Evidence-based practice has a blind spot: not in the research itself, but in how we rank one form of knowledge above all others.

In this episode, I sit down with physiotherapist and PhD candidate Matt Low to unpack what he calls epistemic fluency, the ability to move between different forms of clinical knowledge depending on what the situation actually requires. We cover why population-based research gives you a map but not your patient's territory, how Aristotle's three forms of knowledge apply directly to clinical reasoning, and why the biopsychosocial model functions better as an analytical tool than a practice guide.

We also dig into the Back Cafe — a 3-arm RCT on lumbar spinal fusion rehab that compared a progressive training program, a video program, and a social cafe setting run by a senior physiotherapist. At the 2-year follow-up, the back-cafe group outperformed the training group on pain and beat both other groups on daily task performance. The study raises hard questions about what the active ingredient in rehabilitation actually is.

Lateral shifts are a common clinical presentation that can respond to specific treatment

Louis Gifford had public criticisms of the push to correct a lateral shift due to the thoughts it was an adaptive response early on.

Some still recommend these are dealt with immediately and there is little time to consider this adaptive and for it to be left alone.

You learned about them. You studied them. You were tested on them. But the real question is, how important are they to use?

Clinical prediction rules (CPRs) were developed using multivariate statistical methods to help clinicians select particular interventions to use depending on a cluster of findings a patient presented with.

Given the black and white matter of how the CPRs were created, this allowed for the opportunity to write a lot of test questions whether it was the NPTE or the OCS exam.

However, as many practicing clinicians know, the clinic is not black and white and we cannot always shoehorn patients into classifications or rules.

We don't necessarily understand their cause and have yet to find a great treatment that speeds up their natural history significantly.
