The Happy Birthway Podcast: Recent Episodes

Chanie Fingerer

Unpopular opinion: A healthy mom and healthy baby are NOT enough. Why don't we pay attention to a mother's experience and perceptions of her birth enough?

I'm an L&D nurse revealing insider secrets that you won't learn in your standard childbirth ed class. We explore every facet (from opposite extremes) of not just birth, but the experience of womanhood and childbearing. Along with women sharing their experiences vulnerably, a slew of experts gift us with knowledge we never thought we needed. I throw in an extra layer to the discussion by exploring the implications of Judaism and Jewish culture on the reproductive experience. This show spans way beyond pregnancy and birth, with topics on sex, birth control, birth trauma, perinatal mental health, postpartum realities, breastfeeding, sleep training, diet culture, infertility, realities of our healthcare system, Jewish law, and so much more.

The information here is not intended to give you medical advice or replace the care of a qualified healthcare provider.

Those who are plus-sized often face a lot of judgment and shame from obstetrical providers. They are made to feel undeserving of the same compassionate medical care that smaller people get. They are automatically labeled as "high risk" even if they have no other risk factors other than their size. Jen McLellan is a published author, founder of Plus Size Birth, and host of the Plus Mommy Podcast. She helps people navigate the world of plus size pregnancy, shares tips for embracing your body, and laughs her way through the adventures of parenthood. Her work has been featured in major publications such as The New York Times, Glamour, Today's Parent, Huffington Post, and International Doula. As a public speaker, Jen has spoken at numerous events, including presenting at the National Institutes of Health. Jen is also a certified childbirth educator, wife, and mother to a charismatic eleven-year-old.

In this second part of a 3-part series on Health At Every Size, and how it relates to the perinatal period, Rachel Tuchman LMHC joins us once again to further challenge the traditional belief that weight loss in and of itself is the solution to better health. She expands on the harm of diets and moralizing of food, solutions for how to confront the stigma of high weight gain during pregnancy, and the feelings of failure when the weight remains postpartum. Rachel also brings awareness to those who are thin on sensitivity toward those who are in bigger bodies, and how their comments, even when well-intended, can be hurtful. Rachel Tuchman is a Licensed Mental Health Counselor (LMHC) with over ten years of experience. She has worked with many diverse populations including incarcerated women at Riker's Island Correctional Facility, kids and adults with developmental delays, kids with behavior issues, overwhelmed parents looking for skills and support, teens struggling with the pressures of adolescence and life in general, adults going through difficult life circumstances (divorce, infidelity, phase of life challenges) and women experiencing infertility.

Rachel is HAES (Health At Every Size) aligned clinician and dedicates a lot of her time out of office to educating parents and kids on the importance of body respect and the behaviors that truly honor our health. Currently, she offers services in her Cedarhurst, NY office for kids and teens ages 5-18, adults, and women experiencing infertility, pregnancy loss and post hysterectomy. Rachel also does speaking engagements for schools, synagogues and various community organizations. You can find Rachel on Instagram: @rachel_tuchman_lmhc and contact her by email: Rachel@lilmhc.com. Exclusive pumping was not the way that Tamari envisioned feeding her baby. After many frustrating weeks of struggling to breastfeed her first baby, she had no choice but to resort to it if she wanted to provide him with breastmilk. She was lost and struggled immensely, having no idea what a wild ride it would be. She took that experience to self actualize by becoming a lactation counselor that specializes in teaching and supporting moms through pumping - whether exclusive or not. You can reach her on Instagram @onewiththepump as well as her website, where she offers various relevant courses and 1:1 support.

Last week, we were "on the other side of the bed" so to speak. I had an encounter in the hospital with my daughter's medical care that left me frustrated and angry. It led me to put the self-advocacy skills that I teach to so many into practice for myself. Were my strong reactions a result of my experience working in a hospital or because I was like any parent who wanted the safest and best medical care for their child? I delve into that in this episode, where you'll hear my story.

Shevi Samet and I get real about how observing Taharas Hamishpacha (Jewish Family Purity laws), is challenging for many. Being niddah in labor, birth and the postpartum period can present additional struggles, especially considering the large amount of support many new mothers need. Shevi is ever empathic toward those who experience these challenges and she doesn’t whitewash their experiences, but in her extraordinarily eloquent way, manages to give some thoughtful perspectives on getting through this time. This episode is helpful to both new mothers and anyone who observes Taharas Hamishpacha.

Shevi is an educator for 15 years and a student for even longer. She is a high school teacher, Jewish bridal educator (aka kallah teacher) and relationship mentor. Her style is very much reflective of her personality and her understanding of young women both in and out of the classroom. It is energetic and engaging, and focuses on text based concepts that explore fundamental topics specifically as they relate to relationships and women in Judaism. True to her essence, she runs the @itsalearninglife Instagram account, a space which provides Jewish women with access to thought provoking Torah ideas, knowledge and inspiration. After years of NY living, she struggles to maintain her Canadian accent, however the reputable warmth and sincerity of her hometown have remained with her. You can reach her by email through sametshevi@gmail.com.

Choosing a doctor or midwife is one of the most important decisions that you will have to make during your pregnancy. What qualities should you look for in an obstetrician to determine whether they are both competent and the right fit for you? To what extent does their “bedside manner” matter in the quality of care that they provide? What are the pros and cons of a solo provider vs. a group practice. Should you see one provider from the group continuously to develop a strong relationship with them or should you meet each one a few times? What happens when you like one provider from the group but not another one? Can you transfer to another provider and what is the latest in your pregnancy that you can do so? What might be reasons that someone would want to transfer from one provider to another? Dr. Fox answers all of these questions and more.

Dr. Nathan Fox is an OBGYN and Maternal Fetal Medicine specialist. He is a Clinical Professor at Mount Sinai and a managing partner at MFM Associates and Carnegie Imaging for Women in New York City. Dr. “Natie” is also the creator and host of the Healthful Woman Podcast and the High Risk Birth Stories Podcast. He is from Chicago and lives in Englewood. When not practicing medicine, you can find him running, swimming, practicing hot vinyasa yoga, or embarrassing his wife and four children. Or most likely, walking the dogs.

Shira is a social media creative and Mo is at the cusp of finishing his 10 year journey to becoming a neonatologist. They retell the birth stories of their 3 children, highlighting the stark differences between having their first 2 babies 10 years ago as newlyweds and their 3rd baby who was born a few months ago. Shira faced quite a few obstacles during her first birth, including an unexpected acute medical condition and her baby showing signs of distress during labor. The couple gives their individual perspectives of this birth, which are quite different. Shira accessed her connection with G-d to get through this difficult time while Mo's medical training gave him a different view. Mo talks about how working with critically sick newborns influenced the way he experienced their latest pregnancy and Shira talks about how being married to a doctor with an intensely busy schedule created the need for more control over her birth. They also talk about the perks of having a neonatologist for a father of a newborn.

Shira's expression of creativity and spirituality takes on many forms. Her trademark rainbow challahs, iconic Jewish pop art and stunning video creations can be found on her Instagram account @itsaconfettiparty. She most recently began selling her original, Jewish inspired, neon hued pop art paintings on her new Instagram page, @theshirashencollection.

What does it mean to have “infertility”? Why is pregnancy after infertility challenging? How can you support someone close to you who finds themselves in this situation? What is the roller coaster of emotions that those who struggled to become pregnant commonly feel when they finally see those 2 pink lines (or get a positive beta test)? What are ways to cope through this tumultuous time? Dr. Aimee Baron's extensive experience - both work and personal - lends itself to a thorough exploration of these questions.

Dr. Aimee Baron MD, FAAP is the founder and executive director of I Was Supposed To Have a Baby, a nonprofit organization that utilizes social media to support Jewish individuals and families as they are struggling to have a child. It provides a warm and nurturing space for those going through infertility, pregnancy loss, infant loss, surrogacy or adoption, in addition to connecting those families to resources in the Jewish community at large.

Dr. Baron was formerly the Director of Innovation and Growth at Nechama Comfort, and has also worked as an attending pediatrician in the newborn nursery and neonatal intensive care unit at St. Luke’s-Roosevelt Hospital before taking a leave of absence after her third miscarriage. You can find I Was Supposed to Have a Baby on Instagram and on their website.

What are the qualities that you should look for when hiring a doula? What is and is not within a doula’s scope of practice? Is a doula supposed to speak up for you in the hospital? Should a doula argue with the staff in the hospital? What happens if your doctor is recommending something against your wishes? What’s the difference between consent and coercion? What happens if you change your mind about something that you planned about your birth? How is the landscape surrounding birth changing in hospitals and who is responsible for this change? Bracha and I dive deep into these issues in this episode.

Bracha Bard-Wigdor is a Jewish bridal educator, sex coach and doula. She is passionate about women’s health, most especially sexual health. Bracha believes that every woman deserves to feel confident in her body and her choices, both in birth and in her relationships. She now has 10+ years of experience dedicated to empowering and educating women. Bracha enjoys normalizing conversations that are taboo within Jewish communities. She appreciates breaking stigmas and is a committed community activist. You can find her on her Instagram page, on Balaboostas.com/forum, an online community for Jewish women, and contact her through balaboostas@gmail.com.

Fewer deaths doesn’t make it ok to fire rockets at civilians. It’s time to tell the truth about what’s going on in Israel and the far-reaching effects that Hamas’s missile attacks have on their targets. Hear about what it’s like to give birth when your country is under attack. Sarah Amar, an American emigre to Israel, tells us about her experience having a c-section an hour before hundreds of rockets began raining on her hospital. Although her entry into motherhood came at the worst time, she manages to laugh and look at the bright side of this horrific experience.

Get ready to laugh until you cry in this second part of Rachel’s birth story. She shares her funny take on labor, being sent away by the hospital when she came for her induction, how she managed when her epidural didn’t work and how to put a medical student to good use. She gives moms who are scared of labor encouragement and strength. The most important lesson she teaches us though is how hard yet empowering it was to say no when things didn’t feel right.

Rachel Safer is the writer and creator behind @modestisthenewblack and rachelsarasafer.com. She is also podcast host to “A Safer Space” which can be found on Patreon. Rachel is an ASL student with hopes to become an ASL interpreter while she juggles all the dreams with the responsibilities of being a full time mom to her little guy AY.

As an adoptee, Rachel Safer tells us how that influenced her feelings around pregnancy. Her story sharing style is hilarious as she bemoaned all of the discomforts of her pregnancy. Rachel tells us about her hospitalization when she got Covid in her 9th month, how sharing her pregnancy with the public brought on lots of unsolicited feedback and teaches us a lesson about self advocacy to get the medical care you deserve.

Despite Dr. Karkowsky's extraordinary knowledge and expertise in the highly specialized and complex area of high risk obstetrics, she infuses her care with a high level of compassion and empathy, remaining cognizant of the complexity of her patients' lives outside of the medical sphere. These patients are often unexpectedly thrust into this world, having to learn a new language and sometimes needing to make very difficult decisions, neither choice being ideal. She is a master storywriter and captivating story teller. We talk about the barriers of a complicated healthcare system, the pitfalls of having too much compassion, her experiences of working as a doctor in Israel and how Rabbinical and medical authorities can clash and merge when it comes to the extraordinary lifecycle event of reproduction. Dr. Karkowsky completed medical school at Mount Sinai, her residency at Harvard University Hospitals, followed by a fellowship in Maternal-Fetal Medicine (MFM). She now works as a high risk obstetrics specialist in New York City. Chavi also lived in Tel Aviv with her family for several years, and worked at an ultra-Orthodox hospital in Northern Israel, experiences she loves to talk about. Chavi has written for the Atlantic, Slate, the Washington Post, and other publications. "High Risk: Stories of Pregnancy, Birth, and the Unexpected" (Liveright/Norton 2020) is her first book, and is available everywhere that books are sold. Chavi lives in NYC with her wonderful partner, Josh, and their four stir-crazy kids. She can be reached via her website, www.chavikar.com. My sister-in-law Rivka Raizel had her wedding and her first baby during the Covid pandemic. Because of the unexpected circumstances, she and her husband (my baby brother!) had to put their plans of moving in Israel on hold. They finally gained entry only to find themselves back in the States to have their first baby near family. As a foreigner, RR found the Israeli healthcare system complicated and confusing, and she compares and contrasts it with her prenatal care in the US. She had a vision for how she wanted her birth to go but alas, as it often happens, things turned out differently. We talk about her hospital doula restrictions, her baby's bris and what happened when her baby went into distress at the end of her labor.
