Your body is your life’s partner, not your life’s project.

What if you could focus on nourishing yourself for vitality and worry less about your body’s size and shape? What if you could step over the threshold of midlife and see elder hood as a time to emerge as your truest self? What if you could approach midlife and beyond as a time to break free from old beliefs about your body? Few things will bring more empowerment and ease into your life than learning to trust yourself and your body.

Debra Benfield, or Deb as we know her, has helped hundreds of women heal their relationship with food, eating, and their bodies in her 35-year career as a Registered Dietitian/Nutritionist specializing in preventing and treating disordered eating. She brings her research, training, passion, clinical and lived experience to the intersection of pro-aging and body liberation.

Deb’s work is rooted in helping her clients: shifting their body relationship from their life’s project to their life’s partner, recognizing internalized ageism and ending it, Untangling from diet/wellness culture, nourishing their bodies in midlife and beyond to support growing older in alignment with their values, and seeing midlife and beyond as a time of Emergence

Last week, in part one of our conversation with Deb Benfield we explored diet culture and the hold it has over our society and how we can learn to look out for it and fight it for ourselves and our loved ones. We also discussed the power of midlife to emerge as our truest most authentic self.

This week, in part two of our conversation we are exploring Deb’s recipe for aging with vitality and body liberation and exploring the power our “body stories” have over us and how the body shame memories from our youth can directly impact embodiment as an adult.

This is an episode you won’t want to miss!

Every year, about 1 in 5 expecting and new parents experience symptoms of depression and/or anxiety. While most new mothers suffer from mood swings within the first few weeks, some pregnant and postpartum parents have intense feelings of sadness, worry, anger, numbness, or self-doubt that persist for months. Unfortunately, services are seriously lacking for parents in the United States, particularly when it comes to postpartum mental health.

Anchor Perinatal Wellness provides unique and specialized mental health programming for pregnant and postpartum people, including a free, walk-in screening clinic, outpatient services and an intensive outpatient program to pregnant and postpartum adults who are struggling with a perinatal mood and anxiety disorder (PMAD).

Anchor was founded by three Latina women who recognized that they shared a passion for expanding mental health services for perinatal people and they opened their program in May of 2022. Today we are joined by two of the founders of Anchor Perinatal, Roxanne Rosenberg, a licensed mental health counselor and Veronica Kemeny, a licensed clinical social worker.

Last week, in part one of our conversation with the Anchor team we dove into a discussion of PMADs, lessons we can learn from other countries, and how PMAD’s are showing up differently in minority populations. Today in part 2 of our conversation, we dive into what intensive outpatient services look like for those suffering from PMADs, and explore reproductive justice and the role it is playing in Anchor Perinatal’s work.

Our discussion today included:

  • An exploration of intensive outpatient programs, how it’s different, why it’s rare in the US
  • What a day in the life of an Intensive Outpatient Program looks like
  • Reproductive Justice defined
  • The first National Maternal Mental Health Task Force and Anchor’s role in it
  • Impact on LGBTQ+ communities of recent legislation

Dr. Karen Sheffield-Abdullah is an Assistant Professor UNC Chapel Hill, a Certified Nurse-Midwife, Mindfulness Instructor, a wife, mother of four and a plant lover. She describes herself as a stress and anxiety researcher who is passionate about perinatal mental health and the impact on pregnancy outcomes specifically in black and brown women.

Dr. Karen is trying to get answers on why, despite decades of research, the black and brown communities have twice the rate of preterm birth and higher rates of death compared to white women. Using her depth of knowledge, experience and education, Dr. Karen is an advocate for birth equity, mental health awareness and mindfulness.

In our ongoing conversation, Dr. Karen begins by elaborating on the differences between culturally competent care and culturally humble care. She discusses the unique stressors to the black and brown communities, and talks about the importance of providing stress screeners with the understanding that they are meant to be a jumping off point to deeper conversations. Healthcare workers need to be comfortable with having the conversations. Dr. Karen describes it as, “getting comfortable with being uncomfortable.” Healthcare providers need to begin to normalize having deeper conversations with clients. We talk about the importance of midwives, the passing of a certain piece of legislation benefiting her research, and incorporating stress management into models of women’s health care for beneficial outcomes.

  • The importance of starting difficult conversations from a humble place.
  • Normalizing mental health in black women during pregnancy.
  • What is the Midwives for Moms Act?
  • Diversifying the nurse midwifery workforce is essential to improve outcomes.
  • Why it’s important to bring more diversity into the health care system.
  • Stress and perinatal mood and anxiety disorders in black women.
  • How do we make it such that when we bring in examples of stress, when we talk about suffering, that it is through a culturally relevant lens?
  • Black women’s pain is notoriously under assessed and under-addressed.
  • The importance of self-reflecting when taking care of this community.
  • The ways her background in midwifery has shaped her view of the healthcare system and her professional goals.
  • Dr. Karen’s research results using self compassion, mindfulness and Mind Body therapies

She received her PhD, MPH, and MSW from the University of Pittsburgh. Her research interests include developing, implementing, and evaluating interventions to address racial/ethnic and socioeconomic disparities in obesity and eating disorders.

In her research, Dr. Rachel Goode discovered that there are a lot of people who don't get treatment for eating disorders because they do not fit stereotypes about who has an eating disorder or what it looks like to have an eating disorder. Racial, ethnic and socio-economic disparities are not taken into consideration in order to treat eating disorders effectively. Individual choices are only a small piece affecting people's ability to make the health changes they are seeking.

Today’s episode includes a discussion of the following topics:

  • Rachel’s background, research interests and how she developed a passion in this area.
  • The most memorable class she took in school.
  • Supporting people on a journey that they may or may not be wanting to take.
  • What it looks like to have an eating disorder that limits us.
  • The ways eating disorders and obesity connect
  • Explanation of Appetite Awareness Training.
  • The results of some of her studies.
  • Why there’s a disparity in the medical community when it comes to recognizing eating disorders in black women.
  • Our primary care visits are our first line of defense against eating disorders, but they’re also ineffective.
  • Interventions Dr. Rachel has seen making a difference.

Our education system impacts parents and children alike. It is rare to meet someone with a deep understanding of the day to day management of a classroom and curriculum instruction as well as a firm grasp of the intricacies of the education system as a whole.

Dr. K. Lea is that person. She has been involved in special education for over 30 years. Her experience is broad in scope, ranging from aiding in the development of one of the initial early intervention programs in Tennessee to working in drug and alcohol rehab, the juvenile prison system, resource classrooms, and consulting for home school groups who enrolled students with special needs. She has presented professional development workshops on topics in Montessori and special education both nationally and internationally. Dr. K.Lea has also worked with state and local governments in special education policy and procedural alignment. Her current research focus is primarily on peer-assisted learning in inclusion classrooms and overidentification of minorities in special education.

In part 2 of our conversation with Dr. K.Lea we discuss valuable literacy strategies that can be implemented at home to help kids succeed and the differences across state lines when it comes to our education system. Unfortunately, it doesn’t matter what state you live in, the system is failing our teachers.

  • What Peer Assisted Learning Strategies look like and how it’s helping students read at grade level
  • Literacy strategies that can be implemented at home
  • The value of knowing sight words, letter recognition, and math facts
  • Differences in the education system across state lines
  • How Common Core leveled the playing field for many families
  • The failures in the education system and how it falls most heavily on teachers
  • How parents can pay attention to ways to advocate for schools

Today we are discussing a difficult but important topic. It’s a subject we believe needs to be spoken about more openly so mothers know they are not alone. About 10 to 20 percent of known pregnancies end in miscarriage. But the actual number is likely higher because many miscarriages occur very early in pregnancy — before a person might even know about a pregnancy.

We hear our expert panelists discuss how parents can navigate pregnancy after loss, what resources are available, that often difficult decision to not have more children and being content with the family that one has, and dealing with the changes that the body goes through after loss, how to approach fitness after miscarriage or loss, and a parent's understandable array of mixed emotions and anxiety of losing a child.

The third HER Health Collective Roundtable of 2022 featured the following HER Expert panelists:

  • Dr. Charryse Johnson – experienced Licensed Clinical Mental Health Counselor offering over 20 years of experience serving as a counselor, consultant, and educator. She holds a PhD in Counseling Psychology, NCC 2021.
  • Nicole Wallace - a Licensed Clinical Mental Health Counselor with over 20 years experience helping adults and children with overcoming trauma, managing life transitions, and developing coping skills.
  • Christy Maloney – Registered Dietitian and Certified Eating Disorders Registered Dietitian – Supervisor, CEDS-S specializing in helping those with eating disorders and disordered eating from her practice, Enhance Nutrition Associates.
  • Jenna Somich - Certified Personal Trainer & Health At Every Size certified Trainer

Today’s episode includes a discussion of the following topics:

  • Practical advice & tips for those navigating a pregnancy loss or miscarriage
  • Recommended resources for parents to turn to in times of grief and loss
  • The impact of loss on family planning & finding peace with the size of one’s family
  • Dealing with the feeling of one’s body betraying oneself
  • Ideas on how to support and what to say and not say to a friend or loved one suffering through the loss of a pregnancy or child
  • Nurturing the body and finding oneself again after a loss
  • How to handle the anxiety that comes through this type of loss and potentially handle future pregnancies
  • How the Roe v. Wade reversal impacts mothers in these experts’ practices

We are back for part two, diving deep and getting personal with two entirely new questions!

We recently read the book Share Your Stuff. I'll Go First.: 10 Questions to Take Your Friendships to the Next Level by Laura Tremaine. Laura says, “In spite of the hyper-connected culture we live in today, women still feel shamed for oversharing and being publicly vulnerable. And no matter how many friends we seem to have, many of us are still desperately lonely.”

We built HER Health Collective to cultivate genuine relationships between moms in an effort to drastically decrease the loneliness they may be feeling. Having relationships beyond the superficial requires digging deep into the nitty-gritty and having openness with others. We are setting the stage and being an example to our community of moms.

So, here we go, we’ll go first… Our discussion today includes our answers to the following questions:

When did it change?

What are you afraid of?

Today we got to sit down with Iris Chen, of the wildly popular website and social media account known as “Untigering.” Iris also recently authored a fantastic book by the same name. Iris is an American Born Chinese who ended up raising kids who are Chinese born Americans. She considers herself a deconstructing tiger mother who is doing her best to become a gentle parent. As she learned to honor her children’s individuality and autonomy, she felt it no longer made sense to subject them to a system that wouldn’t do the same. Iris began to trust in her children’s innate ability and motivation to learn, and found she no longer believed in a compulsory education that forced children to learn according to arbitrary adult standards. Iris practices unschooling with her children and has a second book by that name coming out soon.

Today’s discussion covers these topics:

  • Reparenting and how becoming a parent can be a portal for our own healing
  • Tiger moms and what the Untigering movement is
  • How culture plays a role in parenting
  • The need to do less “reading” and “research” and focus a bit more on our intuition as parents
  • How resentment can impact the parent-child relationship
  • The need for awareness and being proactive when it comes to our own personal boundaries as a parent
  • Viewing gentle parenting as a practice, as opposed to something we have to get exactly right
  • How adult expectations may impede our relationship with our child
  • Unschooling: what it is, what it could look like in practice, and why you may want to consider it

Dr. Kristin Tully is faculty in the UNC Department of Obstetrics and Gynecology and a member of the Collaborative for Maternal and Infant Health at UNC-Chapel Hill. Dr. Tully is a medical anthropologist who seeks to enable health by improving health care services over the “1,000 days” continuum of pregnancy, birth, and postpartum. She is interested in understanding human needs around the perinatal period of the life course, from the perspectives of birthing parents, infants, and those supporting them.

Her research addresses topics such as breastfeeding outcomes, mother-infant safety, sleep practices, maternal health, and transitions through health care, to establish more patient- and family-centered care. The objective is to advance equity by identifying metrics for accountability and continual improvement in health systems.

This episode offers an important conversation about the state of postpartum care in our country. It is so enlightening to hear all the ways Dr. Tully and her colleagues are working to improve care in the fourth trimester and beyond. We hope you leave feeling just as inspired and hopeful as we did.

Summary:

  • The importance of having an inner purpose
  • Role of medical anthropologist
  • Need to put moms and her key support at the center of the medical system of postpartum care
  • The role of science in shaping how we approach different aspects of motherhood, and assessing who we are using as the “norm” in the collection of data and how that impacts the results.
  • The set-up of hospitals and birthing centers, and how that impacts the system of care.
  • We are just as dependent on each other after birth as we are during pregnancy, it just looks different. The current lack of support in the fourth trimester is unacceptable.
  • The Fourth Trimester Project and the work they are doing to change the healthcare system
  • How language of things like intake forms can make a big difference in what information a mom chooses to share
  • The importance of services like doulas and interpretation services, and the need to better integrate them into the system and provide living wages for the people in those fields.
  • The need to change the standard of care to center the marginalized and oppressed in order to make sure every mom receives the care she needs and deserves.
  • The need for credible, trusted resources for moms and how the formatting and placement of those resources can make a difference.
  • The importance of sharing our story and how we can each drive change in postpartum care by helping other moms know they are not alone.

Dr. Joni shares how parents can navigate the healthcare and school systems to best advocate for the needs of their children, all while continuing to care for their own needs.

Listen in as we discuss how girls and boys present with autism differently, some of the common misconceptions of ADHD, autism, learning disabilities, mood disorders and other behavior problems, and how to know who to seek out when building the team of providers that will care for your child. This conversation is packed with so many crucial pieces of advice, you may want to take notes! We hope you get as much out of our time with Dr. Joni as we did.

“Most women in the U.S. are not dying during childbirth because of the complexity of their health conditions, but because of the barriers they faced in accessing high-quality maternal care—particularly those who were poor or faced racial discrimination.”(2010 Amnesty International report; Deadly Delivery: The Maternal Health Care Crisis in the USA)

We have a system that is not adequately caring for our mothers, particularly black mothers. This is a fact that needs to be heard far and wide, and we need all parties to take part in a conversation to figure out the best way to deal with this crisis.

The first HER Health Collective Roundtable of 2021 took place on March 5th and discussed Inequalities in Maternal Outcomes with several of our 2021 Expert Panelists.
