My colleagues and I started the Concussion Fix program when the pandemic first hit as a way of offering an extremely low-cost virtual concussion care service that would be accessible to people all around the world. After 2 years we have helped more than 2000 patients with long-standing post-concussion syndrome - sometimes as long as 7+ years - to fully recover from their concussions and get their lives back.

The idea for this conversation came from frequent requests from PCS patients for something that they could share with friends and family to help them be more supportive and understanding.

Melinda Krynen Hill is a Registered Psychotherapist (RP), trained mindfulness facilitator and Founder of Stillspace, a mindfulness consulting company that offers mindfulness- and compassion-based initiatives to enhance wellness. She has trained in a range of Mindfulness Based Interventions and her academic background includes a Master of Education in Counseling Psychology from the University of Toronto and a Master of Public Health in Health Promotion from the University of Alberta.

Melinda has particular expertise working with chronic health concerns including chronic pain, complex health conditions (e.g. rare diseases, autoimmune), post-concussive syndrome and associated mental health concerns such as health anxiety, depression and adjustment difficulties.

Sleep is so important for concussion recovery, yet very few people do it well. In this episode, I chat with a Neurologist and sleep expert, Dr. Adit Margaliot about the importance of sleep on concussion recovery. Doctor Margaliot is a Canadian-trained neurologist with a fellowship in neuromuscular disorders and further specialization in sleep medicine. She has practiced in both hospital and community settings. She now also practices Functional Medicine, an approach to treating individuals as a whole and addressing root causes of chronic illness. She views sleep as a very important component of an individual’s care and also recognizes the impact that other health issues have on sleep. Why do some people get better in a week while others take months to years?

PCS” is the acronym that was originally assigned to Post-Concussion Syndrome which was defined as having 3 or more concussion-related symptoms 1 to 3 months or more after a concussion injury. The term Post-Concussion Syndrome was replaced a few years ago with “Persistent Concussion Symptoms” and more recently by “Prolonged Concussion Symptoms”. In any case, the acronym remains: P.C.S. Anyone who has struggled with PCS or knows someone who has knows how difficult this condition can be. We are often told by healthcare professionals that there is no treatment for this condition other than TIME. Recent research over the past decade however has uncovered a lot of new information regarding the CAUSES and TREATMENTS of this complex condition. In part 1 of this two part series we break down the CAUSES of persistent concussion symptoms. In part 2 we will discuss the treatments.

Baseline testing is the concept of testing athletes on their mental and physical abilities prior to the season. The results of these tests can be valuable if an athlete sustains a concussion during the sports season as tests can be re-administered to determine the level of impairment and whether an athlete has fully recovered and is safe to return to their sport. We mostly hear from people who have had a concussion months to years ago who are still struggling with symptoms. They always tell us, I wish that I had come across this information earlier and I wouldn’t be in this situation right now.

The current scientific research demonstrates that between 30-40% of people who suffer a concussion will still be suffering symptoms 1 month later and many will still have symptoms at 6 months. The major reasons why some patients suffer from persistent symptoms and others don’t is becoming more widely known - THE GOOD NEWS is that once you know these, you can use them to your advantage!

Prior to age 25 males seem to suffer a greater number of concussions presumably due to greater participation in contact sports, however, after age 25 and every age group after, females suffer concussion injuries in greater numbers. Even in young athletes when we look at sex comparable sports where the rules are the same for both boys and girls (i.e., basketball, soccer, rugby, etc.) we see that girls tend to suffer concussions at a rate of 1.5x that of males. Not only that, women tend to also take longer to recover.

Visual dysfunctions are very common following concussion with more than 50% of patients demonstrating impairments in oculomotor function. In this episode 75 of Ask Concussion Doc, Dr. Cameron Marshall goes in depth with the biomechanics of a concussion. A concussion was often described as a bruising of the brain, but a new theory suggests that it's actually a stretching or a shearing of the brain causing an ion exchange. Once the brain cells have been stretched to a significant degree, it causes the ions inside and outside the pores on the membrane of each cell to exchange with one another therefore causing an excitation of the impacted brain cells. It then leads to the brain going through what is called an action potential and is what causes people to go unconscious or "see stars".

Dizziness and vertigo are common symptoms following a concussion.

Although these symptoms often resolve, they can become persistent and debilitating for some patients. The most frustrating part for patients is the lack of visible findings on examination or magnetic resonance imaging (MRI) to verify their symptoms.

For those that have never heard of vertigo, it is the sensation or feeling of being off balance. A patient often feels like they are spinning, or describe the world as spinning around them.

Injury or dysfunction of the neck has been shown to cause symptoms such as headaches, dizziness, loss of balance, nausea, visual and auditory disturbances and even reduced cognitive function. These are also the most commonly reported symptoms in concussion. In this episode, we discussed how to determine whether or not concussion symptoms are caused by whiplash or concussion, and if "asymptomatic" means that an athlete is ready to return to play. We also reviewed the study of the week, which looked at the prevalence of concussion injuries caused by fighting in ice hockey.

1) Can you explain neuro-fatigue? And, what exactly is happening in the brain when things get confusing or slowed way down in the brain?

2) I saw that CCMI has a 10-day return to learn/work/play protocol. Right now, we use a 5-day return to sport protocol, but that's obviously only for our athletes. We have to abide by the MDs orders regardless, but after that we implement the protocol accordingly. Do you feel like a 5-day protocol is adequate for our situation considering we cannot make educational accommodations for patients?

3) I have 2 elderly patients that all have dizziness and fatigue as their main symptom. Due to this population being super rigid into extension, how can I test for BPPV?
