Welcome to another empowering episode of the Connected Living Podcast. I'm Lecinda van Niekerk, a Functional Nutritionist navigating my own perimenopausal journey. Today, I'm joined by Dr Mayoni Gooneratne MBBS, Bsc, MRCS, AFMCP. In this episode, we dive deep into Hormone Replacement Therapy (HRT) to explore its various aspects, including its types, side effects, dosages, and long-term benefits.
Conversation Highlights:
Not all Hormone Replacement Therapies are the same. There are different types available, each with its own set of benefits and considerations.
Body identical and bio-identical HRT refer to hormone replacement therapies that use hormones chemically identical to those naturally produced by the body. We delve into each one and what makes these treatments unique and their potential advantages.
Like any medical intervention, HRT with synthetic ingredients can come with potential side effects. We discuss common side effects and clarify the structural difference between HRT.
It's crucial to know whether you're taking synthetic or natural progesterone during HRT. We explore the differences between the two and how to identify them. This includes the Mirena coil. If you’re still experiencing symptoms and you have a Mirena coil then this episode is for you.
Understanding the right dosage of HRT is essential for optimal results. We discuss the importance of blood testing and how it informs treatment plans.
Do you need blood testing from an NHS GP to get prescribed HRT? We shed light on the role of healthcare providers in administering and monitoring HRT.
Perimenopause and menopause are distinct stages in a woman's life. We outline the differences between them and how HRT can address symptoms associated with each stage.
We discuss the natural changes that occur within the luteal phase of the menstrual cycle during perimenopause and how this leads to unopposed oestrogen.
Finally, we discuss the potential long-term benefits of using natural, body-identical HRT, including its impact on overall health and well-being.
Conclusion:
Thank you for joining us in Part 1 of All things HRT. We hope this discussion on Hormone Replacement Therapy has provided valuable insights into its various aspects and considerations. Stay tuned for Part 2 with Dr Mayoni where she’s keen on sharing the licensing elements relating to Bio-identical hormones and how she incorporates Testosterone and DHEA into her Treatment Plans if needed.

Welcome to today's Connected Living episode featuring Caroline Webb, also known as The Thyroid Nutritionist. Caroline joins us to share her inspiring journey of overcoming post-partum Hypothyroidism and autoimmune Hashimoto's. Fueled by her personal struggles, she is now on a mission to help others identify and address their unique root causes, guiding them through their own thyroid healing journeys.
In this insightful episode, we delve into the complexities of thyroid issues, exploring the challenges of testing and the common occurrence of receiving 'normal' results while essential biomarkers remain overlooked. Caroline brings her expertise to the forefront, making the intricacies of thyroid biochemistry not only understandable but also enjoyable, offering a clear breakdown of hypothyroidism versus hyperthyroidism.
We discuss the array of tools available for thyroid support and touch on the tendency of Endocrinologists having a more 'wait and see' approach to thyroid care and why that is. Caroline raises thought-provoking questions about the significance of thyroid antibodies, prompting us to consider whether waiting until extensive damage has occurred (80-90% of the thyroid gland) isn’t the best solution and what we can do instead.
But that's not all—Caroline candidly shares her personal experience as a mother, navigating the challenges of early motherhood while grappling with constant exhaustion. Join us for an engaging conversation that not only sheds light on thyroid health but also resonates with the realities of balancing motherhood and well-being.

Before we dive into this podcast, I make a quick correction that I don't ask my clients to fast for 24 hours before a blood draw. I meant to say 12 hours.
How to prepare for your blood draw:
• Stop supplements 48 hours before testing
• Fast for 12 hours before testing.
• Continue to drink water.
• DO NOT stop prescription medicines unless under the guidance of your prescribing medical practitioner.
• Draw blood only on Monday to Thursday. Sooner if the next working day is a UK national holiday.
In this episode Taryn shares her experiences around testing during their fertility journey and all the frustrations that came with it.
Lecinda dives into 5 key mistakes she sees in private practice on a daily basis and why she believes this is driving the ever increasing experience of having test results that say everything is 'normal', yet most of her clients feel far from normal.
This is a huge topic and one that's not often discussed. Lecinda hopes this gives you the permission to keep investigating and become your own health advocate. The answers are out there!

Struggling with Hormonal Imbalance?
Don't miss this episode where Taryn and Lecinda share their personal struggles on this topic. Plus Lecinda answers some difficult questions on testing, continually receiving normal results and the importance that metabolic health has on supporting your hormones.
Lecinda shared more on metabolic health and mentioned 5 markers that can be looked at.
Metabolic Health refers to the state of balance between storing fat and burning it for energy.
Health is adversely affected when this balance is disrupted.
Optimal metabolic health is achieved when someone meets to following markers:
Average Blood Glucose (HbA1c): >5.7% ( > 39mmol/L)
Blood Pressure: <120/80mmHg
Waist Circumference: <88cm for women; <102cm for men
Blood Triglycerides levels: <1.7mmol/L ( < 150md/dl)
High-density lipoprotein cholesterol (HDL-C) <50 mg/dL (1.3 mmol/L) in women; <40 mg/dL (1.04 mmol/L) in men
Metabolic syndrome may be diagnosed if 3 or more of the above markers are exceeded.
This is a very useful place to start when looking at your hormonal health. The body functions as whole intertwined web of biochemical reactions, cofactors, cell signaling etc.
If someone has high cholesterol it's highly likely that their whole steroid pathway might be implicated. Cholesterol is our 'master' hormone if you like and other hormones like progesterone, cortisol and testosterone are made by organs by taking up cholesterol.
That's one snippet of how your metabolic health may influence your hormonal health that Lecinda discusses on this episode.
