On February 24, 2023, the Drug Enforcement Agency announced proposed rules for prescribing controlled substances via telemedicine after the COVID-19 Public Health Emergency expires.

The proposed rules were open for public comment for only thirty days, after which DEA would presumably issue final regulations. The proposal created two new limited options for telemedicine prescribing of controlled substances without a prior in-person exam. The options were complex and more restrictive than what has been allowed for the past three years under the covid waivers.

The DEA’s proposal would discontinue the ability for telemedicine prescribing of controlled substances where the patient never has any in-person exam (with the exception of a prescription period of no more than 30 days’ supply). The initial reactionis the rules are more restrictive than necessary and impose concerning limitations and burdens on clinicians and the patients they treat.

In response to more than 38,000 comments on the proposal, the DEA and the Department of Health and Human Services submitted a draft temporary rule to the Office of Management and Budget that provides a temporary extension of telehealth flexibilities for controlled substance prescriptions. Details about the rule will become public after it is published in the Federal Register. I don't know when that will be.

Is there any negative effects I need to be aware of? I am weightlifting as well and it’s really helped.

  • Wanting to start lifting/exercising after a 5 year break. I get throbbing pain in my biceps after lifting any upper body movement. What does it sound like is happening? Is there anything I can take to minimize this?
  • Is creatine safe and effective - on of off TRT, worth it?

A little back story, I’m 26 years old 6’4 260 pounds the last 3 years I’ve noticed my health go downhill I’ve lost my hair, lost libido, I gain weight super easily even though I stay pretty active. I finally decided to go and get blood work done my total test came in at 246. I feel anxious constantly and just don’t feel like myself and it’s been affecting my relationship.

The Dr I’m seeing prescribed cypionate 200mg a week. I don’t have kids yet though I’m engaged and it’s wanted in our future. I’m a full time firefighter so sleep isn’t always the best.

I haven’t started the test yet as I’ve been trying to weigh everything out. The doctor says it’s no big deal and I’ll be able to have kids. The only reason it makes me nervous is because it seems like he is a sales person for trt.

Sorry for the long post, what I’m getting at is what would you do in my position? I’d love to feel better and get my life back together.

Chris Stevens - My T level was 72 a few weeks ago. I’m 32 years old.

I’ve taken my 4th injection today. I don’t feel any difference at all. Is that normal? I’ve been taking between 200 and 150mg each week.

Not sure how long it takes to see improvement. Energy is okay, zero sex drive, zero thoughts about sex, zero morning erections, focus etc feels not much improved.

Ashley Zulch - Has anyone gone off of hcg and just stuck to testosterone? My husband went on hcg in august and off of testosterone so we could conceive and now is off of hcg and only on testosterone. He stopped taking hcg in November and still feels just as bad as he did when on hcg. He gained 40lbs once on the hcg and felt worse than he did before his low t diagnosis.

We were just wondering if there is a certain amount of time if takes for hcg to leave the body. I’m really concerned because he’s struggling so much.

TRT Community Post - I posted this short blog, about weight loss drugs in the TRT Community Facebook group. The first sentence was something like, "Losing weight can be an incredibly difficult endeavor…" A fellow responded and informed me that losing weight is not difficult and a conversation ensued.

Just because something has a step process you can follow, doesn't make it easy. I could rebuild a transmission with the proper instructions, but there's a reason specialty shops exist. Have you ever had a pack of clutch plates laying on a work bench???

Topic One: Semaglutide & Tirzepitide

Semaglutide -

  • it's sold as Ozempic and Rybelsus for DM treatment and Wegovy for weight loss.
  • Brain uses hormones to regulate functions, including digestion and metabolism
  • Semaglutide mimics the action of GLP-1 class hormones
  • GLP-1, or glucagon like peptide-1 help regulate digestion and metabolism
  • The more GLP-1, the more full you are

  • Side effects - Diarrhea, Nausea, Constipation, vomiting

Tirzepatide

(Mounjaro) currently approved for DM treatment

  • Same GLP-1 effects as Semaglutide
  • Also effect on GIP (glucose=dependent insulinotropic polypeptide)
  • GIP enhances the action of GLP-1, which tells the brain to stop eating.
  • The way I think about it is like this: Semaglutide, Ozempic, Rybelsus, Wegovy - it's like having a fellow in your brain telling you to stop eating. Tirzepatide or Mounjaro is like have 2 fellows in your brain TELLING you to stop eating.

  • The most effective weight loss medication ever made available with an average weigh tloss of 22.5% over the course of a year.

Personal experience - started taking Semaglutide about 7 months ago. Lost about 10% BW in a few months and plateaued. Started Tirzepatide this month. Not much side effects. Feeling bloated/full most of the time

Did TRT improve your anxiety and confidence? Maybe is the fact that I’m autistic, but all my life Ive been prone to being anxious when interacting with others regardless of how many times I do it. I’m 38 and even if I’m “relaxed” there is still a “slight” anxiety underneath that’s probably unseen by others yet I feel it. Maybe I’m just a nervous person but I have noticed my anxiety has gotten worse compared to 10 years ago. I feel more confident than when I was younger from life experience and maturity but still anxious. I’m also more prone to being fearful than some but I fight it and contain it. I wish I was like those guys seem to have no fear and have nerves of steel. That’s life but who knows maybe i have a bigger weiner or higher IQ than them.

So many folks focus on the physical improvement aspects of TRT. The mental health benefits are what I'm most passionate about. Before TRT - I was anxious daily, no confidence, counting hours before I had to do things like go to work, afraid to speak up, engage people in social situations, seeking validation from other people and manipulating them into providing you with value.   • I've been looking into getting on trt. Even sent in my labs. But the more I look at this group I wonder what are the actual benefits? All I see is balding issues, shrunken balls, erection problems etc. I thought it should help some of those issues. It's expensive and seems to have negative effects in some instances. What's the actual benefits of doing it?
  • Buddy of mine has a hematocrit of 58%. He says he doesn’t believe it’s dangerous because he’s injecting test. Is he right? I was told it is in fact dangerous to your health. What say you?

  • My 10ml vial of testosterone cypionate expires says to discard after 28 days, however it’s supposed to last me 12+ weeks, is this the norm? Does it actually expire become less effective or what?
  • Scared to start TRT. I am 27 and have a prescription at a pharmacy ready for me to go pick up. Im just very afraid of dying from a blood clot or pulmonary embolism. My father died of a pulmonary embolism caused by untreated polycythemia vera. I am pretty healthy and workout but i’m scared TRT wil cause a blood clot. I seen it can through a couple of studies.
  • I was just prescribed Deca .25mg twice a week. For those that are on it for joint pain, how long does it take to start working. I just did my 2nd injection.
  • Does TRT englarge the heart?
  • Why does some test cyp say for intramuscular use only

What's going on guys? This is Brandon with the TRT community. You are listening to the All Things Testosterone Podcast and today we are talking about polycythemia/elevated hematocrit.

Brandon: (00:27)
Before I get into that, I want to just, uh, give a shout out to Michael Ashford of the Fit Dad Fitness podcast. He interviewed me last week. We talked for like 45 or 60 minutes or so and it really just kind of flashed me back to five years ago. It reminded me why I do this. There's an entire world of people out there that don't, that don't know anything about TRT. The only that only know what they hear they see on daytime TV commercials and you know, sometimes I get lost in the little TRT community bubble and I just take it for granted that, you know, everyone at least has knows something about it. But the reality is that there are millions or billions of people that, that think it's unhealthy or that just have never heard of it. So it was nice to talk with him and get outside of the TRT community bubble and yeah just to be able to remember that there are guys out there that are super fit and super knowledgeable but they may not be knowledgeable in TRT. So it was a pretty cool conversation. I'll let you know whenever it gets published.

It's going to be customer service first kind of clinic and I mean that more than anything. I mean a lot of people will say it, but we are going to take care of you. We are going to be reasonably priced. We're going to be competitive with everybody else out there and we are going to answer the phone when you call. We are going to take care of you. We have knowledgeable practitioners. We know what we're doing, it's going to be amazing. Um, I'm really excited about it.

Brandon: (02:13)
So I did a poll on the TRT Community Facebook page and I basically asked just how many TRT patients have experienced elevated hematocrit or polycythaemia. 74% said yes, this is probably the most, if not one of the top three most common side effects to TRT and it's super easy to avoid. It can be super dangerous and it can cause your doctor to discontinue treatment if he's not super well versed in TRT.

Brandon: (02:43)
First let's talk about some of the issues that can arise if you have prolonged exposure to elevated hermatocrit. It's what gives you all the issues. Elevated HCT is where the heart attacks come from. That's where the strokes come from. That's where everything you see on daytime TV, you know, class action lawsuits come from.

Brandon: (03:09)
So it, it's dangerous. It causes blood clots and if nothing else your Dr. May stop your treatment and you definitely don't want that. So basically all it means is that your blood is thicker because you have more red blood cells and your heart has to work harder to pump the same amount of blood. So it makes sense why it can be dangerous because your heart's going to give out quicker if it's having to work harder to do the same job. Let's talk about the symptoms of it. Blurred or double vision, shortness of breath, headaches, itching, flushed, skin, tiredness and excessive sweating. My tell-tale signs are probably shortness of breath, headaches and excessive sweating. When this starts happening, I know that my hematocrit is getting up there and that I need to do something about it.

Brandon: (03:59)
So the simple solutions, you can drink more water and I mean a lot of water, you can eat grapefruit every day. You can try subcutaneous shots and you can do more frequent smaller doses. This sub Q and or more frequent smaller doses handles it for a lot of guys. That's all they change and they're good to go. If that doesn't do it for you, you can also donate blood. It's probably the most common way that guys handle or you know, manage it. That's what I do. I donate quarterly and that, that does it for me. Um, I just, you know, I wanted to, to be able to mention it because not only is it dangerous and you need to avoid it, but your doctor might stop your treatment, like I said, and, and you know, it's just unnecessary. Just go donate blood.

Brandon: (04:43)
Some guys will get hung up on crashing Ferritin by donating too often. I mean, I've always thought that's absurd. Not that it can't happen, but, you know, that's something that is even talked about when donating blood can save your life in someone else's life. But the hangup is you don't want to crash your ferritin. I mean it, it's so rare. It just doesn't happen often. It's almost like it happened to this one guy and he's just telling it to everyone. I think it's kind of ridiculous. So I wouldn't worry about that. If it becomes an issue. It's not that big of a deal. Um, but you're only donating, you know, in regular intervals of like what, 56 days anyway. So if it was going to happen to you, it could happen to other people too that donate frequently.

Brandon: (05:27)
So, I'm talking to BignoKnow (Noah Thomas) on Tuesday. So in just a couple of days. In fact, by the time you're listening to this, I'm, I'm probably gonna be interviewing him and I'm more excited about that interview than I have been about any interview. I love Noah. I've talked about him a lot on this podcast, so it's no secret. I was shocked to learn that only something like 20% of the TRT community listens to him or knows who he is. I mean, he, he was my version of the TRT community whenever I started TRT. He's the one that, that I got all my information from and he's just so vulnerable, so open, so honest. I really, really love that guy. So I'll be talking to him soon. Uh, yeah. Let me know what, uh, what you would change about your current TRT provider. Is it too expensive, as a service? Terrible. Um, do they not have enough extras for you, you know, the random get jacked supplements that they brand and sell to you? Or is their protocol bad? Just what are your pain points? I'd like to, with this Texas clinic and with the rest of the clinics that we plan on opening, I'd like to avoid as many as possible. So let me know, email me at brandon@trtcommunity.com.
