On January 11, 2022 this medical helicopter crashed into a church in Drexel Hill, Pennsylvania while transporting a 2-month old infant. How in the world did this kid, and the entire crew, survive? I will also quickly review what little is known about the Hawaii Life Flight fixed-wing air ambulance crash that took place on December 15, 2022 just south of Maui, Hawaii, in a Beechcraft King Air C90A with tail number N13GZ.
Finally, I will also quickly recap the most recent incident regarding REMSA Health's Care Flight fixed-wing air ambulance crash that took place on February 24, 2023, just west of Reno, Nevada, in a Pilatus PC-12, with tail number N273SM.
All 3 of these crashes have left the air ambulance and flight communities with more questions than answers and all 3 crashes have occurred with the same company, Guardian Flight. In this episode, I will tell the story of the fatal ARCH Air Medical helicopter crash that took place on March 6, 2015 in downtown St. Louis, MO, U.S.A. The initial impact itself was listed as survivable, but this medical helicopter was not equipped with a crash resistant fuel tank, and subsequently exploded after impact and killed the pilot.
There are hundreds of AS350s, H125s, and EC130s across the world that are used for medical evacuation and the tourism industry. While the FAA, Airbus, and the EASA all acknowledge that this airframe tends to explode or catch on fire after impact, none of them have required the available crash resistant fuel tanks/systems, which are readily available, to be installed.

In this episode, I will tell the story of the fatal SkyLife 4 medical helicopter crash that took place on December 10, 2015 just north of Bakersfield, CA. The pilot, flight paramedic, flight nurse, and the patient, were all killed in this accident which occurred in a perfectly functioning helicopter. The pilot may have had a lack of training and experience with this model helicopter (Bell 407), the advanced avionics, and did not use night vision goggles even though they were required by the operator. I will also briefly hit on the over use of HEMS in the U.S. as well as how Worker's Compensation can come into play, even in fatal accidents such as this.

In this episode, I will tell the FULL story of the Survival Flight fatal medical helicopter crash that took place on January 29, 2019 in southern Ohio. This flight was operated by Viking Aviation, LLC, DBA Survival Flight, Inc. The pilot, flight nurse, and flight paramedic were all killed in this crash. Rumors were heavy that a poor safety culture played a role in the root cause for this accident. In this episode, I will tell the FULL story on helicopter N72EX which was flying Kobe Bryant, his 13-year-old daughter, and 7 other people, including the pilot, when it crashed suddenly into the side of the Santa Monica Mountains just north of Los Angeles, CA on January 26, 2020. This story will focus on contributing causes of this crash that were almost completely omitted by the media and the NTSB. There was far more too it than a pilot who simply flew into the clouds.
And if that wasn't bad enough, I will also talk about the terrible decision that law enforcement and fire personnel made when they decided to share gruesome photos of the accident victims at places like television awards events and bars.
While these events were tragic and the results have caused pain to millions of people, there are still lessons to be learned so that these same mistakes are never made again.

In this episode I will tell the story that took place in August of 2022 in Woonsocket, RI, USA. Two EMTs find a mother who has just delivered a premature baby into her toilet. The baby is left in the toilet, determined to be dead, transported to the ER in a red bio-hazard bag with the mother, only for ER staff to determine that the baby is still alive. How could this have happened? Would routine simulation, practice, and the use of checklists have possibly prevented this from occurring. All of this and more on this episode...

In this episode, I will explore the story behind the Med-Trans Regional One EMS helicopter crash that took place on July 13, 2004. What went wrong? Was there something wrong with the helicopter? Could this have been prevented? What in the world is "helicopter shopping" and may have it affected the outcome of this flight?

Edit - I misspoke at the beginning and stated that this helicopter flew straight into the side of a mountain. For clarification, while the Sumter National Forest is technically a part of the Appalachian Mountain range, the area of this crash is more of a densely forested area with 100-foot-tall pine trees.
Keep in mind that this story is from 2004. Since then, Med-Trans is under new ownership and new management and many of the shortcomings that are listed as contributing factors in this episode have since been addressed. Not only have they been addressed, but Med-Trans has somewhat paved the way in the industry by mandating many things that are not even required by the FAA such as autopilots, crash-resistant fuel bladders, participation in an FAA safety management system, and much more.
