weak or dysfunctional pelvic floor muscles may not contract effectively during sexual arousal, leading to difficulties in achieving or maintaining strong erections. The pelvic floor also plays a role in ejaculation. As the muscles contract during orgasm, they help propel semen through the urethra and out of the body.

Good pelvic floor health is essential for sexual function and overall quality of life. Regular exercise and pelvic floor muscle training can help maintain strong and healthy muscles, which can improve sexual function and prevent sexual dysfunction. Pelvic floor exercises, also known as Kegels, are a simple and effective way to strengthen the pelvic floor muscles.

Are you aware of the dangers that excessive porn consumption can have on your brain? In this episode, we will take a deep dive with Cognitive Neuroscientist and Certified Sexual Addiction Recovery Coach, Dr. Trish Leigh, into the effects that porn can have on the brain and explore how it can lead to erection problems. In this era of modern technology, access to porn has become increasingly prevalent. Nevertheless, what many fail to realize is that porn can have long-lasting negative effects on both the brain and the body.

Studies have shown that porn addiction can lead to changes in brain structure and function. In particular, it can cause desensitization to sexual stimuli, leading to a decrease in sexual responsiveness. This can result in difficulties achieving and maintaining an erection, known as erectile dysfunction (ED).

According to Dr. Leigh, the more we watch porn, the more our brains become desensitized to dopamine, requiring us to watch more and more extreme content to achieve the same level of satisfaction. This desensitization can lead to a decrease in sexual arousal and performance, ultimately causing ED.

Moreover, excessive porn consumption can also lead to anxiety and depression. This is because porn can create unrealistic expectations of sex and intimacy, which can lead to feelings of inadequacy and disappointment in real-life sexual encounters. This can further exacerbate the problem of ED, as anxiety and depression are also common causes of ED.

Watching porn can have significant effects on the brain, leading to changes in brain structure and function, and can cause erection problems. It's essential to be aware of the potential risks associated with excessive porn consumption and seek help if you or someone you know is struggling with porn addiction or ED. Remember, Your Brain on Porn can have lasting effects on your sexual health and overall well-being.

Urologist, Dr. Amy Pearlman, joins the Erectile Dysfunction Radio Podcast this week for a chat directed at couples looking for a more satisfying sexual experience and better sensation through the use of toys, devices, and products.

Dr. Pearlman is a urologist specializing in men’s health and genitourinary reconstruction. She joined us for a previous episode about Fighting Erectile Dysfunction. You can listen to that episode here: https://erectioniq.com/amy-pearlman/

This interview explores a host of different products that may help you have better sex, better orgasms, and even fight some forms of erectile dysfunction.

As we learn today, despite how some devices can assist with certain sexual dysfunction, it is important to remember that the use of sex toys in the bedroom does not mean there is a dysfunction present. The use of a sex toy can show that you care about your partner’s pleasure.

Advice given by Dr. Pearlman in this interview also includes encouraging partners to make the use of your sex toys about your partner’s pleasure, not just your own pleasure.

We are excited to once again be joined by erectile dysfunction patient advocate Skip.

Skip has dealt with the mental and physical sides of Erectile Dysfunction. When seeking help for erection issues, it is always encouraged to be checked out by a primary care physician or urologist before (or in tandem with) seeing a mental health professional to deal with the emotional damage brought on by this condition.

Skip previously joined us for the popular episode called “Why a Second Opinion Matters When Facing Erectile Dysfunction” and you can listen to that here: https://erectioniq.com/second-opinion/

As we learned in that episode, Skip had been facing erectile dysfunction for decades. He later found the right urologist to help him out and he eventually found a permanent solution to his erection issues.

He joins us once again today to chat about the mix of both mental and physical issues that led to and worsened his erection and relationship difficulties and how he found some lasting solutions.

You can get in contact with Skip by sending him an email: phdskip@gmail.com

Shared decision making (SDM) is often referred to as a communication process within the patient-doctor relationship. In an ideal situation, both patient and doctor make the healthcare decisions together based on an open dialogue where the patient’s best interests are discussed.

This is important because many treatment options for various medical conditions are nuanced. This means that a treatment that’s best for one patient may not be right for someone else. Even if it is the same condition.

When it comes to the various treatment options associated with erectile dysfunction, it is safe to say that a shared decision-making process is of vital importance.

This episode of our podcast is about erectile dysfunction treatment & shared decision making and it’s an interview with Dr. Amy Pearlman. Dr. Pearlman is a men’s sexual health specialist and board-certified urologist. She currently serves as Clinical Assistant Professor and Men's Health Program Director with University of Iowa Health Care.

We wanted to find out if being "extremely excited" for a sexual encounter could lead to issues like erectile dysfunction and premature ejaculation on this week's episode of the Erectile Dysfunction Radio Podcast. We also wanted to learn if something like "overexcitation" can be controlled.

From the perspective of a sex therapist, the answer to that question varies. Like most complex things related to mental health, there are many factors involved and there is a lot of helpful education to be gained by exploring this topic.

Today’s episode of the Erectile Dysfunction Radio Podcast explores masturbation while in a relationship and how to address the conflict it could cause between partners in an otherwise loving relationship.

Through feedback we have received on ErectionIQ.com’s email inbox, and by investigating men’s health forums on Reddit, we know that masturbation – and whether or not it is “healthy” to engage in while in a relationship is a recurring question we have seen.

The psychological fear of a penile fracture alone is enough to cause erection difficulties. With this in mind, we decided it was necessary to have an entire episode dedicated to this topic with urologist Dr. Justin Houman. Some of the questions we cover on today’s interview include:

  • How common is it to fracture your penis?
  • How do you “fracture” a penis if there is no bone?
  • What (if anything) can increase or decrease your risk?

As Dr. Houman reminds listeners in today’s podcast, a penile fracture is an emergency. Do not wait around if you suspect that you -- or your sexual partner -- has experienced a penile fracture, it should be dealt with immediately.

Dr. Houman is a urologist specializing in men’s health, including the treatment of men dealing with erectile dysfunction and penile fracture. He is based in Los Angeles California with Tower Urology.

Today we’re joined by cardiologist, Dr. Jeffrey M. Banker, for part three in our four-part series covering a case study that illustrates the complex nature of erectile dysfunction. This case study required a multidisciplinary approach to treating the patient’s erectile dysfunction.

Dr. Banker is a board-certified cardiac electrophysiologist and general cardiologist with LifeBridge Health in Baltimore, Md. His input to this conversation is vital because the patient featured in this case study reported having shortness of breath and fainting spells after sexual activity.

As a quick recap, our podcast host, certified sex therapist Mark Goldberg, has found that too many men assume their erectile dysfunction is caused by either a physical or mental factor. In reality, many erectile dysfunction cases are complex with both mental and physical factors contributing to the erection problems.

This is part three of the four-part series exploring a unique case study showing how both physical and mental components factor into erection problems.

You can listen to part one here: https://erectioniq.com/erectile-dysfunction-case-study-part-1/
