Brent: "Are the rehab stages considered 'strength' training and, if so, is it too much strength, taking away from my runs or bike days?"
Jennifer: "I saw a PT who said my hamstring was constantly under tension because of a pelvic tilt. Do you know if anterior pelvic tilts can be a cause of PHT or if there's any benefit to correcting the tilt?"
Rebecca Appleby: "When the PHT is responding well to a solid rehab plan, at what point can strides be reincorporated into a return to run routine, or does the PHT need to be remedied first?"

In today's episode, Brodie answers the following questions:
Julia: Hi Brodie I have a general query about the fact that this condition is so difficult to treat within the physio/orthopedic community with seemingly only a handful of specialists (like yourself) who understand how to rehab it successfully. Is this a new 21st-century condition that has been amplified by our sedentary lifestyles and long hours of sitting at work? It just seems strange that no one had this problem back in the 80s and 90s when jogging/running became so popular.
Harvey: Hi Brodie, Maybe you could expand on how a PHT sufferer assesses whether they have a relatively strong or weak tendon. For example, a slight increase in a strength exercise or perhaps too much sitting in a given time period will cause a flareup and will often take 4-5 days for the tendon to settle back down.
Trina: Rehabilitation exercises are great; usually doing the job to strengthen and “repair” if following doctor’s orders. However, once the hamstring and/or tendon no longer cause pain, is it expected that someone would continue the exercises, and if so, would it be less frequently, fewer reps, lower intensity, or something else? In other words, what is the post recovery protocol?
Jen Johnson: Hi Brodie, my question is, how common is it to have pain in the "muscle belly" of the hamstring? Is this truly connected to this PHT injury? I occasionally have pain at the high tendon attachment but it is rare. I have really stalled in my rehab and I am wondering if it could be something else. I should clarify, that in the beginning, it was more prominent in the attachment but not now.

Exercises, that should heal or prevent from PHT!
Single leg elevated hip thrust and single leg glute bridges with straight legs.
I never had any tendon problems before my injury 2 years ago on my left leg.
So how can these exercises cause PHT on a healthy leg??
And what should I do for rehab when PT exercises cause PHT?"

This episode covers the beliefs behind stretching and when it is safe and not safe with proximal hamstring tendinopathy.
Stretching was once a common treatment option for this condition but as our understanding has changed, so has the management.
But a lot of people with PHT need to return to stretching. Whether it is a pilates or yoga class or stretching without their chosen sport or physical activity. In addition, stretching the hamstring is a part of everyday life. Bending forward to put on shoes or picking something up off the floor is required but can sometimes cause discomfort.
So, when is stretching okay for proximal hamstring tendinopathy?

In today's Q&A Brodie answers the three following questions from podcast listeners:
"I'm a marathon runner and I've been suffering from PHT for 3 years. Progressive loading exercises didn't work and eventually, I gave up. Nothing seemed to work. I've now started running again building up very slowly (started with 3x1 minute running and a minute walk between). I can now run 3.5 miles pain-free. In your opinion is this something that could continue to work for me and has anyone ever recovered from PHT in this way?"
“Is there any technique to build LL muscle with this tendinopathy? PHT about 3 years. Finally, a year ago I found a PT which made me workout through pain (at no more than 4-5) with controlled movements, which has helped me be able to squat, deadlift, and some other movements at 75%-85% of my 1RM, which has improved my body quite a lot. I still find limits in what I can do or how much stress I can put into my workout. So please some advice on this subject would be greatly appreciated!”
“Would love to hear more about rehabbing PHT while training for a race. I'm scheduled to run Boston in April and have been rehabbing since December 3rd. I haven't yet introduced speed back in but slowly building up mileage. Trying to decide if I should back out of the race or if I should run it and just take it easy. Just don't want to jeopardize my progress”.

Nathan Carlson is a US-based Physical Therapist and co-host of the Runners Zone podcast. Brodie and Nathan talk about the importance of developing independence during a runner's rehabilitation and how a therapist needs to nature the independent process.
This is important for every PHT sufferer to know. Some therapists may misdirect rehabilitation toward a short-term, dependant strategy, resulting in high consultation frequency & poor evidence-based approaches.
Nathan talks about hands-on, passive treatments and when they are validated versus exercise-based rehabilitation. He also discusses the importance of empowering the runner through this process.
Finally, Brodie and Nathan discuss red flags to look out for to see if you're on the wrong rehabilitation path.

In today's episode, Brodie breaks down the comments written by his past PHT client. Here is their submission when asked what their biggest insights and revelations were during Proximal hamstring tendinopathy rehab:
"There’s a lot of ‘horror stories’ out there which can easily lead to panic and catastrophising, causing increased attention to the pain".
"Keeping routines and contact with friends has been hugely important. It would be easy to hide away and sulk, but that wouldn’t help in the long run. I’ve continued to be very involved with my club, helping at races and making sure I’m there to support in any way I can".
"With the pandemic, I’m just so happy to be part of the running network again! Most of my friendships are through running therefore I’ve continued to nurture and value these".
"Finally, of course, the physical rehab has to be carefully managed and progressed, and you have to listen to your body, which can be quite difficult for us runners".

In today's episode, Brodie breaks down the comments written by his past PHT client. Here is their submission when asked what their biggest insights and revelations were during Proximal hamstring tendinopathy rehab:
"My main learning points from my 2.5 years would be:
Don’t think that PHT will go away by completely resting. Pain will decrease (with rest) but once you attempt to return to activity, then you’ll have more than PHT to worry about. Seek input from someone who has experience in treating this condition. Keep notes on what activity you do and reactions to this, both rehab and what other daily activities might impact. Do not accept corticosteroid injection if suggested. Do seek MRI or ultrasound to help with diagnosis if unclear. Talk about the condition and its impact on you with those close to you - it’s a nasty injury that impacts many aspects of life. Be kind to yourself! Make sure to get enough sleep. Eat well, including regular protein in your diet. Realise that in order to make progress you need to really commit to your rehab. There will be progress, but also dips/flare-ups which can mess with your head. Get a sit/stand desk for work, but don’t become fearful of sitting - you can’t stand at all times.

In today's episode, Brodie unpacks the reflections and revelations from a past client. Here is their response when Brodie asked what really helped their recovery:
"Well definitely the exercises you said to do! And made sure I stuck to doing them. I stopped doing the glute bridges etc and focussed on increasing weight-bearing ones. I had read about it and self-diagnosed at first and thought I knew what exercises to do but didn’t know how many to do or how to progress through the exercises. I didn’t realise that I could have kept running. And if I could start again, exactly how. I avoided long sitting and if I sat, I used a cushion. I found other ways to exercise like swimming which I found I could do without making it a flare-up. Definitely need to find a physio that knows how to diagnose PHT and how to treat it. I definitely needed someone to relate the treatment to my particular condition and what I was able to do or not do. It’s not a quick fix so don’t give up! I still have some runs where I feel it but the next day it’s fine again".

In today's episode, Brodie reads out responses from current and past clients. These are insights they have had when managing their own Proximal Hamstring Tendinopathy:
Client A: "I guess my key takeaway is not a fun one and it’s one that I’ve tried to avoid but for me, it was unavoidable - patience. I need to be patient with my recovery and not start doing too much, too soon. I've had other times when my PHT has alleviated only to flare up badly again because I did too much”.
Client B: “I believe the key is to stick to the program. As much as I would have loved to jump ahead I stuck to the program and have had no recurring problems”.
Brodie talks about why it is so hard to be patient and trust the process. He then dives into practical tips to help, along with some examples of a sensible management plan.

On today's episode I answer Mark's submitted question:
"I would like to understand how an injured/damaged/weak tendon can go on to cause such widespread symptoms? In addition to tendon pain, I get chronic muscle tightness in my hamstring, thigh, calf, hip, groin and buttock. Also, get nerve pain throughout the whole leg. These symptoms get particularly worse through a flare-up. It can be hard to remain focused on the rehab with such widespread symptoms and feel it would help keep the focus of having an understanding of why it does this?"
Firstly, I discuss the components of 'chronic muscle tightness' and certain explanations for these symptoms.
Next, we dive into nerve pain and chronic pain to help understand why these symptoms may occur.
Lastly, I give my final tips all to do with widespread symptoms.

We often misinterpret weakness when it fact, it could be just an increase in structure sensitivity.
When progressing a rehabilitation program and executing your rehab exercises, it's not uncommon for a hiccup and mild flare-up to occur. This is often followed with pain & irritation when attempting activity that was once well managed.
This can be interpreted as weakness and without recognising the difference, we often go through high amount of frustration, anger & confusion.

During these success story segments, Brodie interviews a runner who has managed to overcome PHT after much hardship and frustration.
When injured, it is easy to find social media posts & stories from other injured runners which are discouraging and worrying. Tales of chronic pain, debilitating symptoms and failed treatment techniques often manifest anxiety and thoughts of the worst outcome.
However, this is only a small fraction of runners and a skewed bias around the injured population. So, this segment aims to bring hope and optimism, while sharing treatment success.
On today's episode, Brodie talks with Todd Charnetski, a passionate runner who developed bilateral proximal hamstring tendinopathy back in 2007 after pushing hard during his half marathon preparation.
Todd had tried years of different treatments. Icing, massage, stretching, compression, strengthening, medication, PRP, cortisone, tenex and several other remedies. It wasn't until 2018 when a shift in his management started showing drastic improvement.
Todd is currently 100% better on one side and 95% better on the other and is excited to share his story.

We tick off our last universal principle to overcome your proximal hamstring tendinopathy. Brodie introduces the Pain, Rest, Weakness Downward Spiral which is a common trap for tendon pain.
It is human nature to retreat from activity if it causes a flare-up, but in reality, if you are constantly removing aggravating factors without replacing it with a proactive exercise, the tendon becomes weaker and less able to handle rehab exercises.
Brodie dives into detail and how you can use this concept to your advantage.

This universal principle focuses on the difference between external tissue loading & internal tissue capacity. In particular, your adaptation zone & max capacity line is not static. In fact, certain environmental influences and mood states can fluctuate this line and influences how much load your proximal hamstring tendon can take!

The chapter of the e-book is titled: 'Sleep on it: Interpreting your pain over 24 hours'. Your tendinopathy recovery will be drastically disabled if you cannot accurately interpret symptoms. What makes matters tricky, is sometimes pain can be a good thing. Brodie uncovers this dilemma on today's episode.

On today's episode, Brodie dives into the stages of PHT rehab. We first touch on the responsibility of pain during rehab exercises, and how we can interpret productive pain from unhelpful pain.
Next, we discuss the 3 stages of rehab. Stage #1 includes how to settle down pain from a reactive flare-up, including the appropriate use of isometrics, daily load modifications and medications.
Stage #2 covers different levels of strengthening exercises. Brodie explains how the tendon can start 'out of compression' and slowly build on compressive loads throughout the rehab process. Certain exercises include hamstring curls, Swissball bridging, swissball rollouts, deadlifts, lunges & squats.
Stage 3 incorporate speed, power & addressing the whole kinetic chain with exercises like KB swings, rocket jumps, plyo lunges, wall balls & single leg deadlifts.
Lastly, Brodie touches on the psychosocial factors of your rehabilitation and how treatment and be influenced by beliefs, thoughts and emotions.

After learning the basics in episode #1, Brodie dives a level deeper into Proximal Hamstring Tendinopathy specific information.
We start by exploring the key anatomy features and the common hamstring tendon sites for pathology. We then dive into specific sports, tasks, movements and positions that elevate tensile and compressive strain on the hamstring tendon.
Brodie helps explain the walking/running action and the function of the hamstring during it's swing phase, contributing to high eccentric loads.
We then talk about proximal hamstring tendinopathy symptoms including deep, localized pain in the region of the ischial tuberosity that often worsens during or after running, lunging, squatting, and sitting.
We finish off with describing provocation tests & potential differential diagnoses such as lumbar, hip, sacroiliac joint, somatic referral, radiculopathy or sciatic nerve involvement.
