In this episode of Air Traffic Out Of Control, we listen in on the recent incident that took place on Alaska Airlines flight 1282 on January 5th.

The Boeing 737-9 Max took off from Portland, Oregon bound for Ontario, California, but when it reached 16,000 feet, a portion of the fuselage blew off and the plane immediately depressurized.

The pressure was so intense that a young boy on the flight had his shirt torn completely off by the suction.

Oxygen masks were deployed on board for the 177 passengers.

You will hear the pilot in the recording declare an emergency and immediately request to return to Portland, while attempting to take the plane back down to 10,000 feet due to the depressurization.

In this episode, we go over to Japan as we just learned about the incident that took place in Tokyo.

A Japan Airlines Airbus A350 arriving from Sapporo to Tokyo Narita was touching down when the aircraft impacted a Japanese Coast Guard Dash-8 and burst into flames.

Everyone on board the Japan airlines aircraft was evacuated.

5 of the 6 occupants of the Coast Guard aircraft suffered fatal injuries. The Captain survived but sustained serious injuries.

Unfortunately, the audio prior to the incident was low quality so we could not publish it here. The Japan Airlines crew did receive clearance to land from ATC, and red back a confirmation. The Coast Guard aircraft had been instructed to hold short of the runway at intersection C2.

Delta flight 1092 from Atlanta to Charlotte contacted the tower due to a landing gear warning. The nose landing gear would not extend on the Boeing 717, so they would need to perform an emergency landing.

Listen in as the pilot arranges the landing with the tower.

Unfortunately the audio from the landing itself was cut off, but rest assured, all 104 passengers and crew aboard the aircraft were safely evacuated once the plane was on the ground.

Earlier this month, at Austin International airport, a FedEx aircraft narrowly avoided a Southwest plane as it attempted to land.

Controllers had cleared the arriving FedEx seven sixty seven and a departing Southwest Airlines seven thirty seven jet to use the same runway, and the FedEx crew realized the issue before it became a disaster.

The FedEx pilot then told the Southwest crew to abort taking off, then climbed as its crew aborted their landing to help avoid a collision.

In today's Air Traffic Out Of Control Fly By we have a recent clip from a close call at JFK.

At around 8:45 p.m. last Friday evening, Delta Air Lines Flight 1943 was taking off from Runway 4L when  American Airlines flight 106 crossed the same runway. 

In this recording, you will hear the moment an air traffic controller noticed the incident and a potential collision was avoided. 

The Delta aircraft rejected its takeoff run and stopped approximately 1,000 feet from where the American Airlines wide body jet had crossed. 

Investigations are being conducted by the NTSB to get to the bottom of why this occurred.

On January 11th, some corrupt files in the antiquated FAA software system caused a nationwide outage across the United States which grounded flight departures across the country.

The system which went down (NOTAM) provides advisories to pilots on safety-critical conditions at airports and other areas aircraft may traverse, including everything from warnings about bird activity to runway construction.  

So, as you can imagine, it's a critical system for pilots in ensuring safety.

After investigation into the problem, the issue seems to be rooted in a software developer error and the inclusion of a corrupt file in the fragile, outdated software.

The issue was resolved by the following day, but not before over 1 million people had their flight plans disrupted.

On this day (January 15th) in 2009, Chesley "Sully" Sullenberger landed US Airways Flight 1549 in the Hudson River after the Airbus A320 serving the flight struck a flock of birds shortly after take-off from New York, losing all engine power. Unable to reach any airport for an emergency landing due to their low altitude, Sully and co-pilot Jeffrey Skiles glided the plane to a ditching in the Hudson River off Midtown Manhattan. All 155 people on board were rescued by nearby boats, with only a few serious injuries.
