In preparation for bathing suit season, we present a discussion of the current status of hair removal treatments. Traditional hair removal methods like shaving, waxing, sugaring, and threading work temporarily, but medical-grade, in-office treatments are more powerful.

The use of Ozempic/Wegovy and similar medications such as Mounjaro for weight management is rapidly evolving. As they continue to be studied further and receive FDA approvals, including the approval of Zepbound this month, the question becomes whether they can be a meaningful substitute for bariatric surgery.

Ozempic has been used to treat type 2 diabetes since 2017. When used for weight loss, semaglutide is used at higher doses and marketed as Wegovy. Newer medications such as Mounjaro (tirzepatide) have entered the market for and will be marketed under the name Zepbound for the sole purpose of weight loss.

Lifting the face and neck without surgery is the holy grail of aesthetic medicine. While there’s no meaningful substitute currently available that can give the degree of laxity correction in the face, neck and jawline as the facelift, there are good non-surgical skin lifting and tightening treatments for mild to moderate signs of aging.

Non-surgical skin treatments started with various smoothing treatments such as chemical peels. As doctors noticed the more aggressive use of these treatments created some tightening as well, the idea of developing a standalone skin tightening treatment in and of itself piqued their interest.

In the 1990’s, there were attempts to use lasers and other infrared light sources to tighten skin, but it wasn’t until the early 2000’s with the introduction of Thermage and fractional laser treatments that the modern era of non-invasive skin treatments emerged.

Through trial and error, doctors discovered that it’s safer and more effective to treat with these energy devices over time rather than do one really aggressive treatment.

More recently, the FDA has cleared ultrasound treatments Ultherapy and Sofwave for lifting and tightening the skin. Today, energy can be applied beneath the skin, too, with treatments like FaceTite, which delivers radiofrequency energy.

Although relatively small, the eyes are the first area of the face to show changes, and have a big impact on one’s appearance.

Until the 1980’s, doctors used a small set of lasers as a continuous wave for every medical and cosmetic skin condition, heating and burning off the surface of the target. Dermatologist Dr. Vic Ross characterizes this approach as being like “napalm in the forest,” Then, Harvard researcher and dermatologist Dr. Rox Anderson recognized these treatments would be much more selective if they were delivered in short pulses and tailored to each condition.

From there, researchers in laser medicine began investigating and experimenting, starting with pulsed dye lasers for redness and frequencies targeting red and brown spots. With any surgery, it’s normal to be concerned about the scars you'll be left with and whether they heal properly. In this episode, Dr. Bass covers the different types of scars and what to do if you find yourself with an unpredictable scar during your recovery.

Two conditions which sometimes occur with scar healing are hypertrophic scars and keloid scars. A hypertrophic scar is red and thick, caused by inflammation combined with your wound healing biology and how your skin behaves. Keloid scars are the result of specific genes, growing thick, raised, and wider than the original incision or wound.

With scar issues of redness and thickening, the scar will be targeted through gel bandages, compression therapy, or lasers and energy based devices. Fractional lasers have revolutionized the treatment of scars, both traumatic and surgical. For widened scars, surgical revision is typically needed, trimming out the old scar and resewing the skin edges no sooner than 6 months after surgery.

Good scars start with good surgeries. Be cautious that you’re teaming up with a surgeon who knows how to place incisions well and treat problem scars if you’re one of the rare people whose skin reacts differently.

Once an unknown niche procedure, buccal fat pad removal is now everywhere in the media. Buccal fat pad removal surgery allows us to flatten the cheeks of somebody who has "chipmunk cheeks," reducing the bulge below the cheek mound to be relatively flat or just slightly concave.

Mostly for young adults, this surgery can be a powerful tool for facial shaping when the right amount of fat is removed. But if too much fat is removed or the patient does not have enough fat to begin with, it can result in an unnatural look.

Finding the right device means first consulting with and building a relationship with an expert dermatologist or plastic surgeon, who will help you select the treatment that will best address your concerns.

Getting to your goals requires an entire toolbox, not a singular device. If you already have lines etched in the skin, Dysport or Botox won’t help. Neuromodulators are for prevention, not rejuvenation, and you need laser resurfacing to remove them.

Mild improvements in fine lines can be obtained with the new superficial fillers, but when lines are deep and etched, slight improvement is made with chemical peels and more significant improvement is made with laser peels (laser resurfacing). If you have had a cosmetic procedure and wish it could be undone, or if you are considering a cosmetic procedure but afraid you'll regret it, there are options for going back. There are options available if you don't love the outcome after most procedures.

Implants and fillers can be changed, and it is possible to rework other cosmetic procedures. Brazilian butt lifts and other fat grafting shaping procedures can be treated with liposuction like techniques to go in reverse. Trends in appearance are okay for fashion and makeup, but not ideal for aesthetic treatments, especially plastic surgery.

Thinking through your procedure decisions and planning properly is the key to good outcomes. Carefully plan and think through your wishes and desires before getting a cosmetic procedure with tools like visual imaging and computer simulation to understand the results you might see.

It's always better to avoid an unwanted result than have to undo it, but there are ways to rework procedures, if not necessarily reverse. And be kind to your body tissues – perfect is the enemy of good.

Rumors are spreading that patients are “over it” when it comes to injectable fillers, but Dr. Bass says that's far from the truth. The core strategy of facial rejuvenation treatments at most stages of aging includes a program of injectable filler treatments.

Filler is being used in more areas and more creative ways than ever before to combat the general facial volume loss that accompanies aging. Also, these products are great for features in which there is no better option, like lips that have lost volume or the tear trough at the lid/cheek junction.

If you’re considering fillers, it's important to know where and when filler is applicable, and how much is too much. The goal is to add back the youthful volume you previously had, and should never be overdone in an attempt to avoid another treatment that may be more beneficial.

| You know you are not ready for a facelift yet but your face is not looking as youthful as you want. If you’re avoiding the plastic surgeon’s office for fear of being sold something you might not need, know that it is much easier and more affordable to prevent your face from aging now than it will be to fix it later. Many people seek nose surgery for aesthetic reasons, to correct a bump or a hump, or change the tip of their nose. Aesthetic breast surgery has made many advances to provide plastic surgeons and their patients more control over the results.  Capsular contracture is a classic problem with breast implants where the healing membrane around the implant distorts shape and symmetry and degrades softness.  This was once largely the result of silicone gel that leached out of intact implants.  Modern implants are low-bleed and factors like blood and biofilms (colonies of bacteria) on the surface of the implants are now thought to be the leading causes.  A number of developments help prevent capsular contracture including no-touch technique, bloodless dissection, use of antibiotic and antibacterial irrigation, among others.  The controversial role of incision selection is discussed by Dr. Bass and his guest expert, Dr. Jason Pozner from Boca Raton, Florida.   Mild capsular contracture can often be treated with ultrasound, non-invasively.  Moderate or severe capsular contracture requires surgical correction including capsulotomy (cutting the capsule), partial or total capsulectomy, implant exchange and sometimes change of plane from subglandular to submuscular implant placement.  Acellular dermal matrix materials like Alloderm or Stratice can also help reliably expand and maintain the implant pocket after capsular contracture.
