In this episode, Dr. Alice Hoyt sits down with Dr. Doug Jones — board-certified allergist, co-founder and president of FAST OIT (Food Allergy Support Team), and host of The Immune Edit podcast — fresh off the FAST OIT Conference in Dallas.

Dr. Jones shares the patient encounter that changed the trajectory of his career: a family facing five simultaneous food allergies, a room full of tears, and a doctor who left the appointment and put his head on his desk in frustration. That moment sent him on a path to find something better, and eventually to Dr. Richard Wasserman, OIT, and building one of the most influential food allergy treatment communities in the world.

Together, Dr. Hoyt and Dr. Jones cover a lot of ground — including the complicated history of why OIT took so long to reach patients, what happens when business models and patient outcomes don't align, and why the next frontier is getting this treatment into mainstream allergy training.

In this episode:
~Why telling families "just avoid it and carry your EpiPen" never felt like enough
~The family that changed Dr. Jones's career — and what he did after they left his office
~How connecting with Dr. Wasserman in 2012 set everything in motion
~The divide between private practice and academia on OIT, and why it's finally shifting
~Why Palforzia mattered even for doctors who weren't using it
~What FAST OIT's email list of 700+ allergists is doing to break down silos
~The importance of patient goals — and why "bite proof" means something different to everyone
~Why teens and college students with food allergies are a population we need to reach
~What it means to ethically market your practice as an allergist
~Dr. Jones's biggest aha moment from this year's conference
~FAST London 2027 — and why the global expansion matters

It feels like everywhere you turn, something else is being labeled as dangerous. Seed oils. Food dyes. Processed ingredients. But for families already navigating food allergies, layering in more restrictions can quickly become overwhelming — and not always for good reason.

In this episode, Dr. Alice Hoyt sits down with Molly Kimball, registered dietitian and founder of Eat Fit, to sort through what the science actually says. They break down what seed oils really are, why not all of them deserve the same bad reputation, where synthetic food dyes are hiding in your food (and your medicine cabinet), and how to know when food restriction is helpful versus harmful.

Resources mentioned:

  • Chemical Cuisine by CSPI — a free guide to food additives, color-coded by safety
  • Molly's podcast: Fueled (episode on synthetic food dyes featuring food scientist Thomas Galligan)
  • Eat Fit — Ochsner's nonprofit nutrition initiative across Louisiana restaurants, schools, and markets
  • Watkins natural food dyes

About Molly Kimball: Molly Kimball, RD, CSSD is a lifestyle dietitian and board-certified specialist in sports dietetics. She founded Eat Fit through Ochsner Health and has spent over two decades helping thousands of clients find evidence-based, personalized nutrition approaches. She also hosts the podcast Fueled and contributes regularly to local TV and print media in New Orleans.

The headlines makes it sound simple: omalizumab (Xolair) performs as well as or better than multi-food oral immunotherapy for food allergy. But once you look under the hood, the story gets a lot more nuanced and a lot more real. Fresh from the AAAAI 2026 conference, I walk you through what the new data actually show, why the study design isn't ideal, and what parents should ask their allergist before choosing a path, whether that be with Xolair, OIT, SLIT, or avoidance.

The world of food allergy treatment has been buzzing with headlines claiming that Xolair (omalizumab) is "superior" to oral immunotherapy for treating multiple food allergies. As a board-certified allergist who both prescribes Xolair and performs multi-food OIT, I'm here to cut through the hype and provide clarity on what these treatments actually offer.

Let me take you behind those headlines to examine the OUtMATCH study that sparked these claims and we will decide whether Xolair truly is superior to OIT.

Links I mention:

Here is a link to Dr. Platts-Mills' amazing article.

Regarding OUtMATCH stage 2, the study results are not published (at the time of this publication - my info came from the presentation at the AAAAI 2025 and the photos I snapped of the presentation of the data). Here is the NIH link I mentioned.

Here is my Doximity article on this topic.

Accidental ingestions of food allergens happen all too often, so kids with food allergies need access to epinephrine. While them having their own epi is ideal, that just isn't always the reality.

Stock epinephrine is epinephrine prescribed to a school or other entity to be used to treat a child or adult having an anaphylactic emergency. In this episode of the podcast, Dr. Hoyt shares a training she led for fellow prescribers. In this training, she teaches the ins and outs of stock epinephrine. In this episode, you'll learn:

  • What really is stock epinephrine?
  • How do you determine what your state's law says about stock epinephrine?
  • How can you prescribe stock epinephrine?

If you are not a prescriber, still check out this episode so you can learn about this process, and share it with a doc you know!

Oral immunotherapy (OIT) has revolutionized the way we manage food allergies in children. Join Dr. Alice Hoyt as she interviews Dr. Richard Wasserman, a true pioneer in this groundbreaking treatment.

Dr. Wasserman shares his journey on how and why he started offering OIT to patients over 15 years ago. Dr. Hoyt and Dr. Wasserman discuss the history of OIT and its similarities to allergy shots. They also emphasize the importance of collaboration among allergists to make life-changing therapies more accessible worldwide.

OIT presents unique challenges across different age groups, and this conversation sheds light on these nuances. With relatable analogies like gym workouts, Dr. Wasserman and I talk through the adjustments necessary to maintain tolerance without overwhelming the patient. From the unique challenges faced by children and teens at different ages to potential family conflicts, this episode offers practical insights for managing food allergies over the long run, helping both the patient and the patient's family feel supported every step of the way.

Dr. Hoyt asks Dr. Wasserman to share his experiences on helping families navigate the myriad challenges that come with OIT, from the impact of illnesses on dosing schedules to the necessity of avoiding certain activities post-dosing. Dr. Wasserman shares his thoughts on the FDA-approved peanut allergy product and shares about his team's efforts to ease the journey of peanut OIT for kids. Concluding on a high note, Drs. Hoyt and Wasserman celebrate the immense relief and joy that OIT brings to many families, many times enabling children to safely consume previously dangerous allergens. Don't miss this opportunity to hear from a true trailblazer in the field of food allergy management!

What is the gold standard in diagnosing an anaphylactic food allergy? It's not a skin test. It's not a blood test. It's an ingestion challenge.

In this episode,Dr. Alice Hoyt discusses the critical role of ingestion challenges when it comes to accurately diagnosing IgE-mediated food allergies. By consuming the suspected allergen under the careful supervision of a board-certified allergist, patients can achieve a precise diagnosis, which is pivotal before embarking on treatment plans like oral immunotherapy or long-term avoidance strategies. She shares valuable insights and tips on how to prepare for an ingestion challenge, ensuring a smooth and reassuring process for both patients and families.

Dr. Hoyt also explores the life-changing impact ingestion challenges can have, especially for young children and their parents. Drawing from the groundbreaking LEAP study, she stresses the importance of confidently introducing potentially allergenic foods to children, highlighting how, in some cases, ingestion challenges provide firsthand safety assurance. Additionally, she touches on the complexities of food allergy skin and blood testing and how these tests should not be considered confirmatory for food allergies.

Tune in to learn how ingestion challenges empower food allergy families and, in some cases, confirms that a child is no longer allergic to a food.

Take a deep dive into the world of oral immunotherapy with Dr. Alice Hoyt, a trusted board-certified allergist and mother, and Pam Lestage, a devoted mom navigating food allergies. They break down the essentials of OIT — from understanding what it is to exploring the risks and benefits. Whether you're an OIT skeptic or enthusiastic supporter, this episode promises insights that will deepen your understanding and enhance your decision-making process.

The Science of OIT

OIT is one method that reprograms the body's immune response, teaching it to be less reactive to a particular food allergen. As in this episode, some immune systems are easier to reprogram than others.

Is OIT Right for Your Family?

Curious about the boundaries of OIT? Dr. Hoyt shines a light on who often is a good candidate for OIT. From children to adults, the conversation extends beyond simple yes or no to why and how.

Risks and Benefits

OIT therapy isn't a casual stroll. OIT takes time, and it can cause mild adverse reaction to severe anaphylactic reactions. That said, OIT can be powerful enough teach the immune system to tolerate a food enough so that a person can be bite-proof or, in some cases, even begin to free-eat the food.

Dr. Alice Hoyt interviews Dr. Edwin Kim about his research in sublingual immunotherapy in children with peanut allergies.

Guest Dr. Edwin KimDr. Kim is a renowned pediatric allergist and immunologist who has extensive experience in treating children with food allergies. In this episode, he shares his research on sublingual immunotherapy, a method of allergy treatment that is gaining popularity among allergists and parents of children with food allergies.

SLITSublingual immunotherapy, also known as "SLIT," is a form of food allergy treatment in which diluted allergen is placed under the tongue daily. The concentration is increased until the patient reaches a maintenance dose. The maintenance dose is continued for years. The goal of SLIT is to desensitize the patient's immune system to specific allergens, reducing the allergic response and decreasing the risk of a severe allergic reaction when he allergen is accidentally ingested.

Take-Aways

  1. SLIT is safe for children.
  2. SLIT is effective in children.

If you have heard of OIT or SLIT (oral immunotherapy and sublingual immunotherapy, respectively), then you've probably wondered how these therapies treat food allergies and, if they are so great, why isn't everyone doing them? Well, in part, it's due to potential risks, but there are also some other factors to consider.

Join Dr. Hoyt as she discusses all this with her colleague Dr. Sakina Bajowala. Dr. Bajowala is a board-certified allergist with specific expertise in the treatment of food allergies. She practices outside of Chicago and helps families navigate the world of food allergy and, in many cases, helps make her patients significantly less allergic to foods. Dr. Bajowala is a Fellow of the American Academy of Allergy, Asthma and Immunology and is incredibly well respected by her colleagues in the OIT space.

Did you know that many food allergies can be prevented? Early introduction of potentially allergenic foods, such as peanuts, can actually prevent the development of food allergies. Awesome, right?

But how exactly are you supposed to feed a baby peanuts? You aren't - peanuts themselves absolutely are a choking hazard, so you have to use a developmentally appropriate form of a peanut-containing food.

That's where it gets tricky.

Dr. Katie Marks-Cogan, board-certified pediatric and adult allergist, had the same concerns, so she and her team invented Ready, Set, Food! It's a series of food powders that allows food proteins to be introduced to babies right through their bottles.

On this episode, Dr. Hoyt interviews Dr. Marks-Cogan, and these two allergists/moms discuss early allergen introduction and the Shark Tank-awarded and allergist-approved food product that is helping moms and dads get these foods into babies' diets early and often.

Note: Dr. Hoyt is NOT a paid spokesperson for Ready, Set, Food! and received no financial compensation for this interview.

Have you ever wondered what an allergist is really thinking when that allergist is asking very specific, particular questions about your kiddo's reaction to a food? That's because the allergist is trying to determine the underlying type of reaction.

While some adverse reactions to foods are food allergies, other reactions are not immune-mediated and, thus, are not allergies. And not all food allergies have the same clinical presentations or treatments!

It's so very important to identify and understand the type of adverse reaction to food that's present so that a clear and effective management plan can be put into place.

This episode has a ton of information that's foundational to understanding food allergies! Don't miss this!

We apologize in advance for the sound quality and plan to re-record a similar episode soon. So why did we post this? Well, there is a LOT of good info in this episode, so we decided to make it available to you even though it does not meet our standard for sound quality.

If your kiddo has a food allergy, you may have heard about OIT.

What is OIT?

"OIT" stands for Oral ImmunoTherapy. It is a months to years long process in which the immune system is taught to tolerate an allergen or allergens.

Dr. Hugh Windom was an early-adopter of OIT and has desensitized MANY patients using safe, effective protocols. Dr. Windom is very well recognized and respected in the OIT allergist community (and he's super nice and easy to talk to!).

Enjoy this discussion (despite the glitchy sound quality), and reach out with your questions!

P.S. This episode is also on video on YouTube and Vimeo!

If you have a child with a food allergy, you've likely heard of IgE-mediated food allergies. But there's another type of food allergy that is less well-known - FPIES. Although FPIES is not an IgE-mediated food allergy, it can still be a severe and potentially life-threatening condition.

On this episode of the podcast, Dr. Hoyt interviews THE WORLD RENOWN EXPERT on FPIES: Dr. Anna Nowak-Wegrzyn. Drs. Nowak-Wegrzyn and Hoyt described the symptoms of FPIES, best practices for management, and some very good news about this disorder.

Stock epinephrine saves lives.

What is "stock epi" ? It's epinephrine prescribed to an individual or, more commonly, to an entity to be used in case of an allergy emergency, such as when someone is having anaphylaxis but does not have their own epinephrine auto-injector.

In this episode, Dr. Hoyt interviews her mentee (future doctor) Maddie Oxford about their recent paper in the Journal of Allergy and Clinical Immunology: In Practice entitled "The Heterogeneity of Stock Epinephrine Legislation in the United States."Does your state have stock epi legislation? Does it apply to individuals or to entities? Are schools required to have stock epi in your state? Tune in for more about stock epi, and click HERE for free access to the article (available through the Journal for a short time!).

Calling all Disney World fans!

If you've ever been to Walt Disney World, then you know that a lot of planning goes into those magical memories. And if you have food allergy and haven't been to Disney lately, then, oh, boy, have we got a treat in store for you.

Our families love going to Disney because we know that we will have LOT OF SAFE FOOD OPTIONS without having to look too hard and without getting "the look" from the wait staff - ahem, cast members (the official term for Disney employees). In planning upcoming trips, we thought it'd be great to have an authorized Disney travel planner with a FOCUS ON FOOD ALLERGY share her tips with you!

So tune into this episode in which we interview Lizzie Reynolds, aka Pixie Lizzie, of Allergy Free Mouse and her own travel company Pixie Lizzie. In addition to food allergy-focused guidance, here are a few tips you'll learn about in this episode!

  1. Know the weather and time of year you're planning your trip (50 degrees in Florida feels so different from 50 degrees elsewhere!)
  2. Know the parks you want to visit during the time there
  3. Consider working with an authorized Disney vacation planner, many of whom (like Lizzie) are NO CHARGE FOR THE TRAVELERS (planners get $ from Disney!)

Oh, and also, Pixie Lizzie shares awesome information about the chefs and the food allergy-friendly menus at Disney.

There's just so much great content packed into this under 30-minute show!

"See ya real soon!"

Schools develop food allergy-related policies to try to keep kids safe, but how many of these policies are evidence-based?

And how do policies impact the emotional well-being of kiddos with food allergy?

Join me as I talk with my colleague Ashley Lahoud of Kent State University's Pediatric Anxiety and Allergy Research Clinic. There, Ashley and her fellow researchers are studying just how school policies are affecting the mental health of children who have food allergy. By better understanding this issue, we will have more information by which to provide beneficial guidance to schools on developing policies regarding food allergy.

My favorite part of this episode is that YOU may be able to participate in this research study! If your kiddo is 5-12 years of age and has a food allergy, then you can take the survey and help Ashley and her team better understand the effects of school policies. Take the survey HERE.

Food allergy management certainly can be challenging, but we continue to have more and more evidence to guide us in making best possible decisions, especially regarding management in schools. I'm delighted to have Dr. Matthew Greenhawt on this episode. He's the Director of the Food Challenge and Research Unit at Children's Hospital Colorado. Dr. Greenhawt has authored over 200 journal articles, most of which pertain to food allergy.

I've previously discussed Dr. Greenhawt's article "Managing Food Allergy in Schools During the COVID-19 Pandemic" on episode #12 when I talked with school nurses about food allergy management during COVID-19. You can find a link to the journal article here.

I think you're going to appreciate Dr. Greenhawt's no-nonsense, straight-talk approach to evidence-based management of food allergy.  Let me know what you think!

Do you have a pile of your kiddo's food allergy information? Maybe you have thought about placing your kiddo's allergy test results into some sort of order - maybe organize all those pieces of paper in a binder? Or maybe you have the results available online, not exactly organized but somewhat in one place?

But it just doesn't seem quite organized, right?

We gain a better understanding of things when we write things out yourselves. So it makes sense that organizing your kiddo’s allergy test results can help you better grasp your kiddo’s disease process as you can clearly see:

  • Initial reaction story
  • Skin test results
  • Blood test results
  • Result interpretations and recommended plans by your allergist
  • Ingestion challenge plans and outcomes
  • Epinephrine auto-injector locations and expiration dates

When you put all this information together onto a page, it helps you visualize the clinical side of your kiddo's food allergy journey. It also allows you an easy way to make sure you and your allergist are on the same page (literally!).

In this episode of the podcast, Pam and I discuss a tool that I created especially to help families manage their kiddo's food allergy clinical information. You can access the Clinical Info Organizer here within the blog post that accompanies this podcast episode.

When your allergist recommends a food challenge, what goes through your mind?
"Is it safe?"
"What if my kiddo has a reaction?"
And back to, "Is it safe?"

So what really is a food challenge, and is it safe?

A food challenge is a procedure performed in an allergist's office that involves your kiddo ingesting increasing amounts of the potential allergen to determine:

  • Has an allergy has resolved?

Or...

  • Is an allergy is present?

On this episode, I interview my friend and colleague Dr. Samantha Knox about her research on food challenge safety. If your allergist has ever mentioned a food challenge, tune in to this episode to learn about food challenges and their safety.

Eating away from home is one of the most stressful situations food allergy families can experience. No matter how much you trust the restaurant and how fantastic its people are, the worry of allergen accidentally getting into your kiddo's food is always there to some degree.

And how much of that allergen would cause an allergic reaction? A microgram? A milligram?

In this episode, I go through the super interesting article "Risk of shared equipment in restaurants for consumers with peanut allergy: a simulation for preparing Asian foods" from the peer-reviewed journal Annals of Allergy, Asthma & Immunology, the journal of the American College of Allergy, Asthma & Immunology.

Relevant here is immunotherapy, such as oral immunotherapy (OIT). Immunotherapy can make kiddos with food allergy more tolerant of the allergen. This means a kiddo would be less likely to have an allergic reaction when ingesting allergen.

But does that tolerance translate into real world benefits, like improving safety while eating at a restaurant? Tune in to this episode to hear what this article's researchers found!

And visit the infoblog for the link to the article and more information on safe eating strategies and on immunotherapy!

You've probably heard about doctors recommending babies eat peanut to prevent peanut allergy. This is a major shift from the recommendation made 20 years ago which recommended babies avoid this potentially allergenic food. Allergists did not have much evidence upon which to make that recommendation.

But now we have evidence that supports actually introducing peanut - in an age-appropriate, safe way - to babies who are at risk of peanut allergy as a way of preventing the development of peanut allergy. And in some babies who have already developed a peanut allergy, peanut products may be used to teach the immune system to tolerate peanut.

Does all this sound confusing? Tune in to this episode where I discuss with my friend and colleague - Dr. Jackie Bjelac - how peanut can be introduced to at-risk babies, and we go into detail about what to expect at a "food challenge."
