Today, we're diving into a crucial yet often overlooked aspect of caregiving: documenting your loved one's medical history in real time. By documenting real-time changes in your loved one's health, you'll be helping their medical professionals make better decisions in their care plans and improve their quality of life.
Caring for a chronically ill loved one can be challenging as their health can change day to day. This has been very true in my own experience of caring for my epileptic husband. One thing that has helped me immensely is documenting any changes I see as they happen. Not only does this help me remember critical details, but it ensures as much information as possible is shared with his medical team. Remember, as a caregiver, you are essentially the eyes and ears of your loved one's medical professionals. The information you provide helps them make better decisions for your loved one's care.
Major Health Events:
New diagnoses, surgeries, or hospitalizations.
Was the event related to a current health issue or is it something new?
Changes in Treatment:
Any changes in new or existing medications in terms of dose or frequency or any therapies they receive.
Be sure to update medication changes to any other doctors or specialists to avoid drug interactions.
Health Checkups:
Document annual wellness visits and any follow-up appointments.
Note any changes in medications, treatments, or further testing recommended by the doctor.
Lab Results:
Record routine and emergency lab results.
Document any adjustments to any medications based on those results.
Medications:
Track any changes in dosage, frequency, and rules for administration.
Be sure to document any known medication allergies and details on what symptoms your loved one has when they have a reaction.
Symptoms and Daily Observations:
Note any new or worsening symptoms, even if they seem minor.
Examples might include changes in appetite, mood, or energy levels.

Rishawn Dindai is the co-founder of a website that helps caregivers get resources to manage their self-care and feel supported in their caregiving journey. He has also been a caregiver himself, having cared for his grandfather with dementia and other chronic illnesses for most of his adult life.
Rashawn Dindai cared for his grandfather with multiple chronic illnesses. He had to learn to be creative with his caregiving journey in order to make sure he had time for himself and to be supported while caring for his grandfather. He used digital resources such as Instagram, and YouTube as well as counseling therapy to get the support he needed.
Despite the struggles of caregiving, Rashawn persevered and found a way to find the support he needed.

Miriam Safla is a busy mom and wife, and a mompreneur who is passionate about helping others prioritize themselves in their roles as wives and mothers. She is an expert in transformational coaching and healing and has a deep understanding of how to break generational patterns of trauma.
Miriam Safla had a difficult relationship with her father who she saw as constantly running to help family and friends, neglecting his own desires and needs. As a result, she made a vow that she would never be like him and would take care of herself instead. As an adult, she found herself in a similar caretaker role, neglecting her own aspirations and happiness. Through her work as a shadow transformational coach, she learned to forgive her father, forgive herself, and connect with her true self and desires. She found that the key to breaking the cycle was to make self-care and self-love a priority and to use the Hawaiian forgiveness prayer of 'I am sorry, please forgive me, I thank you and I love you'.

She began dealing with chronic illness began back when she was just eight years old dealing with depression and anxiety and well into her teens, 20s, and 30s have been diagnosed with a number of health issues: tension headaches, migraines, neurology headaches TMJ disorder autoimmune disease arthropathy inflammatory arthritis, spinal degeneration.
Yet throughout all the health issues, Lauren refuses to let her chronic illness stop her. She's figured out ways to modify her role as a mom and spouse in a way that helps her prioritize managing her chronic illness. In this episode of the Caregiver Coffee Chat Podcast, I sit down with Claire and Amaranthia, are mother-daughter co-founders of sistacreativesrising.com which is their business for helping marginalized Black Women heal from their experiences of childhood and domestic abuse or bullying through art.Claire's Story:In one week in March 2022, days after launching Sista Creatives Rising with her daughter Amaranthia, Claire lost her mobility by the hour. Enduring emergency surgery to remove a diffuse large B-cell lymphoma from her spine, she let go of all delusions and illusions about her health. Claire is now within the 1% category to walk again after such surgery. Now in remission, Claire is redetermined to help marginalized women gain inner strength by accessing their artistic side.A Buddhist and Frances Perkins Scholar, Claire's journey to scholarship began during her childhood in Barbados when she sought relief from the chaos of living under domestic violence. Claire uses her creative works and writings to encourage women trauma survivors to utilize art for self-improvement. In the 1990s, standing at The Door of No Return in Senegal, where enslaved people last saw their homeland, she reaffirmed her purpose: to overcome intergenerational trauma.Amaranthia's Story:Amaranthia Sepia feels it's critical to use her voice to facilitate the representation of marginalized women. As a Buddhist Black, invisibly disabled woman with C-PTSD, panic disorder, and agoraphobia, she highlights unconventional experiences through art. Amaranthia, along with her Mom, Claire, is the co-founder of Sista Creatives Rising, home of the virtual charitable art event "Art & Mind," which helps marginalized women/femme creatives gain accessibility and visibility in the arts to facilitate personal healing. Currently, she's a resident of Socially Distant Art, a residency focusing on disabled creatives.Returning to America after spending her childhood in Japan, Sepia was severely bullied. Creating works based on fond memories of Tokyo taught her the healing power of art. Since 13, she's coordinated art events on anti-bullying advocacy, BLM, disability, and women's mental health. Jen Rafferty is a mom of two and an expert in cognitive neuroscience, emotional intelligence, and mindset. She is the founder of Empowered Educator, where she teaches teachers and school leaders about their own social and emotional well-being. Jen was a busy mom and public school teacher who experienced a major life shift when her husband decided to get a divorce and she moved out of her house. She used this moment to discover who she really was and to take care of her own mental, emotional, and spiritual well-being. She became an expert in cognitive neuroscience, an emotional intelligence practitioner, and started her own business, Empowered Educator. She encourages moms, caregivers, and mom premieres to practice self-care by nurturing their minds and souls and making sure they have boundaries to protect their well-being.

As a caregiver for an addicted spouse, Tully Rose must walk the line of enabling and helping them recover in order to save their relationship and themselves.
"The reason why I decided to tell my story is that, well, thankfully I'm not the first one to tell my story on a platform. There's many that came before me on social media, other places on the web who are able to show me that telling your story can be a powerful thing."
Tully Rose is a coach and author who helps people heal from codependency, toxic relationships, and abuse. She is a survivor of domestic violence and has been in recovery from addiction for over 10 years. Tully was a caregiver for her elderly grandparents for over a decade and grew up in an abusive household with a father who had addiction issues. She felt the need to always give and was often the person with that others could come with their deepest secrets. She eventually started a coaching practice to help others in similar situations. She wanted to share her story on the Caregiver Coffee Shop podcast to help others understand the complexity of caregiving for an addicted spouse or partner and to highlight the distinction between addiction and abusive behavior. Despite the struggles she faced, she was determined to share her story to help others in similar situations.
