Elfen Lied takes place in Kamakura and Kanagawa, and focuses on the "Diclonius", a newly mutated species. Their appearance is similar to humans, but with several differences, namely horn-like protrusions on the forehead and the presence of telekinetic invisible arms called "Vectors". One such Diclonius, Lucy, is the main character of the series: Initially held in a facility built for experimentation, located off the coast of Kamakura, she manages to escape and wreak havoc, but is injured in the process, an event which causes her to develop a secondary, childlike personality known as Nyu.
Lucy is found by two locals, Kouta, who studies at the local university, and his cousin Yuka. They take her in, and become involved with the numerous, often brutal, attempts to recapture her by a Special Assault Team and a number of other Diclonius, who shift frequently from oblivious to murderous. In order to maintain the state of peace between the rival nations of Westalis and Ostania,[a] a Westalian agent code-named "Twilight" is tasked with spying on Donovan Desmond, leader of the National Unity Party within Ostania. However, due to Desmond being notoriously reclusive, the only way Twilight can get close to him is to enroll a child in the same private school as Desmond's sons and pose as a fellow parent. To accomplish this and present the image of a happy family, he creates the alias of Loid Forger, adopts a young orphan girl named Anya, and marries a woman named Yor Briar. However, unbeknownst to him Anya can read minds and Yor is in fact a professional assassin. Neither Loid nor Yor are aware of each other's true identities, or that Anya knows their true professions. The family later takes in a dog with precognitive abilities whom they name Bond. Despite these unknown factors and Twilight's occasional lapses of common sense due to years of being a spy, he must learn to play the role of the perfect father and husband in order to carry out his mission. To her surprise, she also finds out that 200 years have passed in Leadale since she last logged into the game, a period in which the seven nations she knew were devastated by war and re-organized into three new kingdoms.

Vampires walk among society, existing as part of its underbelly. The protagonist is an old man who had lived his life as the worlds greatest assassin. Due to his advanced age, it was finally decided that he would be able to retire. However, the plane he was on was sabotaged and even his skills as the greatest assassin could not save him. When he died, he was awakened by a goddess who wanted to reincarnate him in a world of swords and magic because she needs his skill to prevent the destruction of that world at the hands of the Hero. Accepting this request, the protagonist awakens as Lugh Tuatha De and swears to finally live his life to its fullest as a human being and not as a tool.
